The short answer

In the big trials, adults without diabetes lost about 15% on semaglutide 2.4 mg, the Wegovy dose. Tirzepatide (Mounjaro) gave 15% to 21%. Both trials ran about 16 months, and dummy injections gave 2% to 3%. Ozempic's lower diabetes doses gave about 4 to 6 kg, against about 1 kg on a dummy. Most people regained weight after stopping.

Key takeaways

  • In the STEP 1 trial, adults without diabetes lost 15% of their weight on semaglutide 2.4 mg over 68 weeks, against 2% on a dummy injection.1
  • On semaglutide 2.4 mg, 44% felt sick (nausea), against 16% on the dummy injection.2
  • In SURMOUNT-1, tirzepatide gave average losses of 15% to 21% over 72 weeks, and 4% to 7% stopped because of side effects.3
  • People with type 2 diabetes lost less in STEP 2, about 10% on semaglutide, and about 1 in 3 of them lost under 5% of their weight.5,2
  • One year after stopping semaglutide in the STEP 1 extension, people had regained two-thirds of the weight they had lost.15

How much weight did people lose in the big trials?

Adults without diabetes lost about 15% of their weight on semaglutide 2.4 mg, the Wegovy dose, and 15% to 21% on tirzepatide.

Ozempic is the same medicine, semaglutide, at lower diabetes doses of 0.5 to 2 mg. In its US-label trials, those doses gave about 4 to 6 kg.7 In the 30-week trial, the dummy group lost about 1 kg.7 The Ozempic results are further down, and how the brands relate is in Is semaglutide the same as Ozempic?

The 15% figures come from two large trials with a dummy injection (placebo) group. Each ran about 16 months (68 and 72 weeks).1,3

In STEP 1, 1,961 adults without diabetes took weekly semaglutide 2.4 mg or a dummy for 68 weeks.1 Everyone also got advice on eating less and moving more.2

The semaglutide group lost 15% of their weight on average, and the dummy group lost 2%.1 In kilos, that was 15 kg against 3 kg.1

In SURMOUNT-1, 2,539 adults without diabetes took tirzepatide at 5, 10 or 15 mg, or a dummy, for 72 weeks.3 Average losses were 15%, 20% and 21% by dose, against 3% on the dummy.3 At the two higher doses, half or more lost at least 20% of their weight.3

Both trials took adults with a BMI of 30 or more, or 27 or more with a weight-related condition.1,3 Those are Western cut-offs.

The 2009 Indian consensus puts obesity at a BMI of 25 or more.29 You can check your own numbers with the BMI and waist tool.

What the averages mean for one person (80 kg example)

For an 80 kg person, a 15% loss is about 12 kg. The dummy group's 2% is under 2 kg.1 At the top tirzepatide dose, 21% is about 17 kg, against about 2.5 kg on the dummy.3

Those are averages, and people spread widely around them. In STEP 1, about half lost 15% or more, but about 1 in 6 lost under 5%.2

What it cost in side effects

Most side effects were in the stomach and gut. On semaglutide 2.4 mg, 44% felt sick (nausea) and 24% vomited, against 16% and 6% on the dummy.2 On tirzepatide 15 mg, 28% had nausea, against 8%.4

Most of these effects were mild to moderate and came during or just after the dose steps.17 Still, 4% to 7% on tirzepatide stopped because of side effects, against 3% on the dummy.3 In STEP 1, 4.5% stopped because of stomach effects, against 0.8%.1

The weight loss barely depended on feeling sick. Across STEP 1 to 3, people lost similar weight with or without stomach side effects.17 Under 1 percentage point of the extra loss came through those side effects.17

RisksThe risks, in brief
  • Stomach effects are common. Nausea 44% vs 16%, diarrhoea 30% vs 16%, vomiting 24% vs 6% (semaglutide 2.4 mg vs dummy, US label).2
  • Pancreatitis. Inflammation of the pancreas: stomach pain that is severe or won't go away, sometimes spreading to the back.2
  • Gallbladder problems. Gallstones were reported by 1.6% on semaglutide, against 0.7% on the dummy.2
  • Dehydration from vomiting or diarrhoea can damage the kidneys.2
  • Thyroid warning. Ruled out by the US label for anyone with a personal or family history of MTC (a rare thyroid cancer) or MEN 2; tell your doctor if this applies. Not in pregnancy.2

How do the trials compare?

The table puts each trial's medicine group beside its own dummy group, because comparing one trial with another can mislead.

Do people with type 2 diabetes lose as much?

