  • GLP-1 medicines are prescription-only. A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.9 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.10,11
  • They have serious risks as well as common side effects: Guide 5: Side effects and risks.
  • This page describes what published guidance says. It is not a diet plan or a calorie target for you.
The short answer

There is no official Ozempic diet. Indian and US guidance describes smaller, more frequent meals, with protein first: dal, paneer, curd, soy, eggs, fish or chicken. It describes adding fibre slowly. ICMR gives about 2 litres of fluid a day for healthy adults. Large, oily or starchy meals, fizzy drinks and alcohol are linked to worse side effects.

Key takeaways

  • No guideline sets a special Ozempic or Mounjaro diet; Indian and US guidance describe smaller meals, protein first, fibre added slowly and enough fluids.1,2,4
  • ICMR's plate for Indians fills about half the day's food with vegetables, fruits and greens, and asks for at least half of cereals as whole grains.3
  • ICMR's protein allowance for healthy Indian adults is 0.83 g per kg a day, and a 2025 US advisory notes 1.2–1.6 g has been proposed during weight loss.2,3
  • India's 2025 endocrinology guideline (ESI) advises small, frequent meals and avoiding high-fat, high-carbohydrate and high-fibre meals, fizzy drinks and alcohol to ease stomach side effects.1
  • ICMR advises healthy adults to drink about 2 litres a day including other drinks, and the US Wegovy label warns that losing fluids can harm the kidneys.3,6

Is there an official Ozempic diet?

No. No guideline we found sets a special Ozempic or Mounjaro diet. Their US labels pair them with diet and exercise.

Ozempic's US label says it is used "as an adjunct to diet and exercise" to control blood sugar in adults with type 2 diabetes.12 Mounjaro's US label uses the same words.7 Wegovy is the same medicine as Ozempic (semaglutide) at a higher dose for weight loss: 2.4 mg a week, against Ozempic's US maximum of 2 mg.6,12 Its US label pairs it with "a reduced calorie diet and increased physical activity".6 India's endocrinologists say these medicines do not remove the need for diet changes and exercise.1

What changes is how much people eat. An Indian expert consensus reports that calorie intake falls by 16% to 39% on these medicines.4 So a 2025 US advisory says the aim is enough nutrients from a much smaller amount of food.2 If GLP-1 is a new term, what GLP-1 stands for explains it.

Who wrote the guidance we use. Four sources do most of the work here:

  • ESI 2025: the Endocrine Society of India's obesity guideline. It lists Indian foods for side effects.1
  • ICMR-NIN 2024: India's official dietary guidelines, with the "My Plate for the Day".3
  • The 2025 US advisory: four US nutrition and obesity societies. NIH and Duke funded it. Five authors declare advisory or speaking roles for Novo Nordisk or Eli Lilly.2
  • The 2026 Indian consensus: 13 Indian diabetes and hormone specialists voted on statements.4 Abbott Nutrition funded it, and two authors work for Abbott.4 It also recommends diabetes nutrition formulas as meal replacements, so we name the funder.4
RisksThe risks, in brief
  • Stomach effects are common. Nausea, diarrhoea, vomiting, constipation and stomach pain lead the labels' lists.6,7
  • Pancreatitis (an inflamed pancreas) is a label warning. The sign is severe stomach pain that won't go away.6,7
  • Gallstones were reported by 1.6% on Wegovy vs 0.7% on a dummy injection.6
  • Low blood sugar is more likely when taken with insulin or a sulfonylurea, a type of diabetes tablet.6,7
  • Dehydration from vomiting or diarrhoea can injure the kidneys.6,7
  • Thyroid warning. Ruled out by the US labels for people with a personal or family history of MTC, a thyroid cancer, or MEN 2; tell your doctor if this applies.6,7 The Wegovy label says to stop at least 2 months before a planned pregnancy.6

What does a balanced Indian plate look like?

