Exercise on Mounjaro or Ozempic: keeping muscle

Exercise on Mounjaro, Ozempic or Wegovy: what Indian and global guidelines say, what the main trial found, and when to take extra care.

Written for India. Reading from the US? US versionComing soon

The short answer

The US Ozempic and Mounjaro labels call each an add-on to diet and exercise. A 2025 US advisory suggests strength work at least 3 times a week, plus 150 minutes of moderate activity. India's ICMR-NIN sets similar targets. The only trial adding exercise to a GLP-1 used liraglutide. Together they cut body fat the most, and exercise helped protect bone.

Key takeaways

  • Guidance for people on GLP-1 medicines puts strength training first: a 2025 US advisory suggests it at least 3 times a week, plus 150 minutes of moderate activity.6
  • India's ICMR-NIN advises adults to do 30–60 minutes of moderate activity on at least 5 days a week, and muscle-strengthening on at least 2 days.2
  • In Denmark's S-LiTE trial, exercise plus liraglutide cut body fat by 3.9 percentage points, about twice either alone, but stomach side effects hit 71% vs 45% on placebo.8
  • A year after stopping, S-LiTE's exercise-plus-liraglutide group regained 2.5 kg less than liraglutide alone (not significant).9
  • On insulin or a sulfonylurea, exercise can bring on low blood sugar unless doses or food are adjusted, says the American Diabetes Association's 2026 guidance.7

  • GLP-1 medicines are prescription-only. A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.18 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.19,20

Why does exercise matter more on a GLP-1?

When you eat less, your body loses some lean tissue along with fat. Strength training is the signal that tells your body to keep muscle.2,6

India's national nutrition institute, ICMR-NIN, says: "Not using muscles causes muscle atrophy."2 It says regular strength training with weights is needed to keep muscle.2

One caveat runs through this page. The only trial that tested exercise alongside a GLP-1 medicine used liraglutide, an older daily injection.8,13 It did not test semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro), and its results may not carry over to them.13

How much lean mass people lose on these medicines is in do GLP-1 medicines cause muscle loss and loose skin?

A 2025 advisory from four US nutrition and obesity groups looked at people on GLP-1 medicines. It says eating more protein alone is "likely inadequate" to keep muscle without strength training.6 Strength or mixed training is "well established" to protect lean mass during weight loss, while aerobic exercise alone does less.6

The medicines are meant to come with activity. The US Ozempic (semaglutide) label calls it "an adjunct to diet and exercise" for type 2 diabetes.21 The US Mounjaro (tirzepatide) label uses the same words.15 These are US labels, not the Indian ones. The US Wegovy label, a higher semaglutide dose for weight, pairs Wegovy with "a reduced calorie diet and increased physical activity".14 India's endocrinologists (ESI 2025) say these medicines do "not obviate the need" for diet and regular exercise.17

The big trials gave activity advice too. SURMOUNT-1, the main tirzepatide trial, advised 150 minutes a week "without a specific strength training protocol".16 STEP 1, the main trial of semaglutide 2.4 mg (Wegovy), recommended the same 150 minutes.6 So the published results already came with activity advice. They are in how much weight people lose on Ozempic or Mounjaro.

RisksThe risks, in brief
  • Stomach effects are common. Nausea, diarrhoea, vomiting, constipation and stomach pain lead the labels' lists.14,15
  • Pancreatitis (an inflamed pancreas) is a label warning. The sign is severe stomach pain that won't go away.14,15
  • Gallbladder problems, such as gallstones, are in both labels.14,15
  • Low blood sugar is more likely when taken with insulin or a sulfonylurea, a type of diabetes tablet.14,15
  • Dehydration from vomiting or diarrhoea can injure the kidneys.14,15
  • Thyroid warning. Ruled out by the US labels for people with a personal or family history of MTC, a thyroid cancer, or MEN 2; tell your doctor if this applies.14,15 The Wegovy label says to stop at least 2 months before a planned pregnancy.14

What do Indian and global guidelines say?

