Does metformin cause weight loss?
A little, on average. In the US Diabetes Prevention Program, adults with raised blood sugar but no diabetes lost 2.1 kg on metformin over 2.8 years. Those on a dummy pill lost 0.1 kg. Stomach upset was more common on metformin. Metformin's US label lists type 2 diabetes only, and we found no Indian approval for weight loss.
How much weight do people lose on metformin?
In a large US trial of adults without diabetes, people on metformin lost about 2 kg more than those on a dummy pill.
The Diabetes Prevention Program (DPP) included 3,234 US adults with raised blood sugar who did not have diabetes. Each got metformin 850 mg twice a day, a dummy pill (placebo) or a lifestyle programme.1 Their average BMI (a weight-for-height score) was 34.1 Over 2.8 years, the metformin group lost 2.1 kg, the placebo group 0.1 kg and the lifestyle group 5.6 kg.1
Over about 10 years of follow-up, the metformin group stayed 2.0% below their starting weight, against 0.2% on placebo.2 The most regular pill-takers kept off 3.5% (3.1 kg).2 At 2 years, 26% on metformin had lost 5% or more, against 14% on placebo.2
Does metformin cause weight loss in diabetes, or in Indians?
In type 2 diabetes the effect looks small, because the US label's trial of metformin against a dummy pill found almost no difference.
In that 29-week trial, people on metformin lost 1.4 lb (0.6 kg) and those on placebo 2.4 lb (1.1 kg).3 A second 29-week label trial switched people from glyburide (another diabetes pill) to metformin alone. They lost 8.4 lb (3.8 kg), against 0.7 lb on glyburide.3 The American Diabetes Association (2026) calls it "neutral (potential for modest loss)".4
India's IDPP-1 trial split 531 Indians with raised blood sugar after a glucose drink into four groups, two of them on metformin.5 Over 3 years, new diabetes was 55.0% in the control group and 40.5% on metformin alone.5 The summary we could open reports no weight results.5
- Stomach effects are common. Diarrhoea in 53% on metformin vs 12% on placebo in the US label trial.3 In the DPP they faded over time.2
- Low vitamin B12. About 7% fell below normal in 29-week trials.3 "Common" (1 in 10 to 1 in 100) on the EU label for Jentadueto, a metformin combination.6
- Lactic acidosis, an acid build-up in the blood. "Very rare" (under 1 in 10,000) on EU metformin leaflets,7 but it has caused deaths.3 Kidney or liver problems and heavy drinking raise the risk.3
- Low blood sugar (hypos) is likelier with insulin or a sulfonylurea (a diabetes pill that makes the body release more insulin).3
Is metformin approved for weight loss in India?
No. We found no Indian approval for weight loss, and the US label lists only blood-sugar control in type 2 diabetes.3
Metformin is missing from the table of approved weight medicines in India's ESI 2025 obesity guideline.8 In 2022, a CDSCO expert committee backed one firm's (USV) slow-release metformin tablets for delaying type 2 diabetes.9 The recommendation covers overweight, high-risk adults still heading towards diabetes after 3–6 months of intensive lifestyle change. It is not a weight-loss indication.9
For polycystic ovary syndrome (PCOS), a 2023 international guideline says metformin "should be considered" at BMI 25 or more, on very low-certainty evidence.10 More is in our PCOS weight-loss post. All the options sit side by side in Guide 3: Your options.
- GLP‑1 medicines are prescription-only in India, not sold over the counter.11
- A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.11 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.12

How does metformin compare with GLP-1 medicines for weight?
GLP-1 medicines gave far bigger average losses in their own trials, though those trials did not test them against metformin.
In STEP 1, semaglutide 2.4 mg (the Wegovy dose) cut weight by 14.9% vs 2.4% on placebo over 68 weeks.13 In SURMOUNT-1, tirzepatide 15 mg (the top Zepbound dose; Mounjaro is the same drug) cut 20.9% vs 3.1% over 72 weeks.14 Both trials left out people with diabetes. Stomach effects such as nausea were the most common side effects.13,14 India's ESI 2025 guideline also lists pancreatitis, gallstones and kidney injury among their warnings.8 The full trial picture is in How much weight do people lose on Ozempic or Mounjaro?