ICMR's "My Plate for the Day" fills about half the day's food with vegetables, fruits, greens and roots. Cereals, pulses, milk, nuts and oils make up the rest.

The plate draws on at least 10 food groups.3 ICMR asks for at least half the cereals to be whole grains, such as millets.3 ICMR prefers whole fruit to juice.3 The table is ICMR's reference day for a 2,000 kcal diet. We show it for the mix of food groups, not as a plan or calorie target for anyone on these medicines.

Most Indians eat a different mix. Cereals give 50% to 70% of daily energy in practice, against ICMR's limit of 45%.3 Pulses, meat, poultry and fish give 6% to 9%, against the 14% ICMR advises.3 On a GLP-1, the Indian consensus favours whole grains, millets and legumes, and restricting refined carbohydrates.4 The US advisory also lists refined grains, flours and added sugars as foods to limit.2

What do the guidelines suggest for vegetarians?

Guidelines put protein first: dal, beans, paneer, curd, milk or soy. ICMR says cereals and pulses eaten together give better-quality protein.

The US advisory suggests eating protein-rich foods first, so you finish them before you feel full.2 It encourages beans, peas, lentils, whole grains and dairy.2 The Indian consensus names pulses, milk, curd, paneer and soy products.4

Plant proteins on their own lack some building blocks (essential amino acids), ICMR says.3 Cereals, millets and pulses together supply most of them.3 For weight loss, ICMR suggests cereals and pulses in a ratio of 3:1 or 5:1.3 Dal with roti or rice is that pairing.

  • Moong dal, 30 g raw: about 7 g protein.5
  • Toor (arhar) dal, 30 g raw: about 7 g.5
  • Soybean, 30 g dry: about 11 g.5
  • Peanuts, 30 g: about 7 g.5
  • Paneer, 100 g: about 19 g.5

Our arithmetic from India's food composition tables (IFCT 2017), rounded.5 Curd is not in those tables, so we left it out.

How much protein. ICMR's allowance for healthy Indian adults is 0.83 g per kg of body weight a day.3 The US advisory notes 1.2–1.6 g per kg has been proposed during weight loss.2 For a 70 kg adult, that is 58 g vs 84–112 g a day. This is our arithmetic, not a target. A doctor or dietitian sets one. Protein amounts get their own post: Protein on a GLP-1, and where Indians get it.

Protein powders and shakes. The US advisory says some people meet protein targets with high-protein shakes, bars or fortified foods.2 The Indian consensus panel agreed that nutrition should not lean on one nutrient, such as protein supplements alone.4 It suggests diabetes nutrition formulas as meal replacements when people can't eat enough.4 Powders get their own Answer: Can you take protein powder or supplements on a GLP-1?.

Vitamins and minerals. The Indian consensus links vegetarian and cereal-heavy diets to low vitamin B12, iron and vitamin D.4 It cites deficiency rates in Indians of 53%, 54% and 61%.4 The US advisory says doctors can consider supplements for vitamin D, calcium or B12, tailored to each person.2

What about eggs, chicken, fish and mutton?

Guidelines favour eggs, fish and skinless chicken, cooked without deep-frying. The US advisory suggests red and processed meats in moderation or less.

The US advisory encourages seafood, eggs and lean poultry.2 In the general population, red and processed meats are linked to type 2 diabetes, heart disease and bowel cancer, it notes.2 ICMR suggests skinless poultry, lean cuts of meat and fish.3

  • Egg, boiled, 50 g: about 7 g protein.5
  • Chicken breast, skinless, 100 g raw: about 22 g.5

ICMR's non-veg plate swaps some dal for 70 g of chicken, meat or eggs.3 For cooking, ICMR suggests grilling, baking, steaming or sautéing with little oil instead of frying.3 The Indian consensus adds boiling.4 The advisory notes that soups and yogurt often appeal more than heavy foods such as red meat.2

Why do Indian experts stress protein and fewer refined carbs?