WHO, ICMR-NIN and Fit India agree on the basics: about 150 minutes of moderate activity a week, plus muscle-strengthening on at least 2 days. Some Indian advice asks for more.1,2,3

Moderate means you breathe harder but can still talk, like walking briskly at 3–6 km an hour.2 Vigorous, like jogging, leaves you unable to talk freely.2 On a 0–10 effort scale, moderate is about 5 or 6.1

GuidelineWritten forAerobic (walking, cycling)Strength
WHO, 2020All adults1150–300 min moderate, or 75–150 min vigorous, a week1All major muscle groups, 2 or more days a week1
ICMR-NIN, 2024Indian adults aged 19–60230–60 min moderate a day on at least 5 days, or 15 min vigorous2All muscle groups, at least 2 days a week2
Fit India protocolsAdults aged 18–653At least 30 min on most days; 60 min on most days to manage weight32 or more days a week (repeats WHO)3
Indian consensus, 2012Asian Indians460 min a day in all: 30 min moderate aerobic plus 15 min work-type activity415 min, at least 3–4 times a week, with light weights4
US advisory, 2025People on GLP-1 medicines6At least 150 min moderate a week6At least 3 times a week6
ADA, 2026Adults with diabetes7150 min or more over at least 3 days; no more than 2 days off in a row72–3 sessions a week, not on back-to-back days7

Only the US advisory is written for people on GLP-1 medicines. ICMR-NIN's 30 minutes on 5 days makes 150 minutes a week (our arithmetic). Fit India was developed by the Sports and Health ministries together. None of these is a personal plan.

The US advisory says the plan should match each person's fitness level.6 The American Diabetes Association (ADA) adds a line for people with diabetes on obesity medicines. Strength exercise "may be beneficial for maintaining lean body mass", it says.7

WHO says some activity is better than none, and that cutting sitting time helps at any intensity.1 The ADA suggests breaking up long sitting at least every 30 minutes.7

What counts as strength training?

Strength training is any exercise that makes your muscles work against weight. ICMR-NIN lists weights, elastic bands, push-ups, crunches, squats and even carrying a child.2

WHO asks for work on all the major muscle groups.1 India's 2009 obesity consensus describes 1 to 3 sets of 8 to 12 lifts each.5 It suggests 2 to 3 days a week, at moderate effort.5 The 2012 Indian activity guidelines say inactive people should start slow and build up.4

Yoga. ICMR-NIN counts yogasanas and pranayama as moderate activity.2 The Indian consensus encourages physically intensive yoga, but says more research is needed.4 We found no trial of yoga alongside a GLP-1 medicine.

Trial evidence on strength training. One US trial put 160 older adults with obesity on a diet with different kinds of exercise.11 Lean mass fell 5% with aerobic exercise alone, vs 2% to 3% with strength or combined training.11 Strength rose 18% to 19% in those two groups, vs 4% with aerobic only.11

That trial also reported exercise injuries to muscles and joints.11 A 2026 review pooled diet trials where people lost at least 10% of their weight.12 Without exercise, 22% of the weight lost was lean tissue (fat-free mass); with exercise, it was 8%.12 These were diet trials, not GLP-1 trials.

What happened when a trial added exercise to a GLP-1?

In Denmark's S-LiTE trial, exercise plus liraglutide cut body fat the most and improved fitness. In a bone analysis, exercise helped protect bone.8,10

S-LiTE enrolled adults with obesity (BMI 32 to 43) and no diabetes.8 First, 195 people lost an average of 13.1 kg on an 8-week low-calorie diet.8 Lean mass fell during the diet too, from 65.3 kg to 60.4 kg on average.8

Each person then spent a year in one of four groups: exercise, liraglutide, both, or a dummy injection (placebo).8 Liraglutide 3.0 mg is a daily injection.8 It is an older GLP-1 medicine, covered in liraglutide, Saxenda and Victoza.

