Ozempic or Mounjaro at Diwali: what changes?

What labels, India's endocrinologists and health bodies say about festive food, low sugar, alcohol, dose days and pen storage.

Written for India. Reading from the US? US versionComing soon

The short answer

Diwali falls on Sunday 8 November 2026. GLP-1 medicines slow digestion, so large or fatty meals can bring on nausea or vomiting. With insulin or a sulfonylurea, late or missed meals and alcohol raise the risk of low blood sugar. Pens in use tolerate room temperature only up to 30 °C.

Key takeaways

  • Diwali falls on Sunday 8 November 2026, and GLP-1 medicines slow how fast the stomach empties, so large, rich meals can bring on nausea.1,2
  • A short letter from Jodhpur doctors reported that, in 67 adults with type 2 diabetes, average fasting blood sugar rose from 137 to 154 mg/dL across Diwali.9
  • The US Ozempic and Mounjaro labels warn that taking them with insulin or a sulfonylurea raises the risk of low blood sugar, including severe episodes.4,5
  • India's ESI 2025 guideline lists avoiding alcohol among its tips for stomach side effects, and the NHS says alcohol can make nausea worse.3,7
  • Under its US label, an Ozempic pen in use keeps for 56 days at 15–30 °C or in a fridge, but not above 30 °C.4

When is Diwali in 2026, and what changes on a GLP-1?

Diwali falls on Sunday 8 November 2026. Festive food tends to mean more sweets and fried dishes, and these medicines change how your stomach handles both.

India Post's 2026 holiday list gives Diwali (Deepavali) as Sunday 8 November.1 A short 2015 letter by Jodhpur doctors describes the festive season as a time of "sweets and fat-rich fried foods".9

Before you go further
  • GLP-1 medicines are prescription-only in India.16
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.16 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.17,18
  • Risks are covered in Guide 5: Side effects and risks.
  • Nothing here changes your dose. Your doctor sets it.

This post uses the US labels for Ozempic and Wegovy (semaglutide) and Mounjaro (tirzepatide).4,5,6 We found no public Indian label for them. The food advice comes from India's endocrinology guideline (ESI 2025), a 2025 US nutrition advisory, the NHS and Diabetes UK.2,3,7,15

Why can rich, fried or sweet food sit badly on a GLP-1?

These medicines slow how fast your stomach empties. A big, fatty meal stays there longer, which can bring bloating, fullness and nausea.

All three US labels say the medicine "delays gastric emptying", meaning food leaves the stomach more slowly.4,5,6 That delay can bring bloating, fullness and nausea, and fatty or high-protein food slows emptying further.2

"Vomiting is more likely to occur with large meals," the advisory says.2 Stomach side effects are also more likely when you start the medicine or move up a dose.2 The US Mounjaro label says most nausea, vomiting and diarrhoea came during dose increases and eased over time.5

In US diabetes trials, 12–18% of people on Mounjaro had nausea, against 4% on a dummy injection (placebo).5 On Ozempic it was 16–20%, against 6%.4

A smaller appetite at the Diwali table is part of how these medicines work. The NHS says semaglutide reduces appetite and helps you feel fuller for longer.7 In the advisory authors' clinics, many users feel less pull towards sugary and fatty foods. Nausea, though, can trigger cravings for sugary comfort foods.2

Vomiting or diarrhoea can dehydrate you, and the US Ozempic medication guide warns this may cause kidney problems.4 Our posts on Ozempic side effects and Mounjaro side effects cover the full list.

RisksThe risks, in brief
  • Stomach effects are common. In US diabetes trials, 16–20% on Ozempic had nausea, against 6% on placebo.4 At Wegovy's 2.4 mg weight-loss dose, higher than any Ozempic dose, it was 44% against 16%.4,6
  • Pancreatitis (an inflamed pancreas) shows as severe or lasting stomach pain, sometimes reaching the back.6
  • Gallstones were reported by 1.5% on Ozempic 0.5 mg and 0.4% on 1 mg (US label). No one on placebo had them.4
  • Dehydration from vomiting or diarrhoea can harm the kidneys.6
  • Thyroid warning: ruled out by the US label for anyone with a personal or family history of medullary thyroid cancer (MTC) or MEN 2; tell your doctor if this applies. Not in pregnancy: it may harm the baby.6

Get help now: call 112 for severe stomach pain that won't go away, sometimes reaching the back. It can be pancreatitis.6 The US Ozempic guide also says to tell a doctor right away about vomiting or diarrhoea that won't stop.4

What do the sources say about Diwali dishes?

Fat slows stomach emptying further on these medicines, a US advisory says. Diabetes UK's Diwali page suggests lighter vegetarian versions of festive dishes.

Diabetes UK's Diwali page, last reviewed 21 September 2026, picks vegetarian recipes for the festival.15 They include baked mini spinach and onion bhajis, vegetable samosas made without frying, and vermicelli kheer.15 Channa dahl, saag aloo and a cauliflower, paneer and chickpea curry are on its list too.15 The points below come from that page unless another source is named.

