How long can Mounjaro stay out of the fridge?

The short answer

The US and EU labels give an opened Mounjaro (tirzepatide) KwikPen up to 30 days out of the fridge, at no more than 30 °C. Then it must be thrown away. A single-dose vial or pen gets 21 days in total. The labels say never to use it if frozen. We found no public Indian label.

  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.12 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.13,14

How long can an opened Mounjaro KwikPen stay out of the fridge?

The US and EU labels say an opened Mounjaro KwikPen can stay at room temperature, up to 30 °C, for 30 days.

The US label says to throw the pen away at whichever comes first. That is 30 days at room temperature, 30 days after first use, or 4 weekly doses. It adds, "even if there is medicine left in it".1 The EU label says the pen "must be discarded 30 days after first use".2

An unopened KwikPen lasts until its expiry date in the fridge. Of the two labels, only the US one allows an unopened pen 30 days at room temperature.1 The EU label allows up to 72 hours below 30 °C, while the pen is supplied to the patient.2

Lilly's UK medical information for healthcare professionals says the 30-day allowance is for the pen in use. Wholesalers and pharmacies "cannot use any part of this allowance".3 It adds that an in-use pen "must NOT be stored with the needle attached".3 Moving an opened pen in and out of the fridge within the 30 days does not affect its quality.3 The KwikPen went on sale in India in August 2025.11

What about a single-dose Mounjaro vial or pen?

A single-dose Mounjaro vial or pen gets 21 days in total out of the fridge, up to 30 °C, under both labels.

The US label says "a total of 21 days". The EU label says "21 cumulative days" below 30 °C.1,2 Cumulative means the days add up, even if the vial goes back in the fridge in between. Lilly launched Mounjaro in India in March 2025 as a single-dose vial.10

What do the labels say about heat above 30 °C, light and freezing?

Every Mounjaro label we read sets 30 °C as the limit and says not to use the medicine if it has been frozen.

The US label says "Protect MOUNJARO from heat and light" and to keep it in its original carton.1 The EU KwikPen leaflet says "If the pen has been frozen, DO NOT USE."2

Neither Mounjaro label gives a rule for throwing a pen away after a spell above 30 °C. The same Lilly UK page says patients should make sure the pen "is not exposed to temperatures above 30°C".3 The US Wegovy guide says to throw the pen away if it has been above 30 °C.6

Outdoor summer highs in India go well past that limit. On 25 Apr 2026, the India Meteorological Department (IMD) reported highs of 40–44 °C across north-west, central and peninsular India. How does that compare with Ozempic and Wegovy?

Opened Ozempic and Wegovy pens (both semaglutide) last longer, 56 days on US labels and 6 weeks on EU labels, up to 30 °C.

The EU labels give 8 weeks for the 8-dose Ozempic pen.5 The EU leaflets say not to inject either one after it has been in direct sunlight.5,7 The Ozempic labels add that the pen should not be left in a car.4,5
