How long does Ozempic take to work?

The short answer

Ozempic (semaglutide) lowers fasting blood sugar from the first dose, says the EU label. The US label says levels settle after 4 to 5 weekly doses. US guidelines recheck HbA1c, a 3-month sugar average, about every 3 months. Nausea is most common during the dose rises. In a trial of Wegovy's higher dose, weight kept falling until week 60.

Before you go further
  • GLP‑1 medicines such as Ozempic are prescription-only in India, not sold over the counter.9
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.9
  • The risks are in Guide 5: Side effects and risks.
  • Nothing here promises any result.

When does Ozempic start lowering blood sugar?

Ozempic lowers fasting blood sugar from the first dose, says the European label, but the 0.25 mg starting dose is not a maintenance dose.2

Half of each dose is cleared in about a week. So levels build up and settle after 4 to 5 weekly doses (steady state).1 Levels peak 1 to 3 days after each shot.1

After 12 weeks of treatment, including the dose rises, fasting sugar on 1 mg had fallen 29 mg/dL. That was 22% lower than on a dummy injection (placebo).1 In a 30-week trial, fasting sugar fell 41 to 44 mg/dL on Ozempic vs 15 mg/dL on placebo.1

HbA1c, your 3-month sugar average, moves more slowly.3 US diabetes guidelines recheck it about every 3 months after a treatment change, to see if goals are met.10 In the 30-week trial, HbA1c fell from about 8.1% to 6.5–6.7%, with almost no change on placebo.1 The type 2 diabetes path explains HbA1c.

The US label warns that a fast fall in sugar can briefly worsen diabetic eye disease (retinopathy).1 In a 2-year trial, eye complications hit 3.0% on Ozempic vs 1.8% on placebo.1

Why does the Ozempic dose go up every 4 weeks?

The US label starts Ozempic at 0.25 mg a week for 4 weeks, then raises it slowly to reduce stomach side effects.1

Your doctor sets your dose. This is the US label. Each rise comes at least 4 weeks after the last.1

Weeks counted by us from the US label's 4-week minimum steps.

In India, Ozempic launched in December 2025 in 0.25, 0.5 and 1 mg strengths only.8 So the 2 mg step is from the US label. We found no report of a 2 mg pen in India as of 5 Oct 2026. Most reports of nausea, vomiting or diarrhoea came during the dose rises.1 Nausea affected 16% to 20% of people on Ozempic, against 6% on placebo.1

How fast does weight come off on Ozempic?

In Ozempic's own 30-week diabetes trial, people lost about 4 to 5 kg on average, against about 1 kg on a dummy injection.1

STEP 2 gave semaglutide 1 mg, an Ozempic dose, to adults with overweight and type 2 diabetes for 68 weeks.6 They lost 7% of their weight on average, against 3% on placebo.7 For a 90 kg person, 7% is about 6 kg.

The clearest weight curve is from STEP 1, which tested Wegovy's higher 2.4 mg dose in adults without diabetes.4 Weight loss showed at the first check in week 4 and continued to a low point at week 60.4 At 68 weeks the average loss was 15%, against 2% on placebo.4,5

Results varied widely. In STEP 1, about half lost 15% or more, but about 1 in 6 lost less than 5%.5 We compare the trials in How much weight do people lose on Ozempic or Mounjaro?

RisksThe risks, in brief
  • Stomach effects are common. Nausea in 16% to 20% on Ozempic vs 6% on placebo.1
  • Pancreatitis. Severe stomach pain that won't go away, sometimes spreading to the back.1
  • Gallbladder problems, including gallstones.1
  • Dehydration from vomiting or diarrhoea can hurt the kidneys.1
  • Hypos with insulin or a sulfonylurea such as glimepiride.1
  • Thyroid warning. Ruled out by the US label for people with a personal or family history of medullary thyroid cancer or MEN 2; tell your doctor if this applies.1