  • Samosas and bhajis. The Jodhpur letter calls festive fried foods "fat-rich".9 The US advisory says fat slows stomach emptying further.2 The page's versions are baked or made without frying.15
  • Kheer and other puddings. For sweet treats, the tip is less ghee, or semi-skimmed milk in place of whole milk.15 The stated aim is fewer calories and less saturated fat.15
  • Dhal, chickpeas, beans and wholegrain basmati rice. The page calls these slowly absorbed, high-fibre foods that keep blood sugar steadier, with an eye on portion size.15 ESI's tips for stomach side effects list avoiding high-fibre meals, so the two pull in different directions.3
  • Cooking swaps. Vegetable oil replaces ghee, and wholemeal flour goes into chapatti or thepla.15 Curries get extra bhindi or aubergine.15
  • A street-food spread. Spicy soya sticks, chana masala chaat, oat tikki and rajma tikki come with salads alongside.15

At a Diwali party: what the sources say

  • Small, frequent meals. India's ESI 2025 guideline lists this first among its tips for stomach side effects.3
  • Not arriving very hungry. The US advisory lists becoming "overly hungry" and large meals among habits to limit.2
  • Treats, in small portions. The same advisory encourages "portion-controlled treats" and "flexibility with food choices".2
  • Little or no alcohol. ESI lists avoiding alcohol; the advisory calls for "minimal alcohol intake".3,2
  • On a sulfonylurea or insulin. NHS advice is not to skip or delay meals, and not to drink alcohol without eating.12
  • Sugar-free drinks. Diabetes UK suggests staying hydrated with sugar-free drinks, and checking sugar more often if you already monitor it.15
  • If nausea comes. ESI suggests ginger candy, cloves, fennel seeds (saunf), or small amounts of buttermilk or kokum juice.3

What do festive sweets do to blood sugar?

For people with type 2 diabetes, more sweets usually means higher blood sugar. A short letter from Jodhpur doctors reported a clear rise across Diwali.

The doctors wrote a one-page letter to a journal. They checked 67 adults with type 2 diabetes within 2 weeks before and after Diwali 2011.9 All took diabetes tablets, and people on insulin were left out.9 The paper does not mention GLP-1 medicines.

Blood sugar before and after Diwali
67 adults with type 2 diabetes, Jodhpur, 2011. Averages, mg/dL.

"After a meal" = 2 hours after eating. Source: Mathur et al., letter to the editor, Int J Diabetes Dev Ctries (online 2015).9 One hospital; the same people measured before and after; no comparison group.

Average fasting sugar rose from 137 to 154 mg/dL.9 Sugar 2 hours after a meal rose from 185 to 207 mg/dL.9 Total cholesterol and triglycerides rose too.9

Diabetes UK's Diwali page says eating more sweet foods than usual can raise blood sugar.15 Total carbohydrate has the biggest effect, it adds.15

Semaglutide works in a glucose-dependent way: when blood sugar is high, it releases more insulin, the US Ozempic label explains.4 A 2015 Indian endocrinology review contrasts this with sulfonylureas and insulin.10 These act whatever the sugar level, so they are more prone to cause low sugar.10 The type 2 diabetes path brings the diabetes pages together.

Can late or missed meals cause low blood sugar?

Yes, if you also take insulin or a sulfonylurea such as glimepiride. US labels warn that the combination can cause severe low blood sugar.

The Ozempic and Mounjaro US labels say this pairing raises the risk of low blood sugar (hypoglycaemia), "including severe hypoglycemia".4,5 Both say lowering the sulfonylurea or insulin dose may reduce that risk.4,5

The US glimepiride label says low sugar is more likely "when caloric intake is deficient" or after alcohol.11 NHS advice on reducing the risk includes not skipping or delaying meals, and not drinking alcohol without eating.12 Diabetes UK lists the same two points.13

The US Ozempic medication guide lists signs of low sugar. They include dizziness, sweating, shakiness, hunger, confusion and a fast heartbeat.4 Our post on low blood sugar on Ozempic or Mounjaro covers this in depth. Fasting for Karva Chauth or Navratri before Diwali has its own post: fasting on a GLP-1.

Get help now: call 112 if someone with very low sugar is unconscious or not responding normally, and one of three things applies.12 First, no glucagon injection is available, or no one knows how to use it.12 Second, glucagon was given and they haven't recovered within 10 minutes.12 Third, they have been drinking alcohol.12 Don't give an unconscious person food or drink; they cannot swallow safely.12

What about alcohol at Diwali parties?

No US label bans it, but India's ESI guideline and the NHS advise avoiding alcohol. The NHS says it can make nausea and vomiting worse.

ESI 2025 lists "avoid alcohol" among its tips for stomach side effects.3 The NHS gives the same advice for semaglutide and tirzepatide, because alcohol can increase feeling or being sick.7,8 The US advisory adds that alcohol may worsen reflux, and calls for minimal intake.2

For people on a sulfonylurea, the glimepiride label says low sugar is more likely after drinking.11 Diabetes UK's Diwali page warns that alcohol can make low blood sugar more likely.15 Our Answer on drinking alcohol on Mounjaro or Ozempic covers the label wording and the trials.