The Indian consensus says many Indians start with less muscle and a cereal-heavy diet. Eating less on a GLP-1 can widen both gaps, its panel agreed.

South Asians have less muscle for their body size than Western populations, the Indian consensus says.4 It cites low muscle (sarcopenia) in 14% to 39% of Indian adults with malnutrition or obesity.4 In national survey data it quotes, 99% in the Northeast and 87% in the South mainly ate refined cereals.4 Its panel agreed that eating less on these medicines can lead to nutrient shortfalls if not monitored.4

India's lower BMI cut-offs have a different basis. A 2009 Indian consensus set them from body-fat and disease (morbidity) data in Asian Indians.8 Healthy is 18.0–22.9, overweight 23–24.9, and obesity 25 or more.8 How much muscle people lose on these medicines is in Do GLP-1 medicines cause muscle loss and loose skin? Weight and health in Indians is in Guide 1.

How much fibre, and when?

The US advisory and the Indian consensus describe adding fibre slowly, with enough fluid. ESI advises avoiding high-fibre meals to ease stomach side effects.

  • ESI 2025 (India): advises avoiding high-fibre meals, along with high-fat and high-carbohydrate ones, to ease stomach side effects.1
  • US advisory: suggests avoiding high-fibre and fatty foods in the first few days.2 After that, it describes adding fibre gradually to help with constipation.2
  • Indian consensus: high-fibre foods help stomach symptoms.4 It advises raising fibre slowly towards 20–30 g a day, with enough fluid.4

The consensus suggests both kinds of fibre: soluble, as in oats, and insoluble, as in whole wheat.4 Indian sources include whole grains, millets, dal, vegetables and fruit.3,4 For scale, 30 g of raw moong dal has about 3 g of fibre.5

Protein and fat slow the stomach further, the advisory says. That can make constipation worse.2 Constipation gets its own Answer: Constipation on a GLP-1.

How much should you drink on a GLP-1?

ICMR's figure for healthy adults is about 2 litres a day, counting other drinks, and more in hot weather. Losing fluids through vomiting or diarrhoea can harm the kidneys.

ICMR advises healthy adults to drink about 8 glasses, roughly 2 litres, including other drinks.3 More is needed in very hot weather and during hard exercise.3 The Indian consensus gives a similar figure for people on these medicines, as water or unsweetened drinks sipped through the day.4 It notes that people on these medicines may feel less thirsty.4

The US Wegovy label says vomiting and diarrhoea can cause dehydration, which may cause kidney problems.6 It asks people to drink fluids, and to tell their doctor if these symptoms don't go away.6 The US advisory says people with, or at risk of, kidney problems need advice on avoiding dehydration and checks of their kidney function.2

Which drinks. ICMR suggests tender coconut water, buttermilk or fresh lemon juice in place of sugary drinks.3 It also says people prone to high potassium (hyperkalaemia) from kidney or heart disease should avoid coconut water.3 ESI suggests small amounts of buttermilk or kokum juice, and avoiding fizzy drinks.1 ESI advises avoiding alcohol; the US advisory says keep it minimal.1,2 More in Can you drink alcohol on Mounjaro or Ozempic? Chai and coffee get their own Answer: Tea and coffee on a GLP-1.

How should meals change, and which foods make side effects worse?

ESI and the Indian consensus describe smaller meals, more often, eaten slowly. ESI links high-fat, high-carbohydrate and high-fibre meals, fizzy drinks and alcohol to worse stomach side effects.

