Orlistat is a capsule taken with meals that stops about 30% of the fat you eat from being absorbed. GLP‑1 medicines are mostly weekly injections that act on appetite. In separate trials, orlistat added about 2 points of weight loss over a dummy pill (5% vs 3%, 4 years). Semaglutide added about 12 points (15% vs 2%, 68 weeks).
Key takeaways
- Orlistat blocks the gut enzymes that digest fat, while GLP‑1 medicines lower how much you eat, most likely by acting on appetite.1,3
- In the 4-year XENDOS trial, people on orlistat lost about 6 kg against 3 kg on a dummy pill, and only 52% finished.10
- In the first year of orlistat trials, 27% of people had oily spotting and 8% had leaking stools, against 1% each on a dummy pill.1
- In the STEP 1 trial, semaglutide 2.4 mg gave 15% weight loss against 2% on a dummy injection, but 44% felt sick against 17%.12
- India banned the diet pill sibutramine in February 2011, after a large trial linked it to more heart attacks and strokes.19,20

What is orlistat, and how does it work?
Orlistat capsules block the gut enzymes that digest fat, so about 30% of the fat in a meal passes out undigested.1
Your gut breaks down fat with enzymes called lipases. Orlistat locks onto these enzymes, so some fat can't be absorbed and leaves in your stools.1 It works inside the stomach and gut, and very little of it reaches the blood.1
The label pairs it with a reduced-calorie diet that gets about 30% of its calories from fat.1 The effect starts within 24 to 48 hours, and it's gone 2 to 3 days after the last dose.1 Guide 3 puts orlistat next to diet, GLP‑1s and surgery: your options, and where medicines fit.
How do you take orlistat capsules?
The US label says one 120 mg capsule three times a day, with each main meal that contains fat.1
It says to take each dose during the meal or up to an hour after.1 If a meal is missed or has no fat, the label says that dose can be left out.1 Doses above 120 mg three times a day gave no extra benefit.1
The label also advises a daily multivitamin with vitamins A, D, E and K. It says to take it at least 2 hours before or after orlistat, such as at bedtime.1
Indian pharmacy listings we checked show 60 mg and 120 mg capsules, and one 120 mg tablet.9
The US label is for people with obesity and a BMI of 30 or more. It also covers a BMI of 27 or more with another risk, such as high blood pressure or diabetes.1 Trials in teens aged 12 to 16 support its use at those ages. Its safety below age 12 hasn't been established.1 These are US label numbers. The Indian guideline's numbers are lower, as the India note below explains.
How do GLP-1 medicines work differently?
GLP‑1 medicines copy a gut hormone and lower how much you eat, most likely through appetite, rather than by blocking fat.3
Semaglutide and liraglutide copy the hormone GLP‑1. Tirzepatide acts on GLP‑1 and a second gut hormone, GIP.2 Semaglutide and tirzepatide are weekly injections, and liraglutide is a daily one.2 In India, semaglutide also comes as a daily tablet (Rybelsus) for type 2 diabetes.29 Our posts cover semaglutide tablets, liraglutide and tirzepatide.
Orlistat, liraglutide, injectable semaglutide and tirzepatide are the four US-approved obesity medicines available in India.4 Our guide what GLP‑1 medicines do explains the hormone and the jargon. Or try the short Answer What is the full form of GLP‑1?.
- GLP‑1 medicines are prescription-only in India.5
- Who may prescribe them: A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.5 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.28 More in Do you need a prescription for GLP‑1 medicines in India?
- Know the risks first: Guide 5: Side effects and risks.
- Check for recalls before using a generic GLP‑1 pack. Some batches were recalled in July 2026.6
- No promise of results. The numbers below are trial averages. Some people lose much less.
Is orlistat prescription-only in India?
Indian pharmacy listings mark orlistat as a prescription drug, even at 60 mg, but we found no CDSCO document that names its schedule.9
India has no separate over-the-counter category. Medicines outside the prescription schedules are generally sold without one.7
In the US, the rule is split by strength. Orlistat 60 mg (Alli) is sold without a prescription to overweight adults aged 18 and over.8 Orlistat 120 mg (Xenical) is prescription-only.1
Several Indian companies sell orlistat as 60 mg and 120 mg capsules.9 We don't list brand names.
How much weight do people lose on orlistat?
In the longest large trial, people on orlistat lost about 6 kg over 4 years, against 3 kg on a dummy pill.10
That trial, XENDOS, followed 3,305 adults with a BMI of 30 or more. Everyone also got lifestyle advice.10 Only 52% of the orlistat group finished all 4 years, against 34% on the dummy pill.10
As a share of body weight, it was 5% vs 3%.1 For a 90 kg person, that's about 5 kg against about 3 kg.
