Is Ozempic used for PCOS?
No. Ozempic (semaglutide) is not approved for PCOS (polycystic ovary syndrome) in India, the US or the EU. India approved it in September 2025 for adults with type 2 diabetes. US rules let doctors prescribe off-label; we found no Indian rule on this. The 2023 International PCOS Guideline says semaglutide "could be considered" for higher weight, alongside lifestyle changes.
- GLP‑1 medicines are prescription-only in India, not sold over the counter.7
- A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.7 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.8,9
- The risks are in Guide 5: Side effects and risks.
- Nothing here promises any result.
Is Ozempic approved for PCOS anywhere?
No. In India, the US and the EU, every approved use of Ozempic is for adults with type 2 diabetes, and none mentions PCOS.
India's drug regulator (CDSCO) cleared Ozempic in September 2025 for type 2 diabetes, with diet and exercise.1 Neither the US nor the EU label mentions PCOS.2,3 PCOS does not appear in the minutes of 2 recent CDSCO expert-committee meetings that discussed semaglutide.4,5
Using a medicine outside its label is called "off-label" use. The US regulator lets doctors do this when they judge it medically right.6 We found no Indian rule on off-label GLP‑1 use.
What does the 2023 International PCOS Guideline say?
The 2023 International PCOS Guideline says weight-loss medicines, naming semaglutide, liraglutide and orlistat, "could be considered" for higher weight in adults with PCOS.10
"Could" marks a weak (conditional) recommendation, used when evidence is limited.10 This one is a consensus view, because PCOS-specific evidence was insufficient.10 It recommends a healthy lifestyle for all women with PCOS. It suggests metformin for adults with a BMI of 25 or more, rating that evidence very low certainty.10
India's ESI 2025 obesity guideline doesn't mention PCOS. It says a doctor may consider medicines from a BMI over 27, or over 25 with a related condition.11 In India, Wegovy (higher-dose semaglutide), Mounjaro (tirzepatide) and some generic semaglutide brands carry approvals for weight management.12,13,14 See Wegovy vs Ozempic; Guide 4 covers who should not take them.
What do PCOS trials of semaglutide show?
The PCOS trials of semaglutide we found are small and short, and report more weight loss and more regular periods but no long-term safety data.
In a 2025 Chinese trial, 100 women took metformin alone or with semaglutide, the medicine in Ozempic. Semaglutide rose to 1 mg a week over 16 weeks.15 1 mg is Ozempic's top strength in India.1 Semaglutide lowers blood sugar, slightly slows stomach emptying and reduces appetite (EU label).3 Of the 80 who finished, weight fell 6.1 kg vs 2.3 kg, and periods became regular in 73% vs 42%.15
Stomach side effects were more common with semaglutide. It ran at one hospital with 40 finishers per group, and everyone knew their treatment (open-label).15 A 2026 Chinese trial of 64 women agreed.16 A 2026 review of 11 trials of other GLP‑1 medicines in PCOS rated the weight benefit modest and low certainty.17 Our longer post is Weight loss for PCOS: what do GLP‑1 medicines and metformin show?.
- Feeling sick (nausea) in 20% on Ozempic 1 mg vs 6% on a dummy injection, in the US label's diabetes trials.2
- Inflamed pancreas (pancreatitis) is "uncommon" on the EU Ozempic label (1 in 100 to 1 in 1,000 people).3
- Gallstones are "common" on the EU Ozempic label (1 in 10 to 1 in 100 people).3
- Thyroid tumour warning: not for anyone with medullary thyroid cancer or multiple endocrine neoplasia type 2 (MEN 2) in themselves or their family (US label).2
Why do contraception and pregnancy plans matter on Ozempic?
The PCOS guideline says women taking GLP‑1 medicines should use effective contraception when pregnancy is possible, because pregnancy safety data are lacking.10
The US Ozempic label says to stop at least 2 months before a planned pregnancy. Semaglutide leaves the body slowly.2 India's ESI guideline says contraception should continue for 3 months after stopping.11 The EU label says Ozempic is not expected to weaken the combined pill. Its effect on human fertility is unknown.3
In the 2025 trial, after semaglutide stopped, 35% vs 15% of finishers conceived naturally in weeks 16 to 40. Total pregnancies, counting fertility treatment, did not differ clearly. One small open-label trial promises nothing about fertility.15 Read more in Mounjaro, Ozempic, pregnancy and the pill.
