Does Mounjaro or Ozempic help fatty liver?

The short answer

Both helped in trials, but only semaglutide 2.4 mg (Wegovy, above Ozempic's doses) holds conditional liver approvals, in India and the US. We found none for Ozempic or Mounjaro. Wegovy cleared inflammation in 63% vs 34% on a dummy; nausea hit 36% vs 13%. Tirzepatide (Mounjaro) 5 to 15 mg managed 44% to 62% vs 10% in a small trial.

Before you go further
  • Wegovy, Ozempic, Mounjaro and other GLP‑1 medicines are prescription-only in India, not sold over the counter.1
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.1 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.2
  • Trial results are group averages. Whether one suits your liver is a doctor's call. Guide 4 covers who these medicines are and aren't for.
  • The risks are in Guide 5: Side effects and risks.

What did the semaglutide liver trial find?

In the ESSENCE trial, weekly semaglutide 2.4 mg (the Wegovy dose) cleared liver inflammation in 63% of people, against 34% on a dummy injection.3

Its first 800 adults had fatty liver with inflammation (MASH) and moderate or advanced scarring (fibrosis stage 2 or 3).3 After 72 weeks, scarring improved in 37% vs 22%, and weight fell 10.5% vs 2.0%.3 Novo Nordisk, the maker, paid for it.3

Nausea hit 36% vs 13%, diarrhoea 27% vs 12% and vomiting 19% vs 6%.4 About 3% in each group stopped because of side effects.4

Of those 800, 27% were Asian, 7% had a BMI under 25 and 3% had “lean MASH” (BMI under 23 for Asians). The US label calls data under 25 limited.5

Is Ozempic approved for fatty liver?

No. We found none; liver approvals are for semaglutide 2.4 mg, sold as Wegovy in the US and India.5,6

In India, Ozempic comes in 0.25, 0.5 and 1 mg strengths and is for diabetes.7 The US Ozempic label lists no liver use.8

The US approved Wegovy injection for MASH on 15 Aug 2025.9 It is an early (accelerated) approval; keeping it depends on results due in 2029.5,9 Europe gave a conditional approval on 26 Mar 2026, as Kayshild.10

CDSCO's expert committee backed a conditional Indian approval on 23 Apr 2026, tied to part 2 of the trial.11 CDSCO approved Wegovy in July 2026, and Emcure's Poviztra, the same Novo Nordisk semaglutide, got the same use.6,12 All three regions cover adults with stage 2 or 3 scarring, not cirrhosis.5,6,10

AASLD's November 2025 guidance says that scarring is found with tests such as FibroScan (8 to 15 kPa), not biopsy.20 An ultrasound “grade 1 fatty liver” grades fat, not scarring.18

Does Mounjaro help fatty liver?

A small phase 2 trial suggests Mounjaro (tirzepatide) may, but we found no liver approval for it in the US, Europe or India.6,12,13,14

SYNERGY-NASH tested weekly tirzepatide 5, 10 or 15 mg against a dummy in 190 adults with MASH and stage 2 or 3 scarring.15 After 52 weeks, inflammation cleared in 44% to 62% vs 10%, and scarring improved in 51% to 55% vs 30%.15

Nausea hit 34% to 44%, against 12%, in this Eli Lilly-funded trial.15 Its authors note that “persons of African and Indian descent were underrepresented” and call for larger, longer trials.15 A phase 3 programme of about 4,500 people, testing tirzepatide and retatrutide, is recruiting at 20 Indian sites.16

RisksThe risks, in brief
  • Stomach effects are common. Nausea hit 36% on semaglutide 2.4 mg in ESSENCE and 34% to 44% on tirzepatide in SYNERGY-NASH. On a dummy, it was 12% to 13%.4,15
  • Pancreatitis is uncommon (1 in 100 to 1 in 1,000) on the EU Wegovy label. Gallstones are common (1 in 10 to 1 in 100).17
  • Dehydration from vomiting or diarrhoea can harm the kidneys.17
  • Thyroid warning. The US Wegovy label rules it out after medullary thyroid cancer or MEN 2, in you or your family.5
  • Pregnancy. The US Wegovy label says it may harm an unborn baby. It is usually stopped at least 2 months before a planned pregnancy.5

How much weight loss helps a fatty liver?

US liver specialists (AASLD, 2023) say losing 3% to 5% of body weight cuts liver fat, but more than 10% is usually needed for inflammation and scarring.18

Europe's 2024 guideline aims for at least 5% for fat, 7% to 10% for inflammation and 10% for scarring.19

Written before ESSENCE reported, it said GLP‑1 medicines could not yet be recommended as liver treatments, but are safe for type 2 diabetes and obesity.19

AASLD's 2025 semaglutide update says lifestyle change remains the cornerstone, and full approval awaits long-term results.20 Our post Fatty liver treatment in India covers diet, exercise and other medicines.
