Written for Canada. Reading from India? India version
Official prices for a 4-week pen: Ozempic $227.94 and generic semaglutide $78.14 on Ontario’s drug plan list; Wegovy $388.64 in the 2025 federal review; Mounjaro and Zepbound $300 at 2.5 or 5 mg and $420 at 7.5 or 10 mg, Lilly’s list prices from 29 December 2025 as reported by The Globe and Mail. Pharmacies set their own cash prices.
Key takeaways
- Ontario’s drug plan lists an Ozempic pen at $227.94 and a generic semaglutide pen at $78.14, and pays the generic price for both.1
- Canada’s Drug Agency used $388.64 for a Wegovy pen, the same at every dose, or $5,066 a year.2
- The Globe and Mail reported that Eli Lilly would cut Mounjaro and Zepbound to $300 for 4 weeks of 2.5 or 5 mg and $420 for 7.5 or 10 mg, from 29 December 2025.3
- Most of these pens hold 4 weekly doses, so a year of weekly injections uses 13 pens, not 12.4,5,2
- Savings-card prices are not list prices, and Novo Nordisk’s Ozempic card does not apply in Quebec.6
- GLP‑1 medicines are prescription drugs in Canada. Nothing here is a reason to start, stop or switch one.7
- We name brands and official prices as facts. We don’t recommend, rank or link to any brand, pharmacy, clinic or program.
- Ozempic, its generics and Mounjaro are approved for type 2 diabetes. Using them for weight loss is off-label (outside the approved use).4,8
- The risks are the same however you get the medicine: see our side effects guide.

What does one pen contain?
Four weekly doses, for most pens: Ozempic, its generics, Wegovy FlexTouch, Mounjaro KwikPen and Zepbound KwikPen.4,2,8,5
That makes one pen roughly a month. A year of weekly doses needs 52 doses, which is 13 pens.
Two exceptions hold one dose each. Wegovy now also comes as a single-use pen, and Mounjaro also comes as a single-dose pen or vial.9,8 In Canada, Zepbound comes only as the 4-dose KwikPen.5
The price of a pen often doesn’t change with the dose inside it. Canada’s Drug Agency noted that Wegovy pens cost the same “for all doses”.2
What are the official prices?
These are the published figures we could find, each with its source and date. We found no published price for Mounjaro or Zepbound at 12.5 or 15 mg.

A formulary price is what a provincial plan recognizes for a product. A pharmacy sets its own cash price. Before generics arrived, Global News reported brand semaglutide at “hundreds of dollars a month”, in some cases over $450.10
Why is generic semaglutide about a third of Ozempic’s price?
Canada’s generic pricing framework sets a non-oral generic at 35% of the brand price once three or more generics exist, and Ontario’s price sits close to that tier.11,1
The pan-Canadian Pharmaceutical Alliance (pCPA), which negotiates drug prices for public plans, sets generic prices in tiers. With one generic the price is 75% to 85% of the brand; with two, 50%; with three or more, 35% for injections.11
Ontario’s $78.14 is 34% of Ozempic’s $227.94.1 Apotex said its generic would be “approximately one-third the price” of Ozempic.10
The maker responded. From late May 2026, Novo Nordisk offered a savings card to people without public or private coverage, “aligning their medication costs with generic alternatives”. The card excludes Quebec.6 We don’t show savings-card prices, because they are not list prices and the terms change.

What does a year cost at these prices?
At official prices, 13 pens of Ozempic come to about $2,963, generic semaglutide about $1,016, and Wegovy $5,066 a year by the federal agency’s count.1,2
Our arithmetic, except Wegovy. Tirzepatide: one 4-week supply each at 2.5, 5 and 7.5 mg, then 10 at 10 mg: $300 + $300 + $420 + 10 × $420. Held at 5 mg, it is 13 × $300 = $3,900. Dispensing fees are left out.
Tirzepatide starts at 2.5 mg for 4 weeks, then goes up in 2.5 mg steps at least 4 weeks apart.5 Your prescriber may move more slowly or stop at a lower dose.
For context: in the 72-week SURMOUNT-1 trial, people on Zepbound 10 mg lost 19.5% of their weight on average. On a dummy injection, it was 3.1%.5 Results varied widely, and the harms are below.
- Stomach effects. About 44 in 100 people on Wegovy 2.4 mg felt sick (nausea), against 16 in 100 on a dummy injection. On Zepbound 15 mg it was about 28 in 100, against about 9 in 100.9,5
- Gallbladder. Gallstones affected about 16 in 1,000 people on Wegovy, against 7 in 1,000 on a dummy injection. An inflamed gallbladder affected about 6 in 1,000 on Zepbound, against 2 in 1,000.9,5
- Pancreatitis (an inflamed pancreas): severe stomach pain that won’t go away, sometimes spreading to the back.9
- Dehydration from vomiting or diarrhea can harm the kidneys.9
- Thyroid and pregnancy. Not for anyone with a personal or family history of medullary thyroid cancer, or MEN 2. Canada’s obesity guideline advises against these medicines in pregnancy, while breastfeeding or when trying to conceive.5,12
Who actually pays?
Public drug plans cover semaglutide for type 2 diabetes, with conditions. As of 12 October 2026, none covers any GLP‑1 for weight management.14,15,16
Each province and territory runs its own plan. The plan-by-plan detail is in does your province cover Ozempic?, and the weight-loss story in why no province covers Wegovy or Zepbound.
Private plans vary. In a 2026 survey, 37% of Canadian employers covered GLP‑1s for weight loss as well as diabetes.17 See private insurance and GLP‑1s.
Advertised prices you see online are a seller’s own. Canadian law lets a prescription drug be advertised to the public only by name, price and quantity.18
Questions people ask
How much is Ozempic in Canada without insurance?
Ontario’s drug plan lists an Ozempic pen, 4 weekly doses, at $227.94. Pharmacies set their own cash prices; Global News reported brand semaglutide at hundreds of dollars a month, sometimes over $450, before generics arrived.1,10
How much is generic semaglutide in Canada?
Ontario’s formulary lists Apo-Semaglutide and Dr. Reddy’s pens at $78.14 each, about a third of the Ozempic price.1
How much is Mounjaro in Canada?
The Globe and Mail reported that Lilly would set Mounjaro and Zepbound at $300 for 4 weeks of 2.5 or 5 mg and $420 for 7.5 or 10 mg from 29 December 2025; Reuters could not verify the report at the time. We found no published price for 12.5 or 15 mg.3
Is Wegovy more expensive than Ozempic?
Per pen, Canada’s Drug Agency used $388.64 for Wegovy, while Ontario lists Ozempic at $227.94. A Wegovy pen costs the same at every dose.2,1
Why does my pharmacy charge a different price?
Formulary prices are what a plan recognizes. Each pharmacy sets its own cash price and dispensing fee, so the till price can differ from the official figure.1
How we researched this
We used official figures only: Ontario’s Drug Benefit Formulary (checked 12 October 2026), Canada’s Drug Agency’s 2025 Wegovy review, and Lilly’s prices as reported by The Globe and Mail. We show no pharmacy, clinic or coupon price.
Year totals are our arithmetic, at 13 pens a year.
Before you move on
- Price the whole dose ladder, not the first pen.
- Count 13 pens a year, not 12.
- Ask your plan what it pays before you fill it.
- Ask the pharmacy for its dispensing fee.
- Treat a coupon price as temporary.