The exercise plan was built to meet WHO's 150-minute target.8 It meant 2 supervised sessions a week: 30 minutes of hard indoor cycling and 15 minutes of circuit training.8 People also exercised on their own, managing about 111 to 118 minutes a week in total.8

Gap between "both" and liraglutide alone (2.7 kg) could have been chance. Lundgren et al., NEJM 2021. Checked 5 October 2026.8

Body fat fell by 3.9 percentage points with both, about twice as much as with either alone.8 The paper's summary says only the combination improved fitness compared with placebo.8 Its results section adds that fitness also rose with exercise alone, not with liraglutide alone or placebo.8 Exercise was linked to more lean mass.8 Waist size fell in every active group, twice as much with both.8

A bone analysis of the same trial compared each group with placebo.10 In the group on both, the change in hip and spine bone density did not differ from placebo, though that group lost more weight (16.9 kg vs 7.0 kg over the whole study).10 Liraglutide alone lowered bone density more than exercise alone, despite similar weight loss.10

Side effects. Stomach side effects hit 86% on liraglutide alone and 71% on both, vs 45% on placebo.8 Serious gallstone problems were more common with liraglutide alone than with the combination, and liraglutide alone raised resting heart rate.8

Limits. The authors say the results may not apply to people over 65 or with type 2 diabetes.8 Nor may they apply above a BMI of 43.8 They may not apply to anyone unable to do moderate-to-hard exercise.8 A 2026 review notes this evidence comes from one liraglutide trial.13 It may not carry over to semaglutide or tirzepatide.13

The trial was funded by the Novo Nordisk Foundation and others.8 We found no Indian trial of exercise with a GLP-1 medicine.

Does exercise help after the medicine stops?

S-LiTE's exercise was supervised and ran only during treatment. After stopping, adding it to liraglutide cut regain slightly, not clearly.9

Of 195 randomised, 109 were weighed a year later.9 In that year, exercise plus liraglutide regained 2.5 kg less than liraglutide alone (95% CI −1.5 to 6.5), not significant.9 Exercise alone (no medicine) regained 6.0 kg less than liraglutide.9

From the start of treatment, those who did both ended 5.1 kg lighter than liraglutide alone.9 Their odds of still being at least 10% below their weight before the diet were about 4 times higher than the liraglutide-only group.9

Only 66% of those who finished treatment came back, and they had done better during treatment.9 A 2026 review says no trial has yet tested exercise started at the point of stopping.13

Fit India's protocol says 60–90 minutes of moderate activity a day may be needed to keep weight off.3 Stopping and what follows is in Guide 9: stopping and the long term. A full post is on the way: What happens when you stop Ozempic or Mounjaro?.

Track your waist, not just the scale.

Indian guidance sets a waist of 90 cm (35.4 inches) for men and 80 cm (31.5 inches) for women.5 Above that, it advises checking for weight-related health risks.5 The same consensus sets Indian BMI cut-offs: healthy is 18.0–22.9, overweight 23–24.9 and obesity 25 or more.5 Guide 1: weight and health in Indians explains both.

Why can exercise feel harder on a GLP-1?

Tiredness and dizziness are listed side effects of these medicines. Indian guidelines say build up slowly, and rest when warning signs appear.2,14

In the Wegovy 2.4 mg trials (a higher semaglutide dose than Ozempic's), 11% reported tiredness (fatigue) vs 5% on placebo.14 Dizziness was 8% vs 4%.14 Resting heart rate rose by 1 to 4 beats a minute on average.14 On the US Mounjaro label (diabetes trials), heart rate rose by 2 to 4 beats a minute on average, vs 1 on placebo.15

ICMR-NIN lists breathlessness, pain, nausea, vomiting and headache as signs your body needs rest.2 It says to raise time and effort gradually.2 The Indian consensus says to stop a strength session for any unusual chest pain or breathlessness.4

Fluids. Vomiting or diarrhoea on these medicines can dehydrate you and harm the kidneys, the labels warn.14,15 Fit India advises care with fluids and clothing when exercising in hot, humid weather.3 It also says sustained, heavy exercise should be avoided in hot and humid conditions.3

The Indian consensus advises drinking enough before activity.4

Getting checked first. ICMR-NIN advises people with diabetes, heart disease, high blood pressure or obesity to see a physician first.2 That applies before starting any exercise programme.2 It says the same for anyone over 40 starting vigorous exercise.2 Nausea gets its own post: Nausea on a GLP-1.

What if you take insulin or a sulfonylurea?

