What does a year on Ozempic, Mounjaro or Wegovy cost in India, all in?

Twelve months at MRP, following each label's dose steps, with doctor visits and blood tests added. Grouped by medicine, never ranked by price.

The short answer

A first year at MRP, with visits and blood tests, came to ₹1,15,680–1,27,780 on Ozempic in our examples. It was ₹1,65,380–1,99,330 on Wegovy and ₹37,320–1,20,120 on generic semaglutide pens, by dose and maker. On Mounjaro (tirzepatide) it was ₹2,22,817–3,20,030. The dose your doctor settles on sets the figure, and insurance rarely pays.

Key takeaways

  • At MRP, a first year on Ozempic, Wegovy or Mounjaro, with doctor visits and blood tests, came to ₹1,15,680–3,20,030 in our examples, depending on medicine and dose.3,4,5,6
  • On Ozempic, Wegovy and Mounjaro the medicine was 89–95% of each year's bill, and 63–87% on generic pens; visits and three rounds of tests added ₹12,820–17,220.19,20
  • Generic semaglutide pens launched at ₹1,800–8,000 for four doses in March 2026, so 13 pens come to ₹23,400–1,04,000 before visits and tests.10,11,12
  • The second year on a steady dose costs about the same as the first, or more at higher doses: on Wegovy 2.4 mg, ₹1,99,330 becomes ₹2,22,480.3,5
  • Standard health insurance in India pays out only when you are admitted to hospital, so a monthly pen bought outside hospital is usually not covered.25
Before you go further
  • GLP‑1 medicines are prescription-only in India, not sold over the counter.1
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.1 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.2
  • A doctor decides whether a medicine fits, which one and at what dose. The examples below are sums, not suggestions.
  • The risks are the same whatever the price: Guide 5.

How do we add up a year?

We count 52 weekly doses at the printed MRP, follow each label's dose steps, then add doctor visits and blood tests.

MRP (maximum retail price) is the price printed on the pack. Sellers may charge less, but not more; we never use discount prices.5

Ozempic, Wegovy and Mounjaro are weekly injections. Each MRP we use covers 4 weekly doses. That is one Mounjaro KwikPen, or one Ozempic or Wegovy pen at four times the maker's weekly price.4,5,6 A year is 52 doses, or 13 pens. A month is about 4.3 doses, so 12 "monthly" pens fall 4 doses short.

The dose climbs in steps, and the price climbs with it. Mounjaro goes up 2.5 mg at a time, after at least 4 weeks on each dose. It stops at 15 mg.17

Your doctor may move more slowly, or stop lower. We used the US labels for the steps, because we could not find the Indian labels online. Our Mounjaro doses post and our Ozempic dose steps answer explain each ladder.

What does a year cost on each medicine and dose path?

From ₹1,15,680 for Ozempic held at 0.5 mg to ₹3,20,030 for Mounjaro climbing to 15 mg, all in.

Grouped by medicine (semaglutide, then tirzepatide), not sorted by price. MRPs checked 5 Oct 2026; visits at ₹1,100 and tests at ₹2,440 a round. Our arithmetic from sources 3–7, 10–12, 19 and 20. The medicine is 89–95% of each branded total and 63–87% of the generic one. Generic visits follow a stepped path; the calculator assumes one generic dose all year (4 visits), giving ₹35,120–1,15,720.

Ozempic (semaglutide, for type 2 diabetes). The label gives 0.25 mg for 4 weeks, then 0.5 mg. It allows 1 mg after at least 4 weeks on 0.5 mg if blood sugar needs more control.15 At MRP, pens cost ₹5,660, ₹8,100 and ₹9,100.3,4,5 So the 1 mg year is ₹5,660 + ₹8,100 + 11 pens × ₹9,100 = ₹1,13,860.

Wegovy (semaglutide, for weight). The label steps through 0.25, 0.5, 1 and 1.7 mg, 4 weeks each. The dose to stay on is 1.7 or 2.4 mg.16 Pens cost ₹5,660, ₹8,100, ₹9,100, ₹12,750 and ₹16,400.3,4,5 The first 16 weeks cost ₹35,610. Then 9 pens at 2.4 mg add ₹1,47,600, for ₹1,83,210.

