What helps constipation on Mounjaro or Ozempic?
Fluids, slowly added fibre and movement come first, then laxatives, in a 2023 consensus and 2025 US advisory. In US label diabetes trials, 6% to 7% on Mounjaro (tirzepatide) and 3% to 5% on Ozempic (semaglutide) got constipated. On a dummy injection, 1% to 1.5% did. On semaglutide 2.4 mg (Wegovy's weight-loss dose), a typical bout lasted 47 days.
- GLP‑1 medicines such as Mounjaro and Ozempic are prescription-only in India, not sold over the counter.13
- A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.13 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.14

How common is constipation on Mounjaro or Ozempic, and how long does it last?
In US label diabetes trials, 6% to 7% on Mounjaro and 3% to 5% on Ozempic got constipated, against 1% to 1.5% on placebo.2,15
Placebo means a dummy injection. Ozempic's figures are for its 0.5 mg (5%) and 1 mg (3%) doses.15
Weight-loss doses are higher, and so are the figures. The Zepbound label (US, tirzepatide for weight) reports 11% to 17%, against 5% on placebo.1 In 3 trials of semaglutide 2.4 mg, the Wegovy dose, 24% got constipated, against 11%.3 A typical bout lasted 47 days, against 35 on placebo, and it stayed more common for all 68 weeks.3 We found no published bout length for tirzepatide.
In 7 Lucknow clinics, 12 of 71 people with diabetes on tirzepatide (17%) reported constipation.4 That study had no comparison group, and 6 authors declared fees from Eli Lilly.4
What does published advice say helps?
A 2023 expert consensus and a 2025 US nutrition advisory both start with enough fluids, fibre from food and more movement.5,6
These medicines may change how fast the gut moves, which could explain why constipation lasts.3 The consensus adds that feeling full makes people drink less water.5
For constipation, the advisory suggests adding fibre gradually, such as prunes or other dried fruit.6 Its tip to go easy on high-fibre food in the first days is for nausea, not constipation.6
For Indian meals, the ICMR-NIN 2024 dietary guidelines name whole wheat roti, millets, beans and lentils as rich in fibre.8 They say fibre "reduces constipation", though they were written for everyone, not GLP‑1 users.8 The NHS (UK) suggests a daily walk or run, a regular toilet time, not delaying when the urge comes, and resting your feet on a low stool.9 More on food is in Guide 8: Life on a GLP‑1.
When do laxatives come in?
When food, fluid and activity changes are not enough, the US advisory lists fibre supplements, stool softeners and other laxatives.6
The consensus says a doctor may lower the GLP‑1 dose if constipation gets worse.5 India's ESI 2025 guideline asks doctors to cut back gut-upsetting medicines such as iron, calcium and opioid painkillers.7
The NHS says laxatives should only be used for a short time.9 The advisory warns that some anti-nausea medicines can make constipation worse.6
The NHS lists reasons to see a GP, including constipation that is "not getting better with treatment", blood in your poo and weight loss without trying.9

When is constipation an emergency?
Constipation needs urgent care when it comes with stomach pain, bloating or vomiting, or you cannot pass stool or wind.10,12
These can be signs of a blocked bowel. The US Mounjaro label lists reports after approval of blocked bowel (ileus) and severe constipation with stuck stool (faecal impaction).2 The EU Ozempic leaflet calls bowel obstruction "a severe form of constipation" with stomach ache, bloating or vomiting.10 India's government lists bowel obstruction among the severe complications of GLP‑1 medicines.13
Diarrhoea after days of constipation is not always relief. The NHS says leaking diarrhoea is the main sign of stuck stool.9 The Mounjaro medication guide says to tell your doctor about stomach problems that are severe or will not go away.11