New type 2 diabetes was less common on orlistat: 6.2% vs 9.0% over 4 years.10 The difference showed only in people whose blood sugar was already raised after a glucose drink (impaired glucose tolerance).10
A small Indian trial of 80 people found about 5 kg vs 3 kg over 24 weeks.11 The Indian guideline gives 5% to 10% loss on orlistat in studies of 1 to 2 years. Placebo gave 2% to 7%.2
What are orlistat's side effects?
Orlistat's main side effects come from the fat it stops you absorbing: oily spotting, oily stools and urgent toilet trips, mostly in the first months.1
In the first year of the US trials, these were the most common, on orlistat vs a dummy pill:1
- Oily spotting: 27% vs 1%
- Gas with oily discharge: 24% vs 1%
- Sudden urge to pass stools (faecal urgency): 22% vs 7%
- Fatty or oily stools: 20% vs 3%
- Leaking stools (faecal incontinence): 8% vs 1%
In year 2, oily spotting fell to 4%, and leaking stools to 2%.1 About half of the episodes lasted under a week. Some people had them for 6 months or longer.1
These effects are more likely when over 30% of a day's calories come from fat.1 Fried snacks like pakoras are one example. In the trials, 9% stopped orlistat because of side effects, against 5% on the dummy pill.1
Less common effects and interactions
- Vitamins. Orlistat lowers the absorption of vitamins A, D, E and K, which is why the label advises a multivitamin.1
- Other medicines. It can change how these work: ciclosporin, thyroid tablets (levothyroxine), warfarin, amiodarone, epilepsy medicines and HIV medicines.1
- Diabetes medicines. As weight falls, the dose of insulin or a sulfonylurea may need lowering.1
- Rare but serious. Severe liver injury, kidney stones or kidney damage from oxalate, and gallstones (3% vs 2% in XENDOS).1
- Liver warning signs. These are yellow skin or eyes, dark urine, pale stools, itching, loss of appetite or pain under the right ribs. The label says orlistat should then be stopped and the liver tested.1
- Not used in pregnancy, with long-term poor absorption of food (malabsorption), or with blocked bile flow (cholestasis).1
How does orlistat compare with GLP-1 medicines side by side?
GLP‑1 trials showed larger weight loss than orlistat trials, but they were separate trials, and the side effects differ in kind.10,12
Both trials measured loss as a share of body weight. In XENDOS, orlistat added about 2 percentage points over a dummy pill (5% vs 3%, 4 years).1 Semaglutide 2.4 mg added about 12 points in STEP 1 (15% vs 2% over 68 weeks).12
In STEP 1, adults without diabetes on semaglutide 2.4 mg lost 15% of their weight over 68 weeks. People on a dummy injection lost 2%.12 For a 90 kg person, that's about 13 kg vs 2 kg. But 44% felt sick (nausea), against 17% on the dummy.12
In SURMOUNT-1, tirzepatide 15 mg gave 21% vs 3% over 72 weeks.13 That's about 19 kg vs 3 kg for a 90 kg person. On the top dose, 6% stopped because of side effects, against 3% on the dummy.13
Only one trial we found tested a GLP‑1 medicine alongside orlistat. Over 20 weeks, people lost 7 kg on liraglutide 3.0 mg and 4 kg on orlistat. The dummy-injection group lost 3 kg.14 Nausea and vomiting were more common on liraglutide.14 The orlistat group knew what they were taking, and Novo Nordisk, liraglutide's maker, paid for the trial.14

Separate placebo-controlled trials, run years apart in different people, so the columns can't be lined up exactly. Orlistat figures are from the US Xenical label (XENDOS). GLP‑1 figures are from STEP 1 and SURMOUNT-1, with warnings from ESI 2025 and the US Wegovy label. Checked 5 October 2026.
- Stomach effects are common. In STEP 1, 74% had a stomach or gut side effect, against 48% on the dummy.12
- Pancreatitis. Rare. The warning sign is severe stomach pain that won't go away.3
- Gallbladder problems. 2.6% vs 1.2% in STEP 1, mostly gallstones.12
- Dehydration from vomiting or diarrhoea can hurt the kidneys.3
- Thyroid warning. Ruled out by India's ESI 2025 obesity guideline for people with a personal or family history of medullary thyroid cancer (MTC) or MEN 2; tell your doctor if this applies. Not in pregnancy or breastfeeding.2

What does each cost in India?
Pharmacy listings print ₹554 for 10 orlistat capsules, while GLP‑1 medicines run from ₹1,290 a month for a vial to ₹25,781 for a pen.15,16,27
We found no orlistat price from a maker, a news report or NPPA, the government drug-price regulator.15 Two pharmacy listings agree on a printed MRP of ₹554 for one 120 mg strip of 10 capsules.15 At the label's 3 capsules a day, a month is about 91 capsules. At that MRP, that's about ₹5,000 (our sum). Prices vary by brand and pack.