Ozempic and Wegovy have the same MRP at 0.25, 0.5 and 1 mg.4,5 Wegovy's year costs more only because its label goes on to higher doses. Our Wegovy vs Ozempic post explains why it's one medicine under two names.

Poviztra, a second brand of Novo Nordisk's semaglutide sold by Emcure, costs ₹3,999 a month at 0.25 mg and ₹8,999 at 2.4 mg.9 Its middle doses have no published price, so we could not add up its year.

Mounjaro (tirzepatide). KwikPen MRPs are ₹13,125 at 2.5 mg and ₹16,406 at 5 mg. They are ₹20,625 at 7.5 or 10 mg, and ₹25,781 at 12.5 or 15 mg.6 Climbing to 15 mg means one pen at each of five steps (₹96,562), then 8 pens at 15 mg (₹2,06,248).

Mounjaro's single-dose vials cost ₹3,281 (2.5 mg) and ₹4,102 (5 mg), the same as a pen per dose. Pharmacies list only those two strengths.6 Yurpeak, the same tirzepatide sold by Cipla, has no published MRP. Cipla says "the price will be the same as Mounjaro".8

What about generic semaglutide pens and Natco's vials?

Generic pens launched in March 2026 at ₹1,800–4,200 for four starting doses and ₹4,200–8,000 at the top doses makers published.10,11,12

Alkem published ₹1,800 for a pen of four doses.10 Dr Reddy's priced both its pens at ₹4,200 a month, at least 4 weekly doses each.11 Sun Pharma's weight pen runs from about ₹900 a week at the first dose to ₹2,000 at 2.4 mg. That is ₹3,600–8,000 for four doses.12 We list these as published ranges, not as picks.

Most makers published one launch price, mostly for a starting dose, so we bracket the year.

  • 13 pens at starting-dose prices: ₹23,400–54,600.10,11
  • 13 pens at top-dose prices: ₹54,600–1,04,000.11,12
  • A year that climbs the steps lands between the two. With the Wegovy-style steps, 36 of 52 doses are at the top dose, so it sits nearer the second line.16

Natco's vials are priced another way. BusinessToday reported ₹1,290 a month for lower strengths and ₹1,750 for the highest.13 The report doesn't say how many doses a vial holds, so we can't count 52 doses. Twelve such months come to ₹15,480–21,000 for the medicine.

A vial means drawing each dose up with a syringe yourself. Guide 6 explains why that is where dosing mistakes happen.

We found no newer maker or dated news price by 5 October 2026. Medical Dialogues put generic monthly therapy at "around Rs 1,290 to Rs 8,000" on 10 July.14 The same report covers the July 2026 recalls of some generic batches.14 Our semaglutide generics post covers what changed after the patent expired.

What do doctor visits and blood tests add?

About ₹12,820–17,220 in the first year, at a ₹1,100 endocrinologist fee and ₹2,440 for each round of blood tests.19,20

Visits. We counted one first visit and one at each dose step on the label. Then we added a review at 6 months.15,16,17 UK guidance says to consider stopping semaglutide if less than 5% of starting weight is lost by then.18 Reviews at 9 and 12 months are our assumption.

₹1,100 is the median in-clinic fee across 59 endocrinologists on Practo in 6 cities, checked 5 October 2026.19 Fees ran from ₹300 to ₹3,500. City medians ran from ₹850 in Hyderabad to ₹2,000 in Mumbai.19

Blood tests. We priced six common tests: fasting sugar, HbA1c, creatinine (kidneys), cholesterol (lipid profile), liver tests and TSH (thyroid). They come from the Endocrine Society of India's list, which says not every test is needed for everyone.21

One round cost ₹1,604–2,850 on the websites of 7 private lab chains. Dr Lal PathLabs charged ₹2,660, Thyrocare ₹2,390 and Metropolis ₹2,850.20 We used the middle price, ₹2,440. A full kidney panel instead of creatinine alone can add up to ₹980.20

We assumed a round at the first visit and at 6 and 12 months. Home collection, travel and other medicines are left out.

Is the second year cheaper?

Not by much. On lower doses, year two came within ₹300 of year one. On top doses it cost more, since every pen is full dose.3,5,6

In year two we counted 13 pens at the same dose, 4 reviews and 2 rounds of tests (₹9,280).