On insulin or a sulfonylurea, exercise can push blood sugar too low unless doses or food are adjusted. The ADA says medicine doses or carbohydrate may need adjusting, and sugar should be checked around exercise.7

Sulfonylureas are diabetes tablets such as glimepiride and gliclazide.7 With these or insulin, GLP-1 medicines raise the risk of low blood sugar (hypoglycaemia), sometimes severely.14,15 The Mounjaro label says lowering the sulfonylurea or insulin dose may cut that risk.15

The ADA's 2026 standards say activity can cause a hypo if the dose or carbohydrate isn't adjusted.7 People on these medicines may need carbohydrate if their sugar is under 90 mg/dL before exercise.7 A hypo can also come on hours after exercise.7

Without insulin or a sulfonylurea, the ADA says hypos are not common and no routine steps are usually advised.7 It suggests people with diabetes check their sugar during and after exercise.7

The Indian consensus advises against exercise when sugar is above 300 mg/dl or below 70 mg/dl.4 With advanced diabetic eye disease (proliferative retinopathy), hard aerobic or strength exercise may be unsafe, the ADA says.7

Signs of low blood sugar. The US Wegovy guide lists dizziness, sweating, shakiness, blurred vision and weakness.14 Hunger, headache, confusion and a fast heartbeat are on the list too.14

The diabetes route through this site is I have type 2 diabetes. Hypos get a full post: Can GLP-1 medicines cause low blood sugar?.

What about older adults?

Older adults get the same targets as younger adults. Guidelines add balance and strength work on 3 or more days a week.1,2

WHO's aim for that extra work is to prevent falls.1 The Indian consensus calls strength training "particularly beneficial" in older adults, because it may slow age-related muscle loss and falls.4 It advises older people against sudden starts and high-intensity exercise.4

S-LiTE only enrolled people aged 18 to 65.9 The Wegovy label's fracture figures for older adults are in our post on muscle loss.

Questions people ask

Can I go to the gym on Mounjaro or Ozempic?

The US Mounjaro and Ozempic labels describe each as an add-on to diet and exercise for type 2 diabetes.15,21 The only trial that added exercise to a GLP-1 used liraglutide, not either of them.13 A 2025 US advisory for people on GLP-1 medicines suggests strength training at least 3 times a week.6 ICMR-NIN advises people with diabetes or obesity to see a physician before starting an exercise programme.2

Is walking enough on a GLP-1?

Walking counts toward the aerobic target. But a 2025 US advisory says aerobic activity alone does less to protect lean mass.6 WHO and ICMR-NIN both add muscle-strengthening on 2 or more days a week.1,2

Does yoga count as exercise on a GLP-1?

ICMR-NIN counts yogasanas and pranayama as moderate activity.2 The Indian consensus encourages physically intensive yoga but says more research is needed.4 We found no trial of yoga alongside a GLP-1 medicine.

Can a GLP-1 replace exercise?

India's ESI guideline says these medicines do not remove the need for diet and regular exercise.17 In the S-LiTE trial, exercise plus liraglutide cut body fat about twice as much as either alone.8

How much exercise helps keep weight off after stopping?

Fit India's protocol says 60–90 minutes of moderate activity a day may be needed to keep weight off.3 In S-LiTE, adding supervised exercise to liraglutide meant 2.5 kg less regain after stopping, not significant.9

How we researched this

We read seven sets of guidance in full. Three were WHO 2020, ICMR-NIN 2024 and Fit India's protocol for ages 18 to 65. The others were the 2009 and 2012 Indian consensus papers, the 2025 US nutrition advisory and the ADA's 2026 standards.

We read the S-LiTE paper in NEJM and its one-year follow-up in full. The bone analysis, an older-adults trial and two 2026 reviews we read as abstracts. Side-effect figures come from the US Wegovy and Mounjaro labels.

We cut a small six-week tirzepatide and exercise study because we could not read its methods in full. We found no Indian trial of exercise with these medicines, and no trial of yoga with them. Muscle-loss numbers stay in our muscle post.

Before you move on

  • Guidelines put strength training next to about 150 minutes a week of moderate activity.
  • ICMR-NIN asks Indian adults for 30–60 minutes on at least 5 days, plus strength on 2.
  • The one big trial used liraglutide; exercise plus the medicine cut body fat the most.
  • On insulin or a sulfonylurea, exercise can cause hypos; doses or food may need adjusting.
  • ICMR-NIN lists breathlessness, pain, nausea and vomiting as signs to rest; Indian guidance stresses fluids.