Generic semaglutide launched in March 2026. In pens, makers priced 4 starting doses at ₹1,800 to ₹4,200.30,24 For 4 top doses, pen prices run from ₹4,200 to ₹8,000 (Sun Pharma's ₹2,000 a week).24,25
NATCO sells multi-dose vials instead, priced per vial rather than per dose. They start at ₹1,290 a month for lower strengths and reach ₹1,750 for the highest.16 Our post on semaglutide generics in India covers the launches.
Mounjaro (tirzepatide) launched in August 2025 at ₹14,000 to ₹27,500 for a pen of 4 doses.17 On 22 September 2025, GST on most medicines fell from 12% to 5%.26 Pharmacy listings now print MRPs of ₹13,125 (2.5 mg) to ₹25,781 (12.5 and 15 mg) per pen.27 That matches the launch price less the tax cut (our sum: ₹14,000 × 105/112 = ₹13,125). We found no Lilly statement giving the new figures. Guide 6 lists each brand's MRP.
A visit to an endocrinologist costs a median of ₹1,100 on Practo.18 A 2026 Indian review found no health-insurance cover for medicines taken outside hospital.4
Why were sibutramine and lorcaserin withdrawn?
Both were weight-loss pills pulled after large safety trials: sibutramine for more heart attacks and strokes, lorcaserin for more cancers.20,22
The SCOUT trial enrolled 9,804 people with heart disease, type 2 diabetes or both. They took sibutramine or a dummy pill for about 3 years.20 Heart attacks, strokes, cardiac arrests or heart deaths happened in 11% vs 10%.20 Non-fatal heart attacks were 4% vs 3%, and non-fatal strokes 3% vs 2%. Deaths were not higher.20
Sibutramine left the US market in October 2010.21 India banned it, and R-sibutramine, by gazette notice on 10 February 2011.19 India has also banned rimonabant (2009), and fenfluramine and dexfenfluramine (1998).19
Lorcaserin (Belviq) was a US weight-loss pill. In February 2020, the US FDA asked its maker to withdraw it.22 A trial followed about 12,000 people for about 3 years. Cancer was found in 462 people (8%) on lorcaserin and 423 (7%) on placebo.22 This was a US decision. We couldn't confirm whether lorcaserin was ever approved in India.
Since 1964, 26 weight-loss drugs have been withdrawn, about 80% after reports of harm.4
Questions people ask
Does orlistat work as well as semaglutide?
No trial has compared them head to head. In separate trials, orlistat gave about 5% loss over 4 years, and semaglutide 2.4 mg about 15% over 68 weeks.1,12 Their side effects differ. Oily stools are common on orlistat, and nausea is common on semaglutide.1,12
Can orlistat be taken with a GLP‑1 medicine?
We found no trial of the two taken together. The Drugs.com interaction checker rates orlistat with semaglutide as a moderate interaction.23 It explains that as weight falls, blood sugar can drop in people with type 2 diabetes. Diabetes medicines may then need adjusting.23
Is 60 mg orlistat sold without a prescription in India?
Indian pharmacy listings we checked mark 60 mg orlistat as a prescription drug.9 In the US, 60 mg orlistat (Alli) is sold without a prescription to adults.8
What happens if you eat a fatty meal on orlistat?
Oily stools, gas with discharge and urgent toilet trips become more likely. The US label links this to a diet with over 30% of daily calories from fat.1
Is orlistat safe for the liver?
Severe liver injury is rare, but the US label reports cases, some leading to a liver transplant or death.1 Yellow skin or eyes, dark urine and pale stools are warning signs.1
Can you take orlistat during pregnancy?
No. The US label says orlistat must not be used in pregnancy.1 The Indian guideline says the same for GLP‑1 medicines, and adds breastfeeding.2
How we researched this
On 5 October 2026 we read the US labels for Xenical (orlistat 120 mg) and Alli (60 mg) on DailyMed. We found no Indian orlistat label online, so the US one is cited and named as such. We read the Indian obesity guideline (ESI 2025) and the CDSCO list of banned drugs.
We read the XENDOS, STEP 1, SURMOUNT-1, SCOUT and Astrup 2009 trial reports, and noted who paid for each. GLP‑1 prices come from makers' press releases and dated news reports. Mounjaro's and orlistat's MRPs come from two pharmacy listings that agree, cited without links.
We left some things out. We found no orlistat price from a maker, a news report or NPPA. We found no CDSCO document naming orlistat's legal schedule. One pharmacy blog says 60 mg is sold over the counter here. The same chain's product page says prescription, so we don't repeat the claim. We also left out brand names.
Before you move on
- Orlistat blocks fat absorption in the gut; GLP‑1 medicines act on appetite.
- In separate trials, orlistat added a few kilos of loss; GLP‑1 medicines added more, with more nausea.
- Orlistat's oily-stool side effects are common in the first year and depend on how much fat you eat.
- Sibutramine is banned in India, and hidden sibutramine has been found in slimming products.