Meal size and timing. ESI advises small, frequent meals.1 The Indian consensus suggests 4 to 5 a day, eaten slowly, stopping when comfortably full.4 Its panel agreed, 100%, on avoiding large evening meals.4 Vomiting is more likely after a large meal, the US advisory says.2

Long gaps without food matter too. Nausea often comes in the morning or after long periods without food.2 The advisory suggests a small breakfast, then small meals every 3–4 hours with fluids.2

Foods linked to worse side effects. In ESI's list: high-fat, high-carbohydrate and high-fibre meals, fizzy drinks and alcohol.1 The US advisory adds that avoiding large or fatty meals can help with diarrhoea.2 ICMR's weight-loss advice says to swap out deep-fried snacks, sweets, biscuits, cakes, fruit juices and cold drinks.3 Which foods cause which side effects is its own post: Which foods make GLP-1 side effects worse?.

Indian comfort foods for nausea. ESI lists ginger candy, liquorice (mulethi), cloves and fennel seeds (saunf).1 It also lists ice cubes or ice candy, and small sips of buttermilk or kokum juice.1 ESI says these stomach effects are usually mild and pass with time.1 Nausea has its own post: Nausea on a GLP-1, with Indian food.

Treats. The US advisory lists "portion-controlled treats" and flexible food choices as things to encourage.2 Festival food has its own post: Diwali sweets and parties on a GLP-1.

On insulin or a sulfonylurea (like glimepiride)? Low blood sugar is more likely with these and a GLP-1, sometimes severely.6,7 More in Can GLP-1 medicines cause low blood sugar? and the type 2 diabetes path.

Questions people ask

Is there a special Ozempic diet?

No guideline sets one. Ozempic's US label says it is used as an adjunct to diet and exercise. Wegovy, a higher dose of the same medicine for weight loss, is used with a reduced-calorie diet and more activity. Indian and US guidance describe smaller meals, protein first, fibre added slowly and enough fluids.1,2,6,12

Can vegetarians get enough protein on a GLP-1?

Guidelines list dal, beans, milk, curd, paneer and soy as protein sources. ICMR says cereals and pulses eaten together supply most essential amino acids. 100 g of paneer has about 19 g of protein.3,4,5

Can you eat rice and roti on Mounjaro?

The guidelines don't ban them. ICMR asks for at least half of cereals as whole grains, such as millets. The Indian consensus favours whole grains, millets and legumes, and restricting refined carbohydrates.3,4

Is buttermilk good for nausea on Ozempic?

India's ESI 2025 guideline suggests small amounts of buttermilk or kokum juice for nausea. It also lists ginger candy, cloves and fennel seeds, and says to avoid fizzy drinks.1

How much water should you drink on a GLP-1?

ICMR advises healthy adults to drink about 2 litres a day, including other drinks, and more in hot weather. An Indian consensus on these medicines gives a similar figure, sipped through the day. A US advisory says people with kidney problems need advice on avoiding dehydration and kidney checks.2,3,4

Do guidelines suggest vitamin supplements on a GLP-1?

A 2025 US advisory says doctors can consider vitamin D, calcium or B12 supplements, tailored to each person. An Indian consensus suggests checking nutrient levels alongside treatment.2,4

How we researched this

We read four sources in full. These were ESI's 2025 obesity guideline, the US nutrition advisory, the Indian consensus and ICMR-NIN's 2024 guidelines. We took protein figures from India's food composition tables (IFCT 2017). We checked side effects against the US Wegovy and Mounjaro labels; we found no Indian label online.

The Indian consensus was funded by Abbott Nutrition, and many US advisory authors declare drug-maker payments. We say so where we use them. Where sources disagree, as on fibre, we show each view.

We cut what we could not source. That includes curd protein, spicy food, katori measures and fluid limits for kidney or heart patients. We give no meal plan or calorie target.

Before you move on

  • No guideline sets a special Ozempic or Mounjaro diet.
  • Guidance puts protein first: dal with roti or rice, paneer, curd, soy, eggs, fish or chicken.
  • Guidance describes adding fibre slowly. ICMR gives about 2 litres of fluid a day for healthy adults, more in the heat.
  • Large, very oily or very starchy meals, fizzy drinks and alcohol are linked to worse side effects.