₹1,27,580Ozempic 1 mg, year two (₹1,27,780 in year one)3,4
₹2,22,480Wegovy 2.4 mg, year two (₹1,99,330 in year one)3,5
₹2,22,558Mounjaro 5 mg, year two (₹2,22,817 in year one)6
₹3,44,433Mounjaro 15 mg, year two (₹3,20,030 in year one)6

Guide 9 covers what happens to weight and blood sugar when people stop, and why it makes sense to budget in years.

RisksThe risks, in brief: the same at any price
  • Stomach effects. In the Wegovy 2.4 mg trials, 44% felt sick (nausea), against 16% on a dummy injection. They show up mostly as the dose goes up.16
  • Pancreatitis (an inflamed pancreas): severe stomach pain that won't go away, sometimes spreading to the back.16
  • Gallbladder problems, such as gallstones.16
  • Dehydration from vomiting or diarrhoea can harm the kidneys.16
  • Thyroid warning: ruled out by the US labels for people with a personal or family history of MTC or MEN 2; tell your doctor if this applies. Not in pregnancy.16,17

Where does GST come into the price?

GST is already inside the MRP. It fell from 12% to 5% on most medicines on 22 September 2025, and printed MRPs came down with it.22,23

The GST Council cut the rate on "all other drugs and medicines from 12% to 5%".22 The price regulator, NPPA, then told makers to issue price lists showing the revised GST and revised MRP.23

You can see it in Mounjaro. Its August 2025 launch price was ₹14,000 at 2.5 mg and ₹27,500 at 15 mg.7 ₹14,000 × 105/112 = ₹13,125, and ₹27,500 × 105/112 = ₹25,781, the MRPs listed now.6 That is our arithmetic; Lilly has not published the new figures.

A doctor's consultation is a health care service, and those are exempt from GST.24 So the ₹1,100 visit carries no GST on top.

Will health insurance pay for any of it?

Usually not, because standard health insurance in India pays out only when you are admitted to hospital, and pens are outpatient costs.25

Those bills are yours to pay unless your policy has an OPD (outpatient) or wellness benefit.25 Some plans with OPD cover include test costs.26

The standard exclusion for weight loss covers surgery, with exceptions such as a BMI over 40.26 We found no rule that requires insurers to pay for GLP‑1 medicines. Ask your insurer in writing before you count on it.

We go through policy wording, OPD add-ons and claims in Does health insurance cover GLP-1 medicines in India?

Questions people ask

How much does Mounjaro cost per month in India?

One KwikPen holds 4 weekly doses. Its MRP is ₹13,125 at 2.5 mg, rising to ₹25,781 at 12.5 and 15 mg. A month is about 4.3 doses, so a year takes 13 pens.6

Do Ozempic and Wegovy cost the same?

At 0.25, 0.5 and 1 mg, yes: ₹5,660, ₹8,100 and ₹9,100 a pen. Wegovy's label goes on to 1.7 and 2.4 mg, at ₹12,750 and ₹16,400.4,5,16

Why do the first months cost less than later ones?

The first pen is the lowest dose, and each step up costs more. Mounjaro's 2.5 mg pen is ₹13,125; its 15 mg pen is ₹25,781.6,17

Is GST added on top of the MRP?

No. GST is already inside the MRP. When GST on most medicines fell to 5% in September 2025, NPPA told makers to issue revised MRPs.22,23

Does insurance cover Ozempic, Wegovy or Mounjaro?

Usually not. Standard policies pay out when you are admitted to hospital. An OPD add-on may cover some outpatient costs, so check your policy wording.25,26

How we researched this

We checked every MRP on 5 October 2026. Prices come from the maker or a named, dated news report. As a last resort, we used the MRP on two or more pharmacy listings that agree. Those listings are named in plain text, without links. We used no discount prices.

Dose steps come from the US labels, because we could not find the Indian labels online. Visit and test counts follow Guide 6 and the cost calculator, except the generic row (see the table note). Business Standard's reports blocked our checks, so we cited Outlook Business and Reuters reports of the same prices instead.

We found no named, dated hospital package price for a GLP‑1 work-up, so we left hospital packages out.

Before you move on

  • Price the whole dose ladder, not the first pen.
  • Count 13 pens a year, not 12.
  • Add your own doctor's fee and your lab's price for each round of tests.
  • Budget for year two at the full dose.
  • Check your policy for OPD cover before assuming insurance will help.