What if your injection day falls on Diwali?

The injection can go in at any time of day. Both NHS pages also allow moving your injection day, if at least 3 days have passed since the last one.

The NHS says semaglutide and tirzepatide injections can be given at any time of day.7,8 Both pages allow changing the day if at least 3 days have passed since the last injection.7,8 The US labels set the gap at 2 days for Ozempic and 3 days for Mounjaro.4,5

The US labels allow a missed dose within 5 days on Ozempic. On Mounjaro the window is 4 days.4,5 After that, the missed dose is skipped and the next one goes on the usual day.5,7 The NHS says not to give 2 injections at once to make up for a missed one.7,8

We found no wording in the US labels or NHS pages about pausing a dose for a festival. The NHS semaglutide page says: "If you miss more than one injection, contact your doctor for advice."7 After 2 or more missed doses in a row, the US Wegovy label restarts the dose at a lower level.6 Its stated reason is to lower the risk of stomach side effects.6 Our Answer on missed doses has the details per brand.

How do you keep your pen safe when travelling?

Unopened pens belong in a fridge at 2–8 °C. Once in use, the US labels allow some time at room temperature, but never above 30 °C.

  • Ozempic pen, in use: 56 days at 15–30 °C or in a fridge (US label).4
  • Wegovy FlexTouch pen, in use: 56 days at 15–30 °C or in a fridge (US label).6
  • Mounjaro KwikPen, once opened: in a fridge or at up to 30 °C. It is thrown away after 30 days or 4 weekly doses, whichever comes first (US label).5
  • India's Mounjaro pen is the KwikPen, launched here in August 2025; Lilly first sold Mounjaro in India as vials, from March 2025.19 The US label's single-dose pens and vials get 21 days in total outside the fridge, up to 30 °C.5

The Ozempic and Mounjaro labels say never to use a pen that has frozen.4,5 All three labels say to protect it from heat and light.4,5,6 The NHS notes that storage rules can differ by brand and pen. Your own pack leaflet is the one that applies.7

Diabetes UK's travel advice for insulin, another fridge medicine, says to carry it in hand luggage.14 The hold can get cold enough to damage it, and bags can be lost.14 It suggests a cool bag, but warns the medicine shouldn't touch frozen ice blocks.14 Some airlines ask for a doctor's letter, it adds.14

Coming next: pens in Indian heat and flying with a pen. Guide 8 has the storage table now.

Questions people ask

Can I eat sweets on Ozempic or Mounjaro?

The guidelines we read don't ban sweets. India's ESI 2025 guideline advises avoiding high-fat and high-carbohydrate meals for stomach side effects. A US nutrition advisory encourages "portion-controlled treats".3,2

Can I skip my injection for Diwali?

We found no guidance on pausing a dose for an event in the US labels or NHS pages. The NHS semaglutide page says: "If you miss more than one injection, contact your doctor for advice." After 2 or more missed doses in a row, the US Wegovy label restarts the dose at a lower level.7,6

Do festive sweets cause low blood sugar?

Sweets raise blood sugar.15 On a GLP-1, low sugar is linked to insulin or a sulfonylurea taken alongside, plus missed meals or alcohol.4,11

Why do I feel full so quickly at festive meals?

These medicines slow how fast food leaves the stomach and reduce appetite. A 2025 US advisory links the slower emptying to bloating, fullness and nausea.4,7,2

Is Mounjaro different from Ozempic at Diwali?

Both US labels carry the same warnings on slower stomach emptying and on low sugar with insulin or sulfonylureas. The missed-dose windows differ: 5 days for Ozempic and 4 days for Mounjaro.4,5

How we researched this

On 5 October 2026 we checked the Diwali date on India Post's 2026 holiday list. We read the latest US labels for Ozempic, Wegovy, Mounjaro and glimepiride. We also read India's ESI 2025 obesity guideline and a 2025 US nutrition advisory.

We searched PubMed and the web for studies of blood sugar during Diwali and found one short letter, from Jodhpur. We found no study of people on GLP-1 medicines during Diwali or any Indian festival.

We looked for research linking firecracker smoke to these medicines. We found none, so this post leaves it out.

We also looked for a health source that names specific Diwali mithai. We found only recipe pages and sites selling health products or programmes, so this post names only the dishes on Diabetes UK's list.

Before you move on

  • GLP-1 medicines slow stomach emptying, so large, fatty meals are the ones most linked to nausea and vomiting.
  • With insulin or a sulfonylurea, late or missed meals and alcohol raise the risk of low blood sugar.
  • Pens in use tolerate room temperature only up to 30 °C; your own pack leaflet sets the rule.
  • Severe stomach pain that won't go away, or someone unconscious with low sugar: call 112.