No. In trials of people with type 2 diabetes, the same medicines gave smaller losses, about 10% on semaglutide and 13% to 15% on tirzepatide.

STEP 2 enrolled 1,210 adults with type 2 diabetes and a BMI of 27 or more.5 After 68 weeks, semaglutide 2.4 mg gave a 10% loss, against 3% on the dummy.5 About 1 in 3 on semaglutide lost under 5%.2

SURMOUNT-2 tested tirzepatide in 938 adults with type 2 diabetes for 72 weeks.6 Losses were 13% on 10 mg and 15% on 15 mg, against 3% on the dummy.6 Between 17% and 21% of the tirzepatide groups lost under 5%.4

Side effects came with these results too. In STEP 2, 64% on semaglutide 2.4 mg reported stomach effects, against 34% on the dummy.5 In SURMOUNT-2, fewer than 5% stopped tirzepatide because of side effects.6

In India, Ozempic is the diabetes brand. It launched in India in December 2025 for type 2 diabetes.18 The STEP weight trials used semaglutide 2.4 mg, the Wegovy dose, which launched in India in June 2025.20 Mounjaro (tirzepatide) is sold in India for diabetes and obesity.19

A 30-week trial of Ozempic's diabetes doses found 4 kg lost on 0.5 mg and 5 kg on 1 mg.7 The dummy group lost 1 kg.7

In a 40-week trial, 2 mg gave 6.4 kg against 5.6 kg on 1 mg.7 That gap was too small to be sure of.7

On those doses, 16% to 20% had nausea, against 6% on the dummy.7 Between 3% and 4% stopped because of stomach effects, against 0.4%.7

What do trials in Asian and Indian people show?

Trials in East Asian adults found losses close to the global trials, but we found no published India-only trial with a dummy group.

STEP 6 enrolled 401 adults in Japan and South Korea, a quarter of them with type 2 diabetes.8,2 Semaglutide 2.4 mg gave a 13% loss over 68 weeks, against 2% on the dummy.8 Stomach effects hit 59%, against 30%, and 3% stopped for side effects, against 1%.8

SURMOUNT-J tested tirzepatide in 225 Japanese adults without diabetes for 72 weeks.9 Losses were 16 and 21 percentage points more than the dummy, on 10 mg and 15 mg.9 Side effects were reported by 84% to 86% on tirzepatide and 69% on the dummy, mostly stomach effects.9

SURMOUNT-CN enrolled 210 Chinese adults, starting from a BMI of 24 with a weight-related condition.10 After 52 weeks, losses were 14% on 10 mg and 18% on 15 mg, against 2% on the dummy.10 Fewer than 5% stopped because of side effects.10

Indians did take part in the global trials. STEP 1 had 13 sites in India, STEP 2 had 16 and SURMOUNT-1 had 6.11 We found no published results for the Indian participants on their own.

What Indian clinics have reported

A Delhi hospital looked back at the records of 150 adults on semaglutide or tirzepatide. Of these, 74 had type 2 diabetes.12

After a median of 6.5 months, the median loss was 8%.12 The middle half of people lost between 5% and 14%.12

People without diabetes lost 11%, against 5% for people with it.12 Nausea was reported by 31%, and diarrhoea and constipation by 25% each.12 There was no dummy group, and looking back at records is weaker evidence than a trial.12

In Lucknow, 7 centres reviewed 71 adults with type 2 diabetes after 3 months of tirzepatide.13 They lost 4.7 kg on average, about 6% of their weight.13 But 22% stopped, most often because of stomach effects or cost.13

One Indian trial compared a generic semaglutide with Wegovy in 249 adults for 24 weeks.14 Both groups lost about 14%.14 It had no dummy group, and the generic's maker, Alkem, sponsored it.14

Open the check

How many people lose less than 5%?

In the trials, between about 1 in 10 and 1 in 3 people on these medicines lost under 5% of their weight.

Five percent matters because guidelines use it as a check. UK guidance says to consider stopping semaglutide if under 5% is lost after 6 months.28 India's ESI 2025 guideline suggests a 3-month trial to judge whether a medicine works and is tolerated.27

The same guideline says doctors must explain that results vary between people.27

Who lost less than 5% of their weight
Share of the medicine group, % (from the table above)
STEP trials from the US Wegovy label, SURMOUNT trials from the US Zepbound label. Delhi figures come from hospital records with no dummy group. Opened 5 Oct 2026.2,4,12

In the Delhi records, 46% of people with diabetes lost under 5%, against 11% of people without.12 On the dummy injection, 69% in STEP 1 and 66% in SURMOUNT-1 lost under 5%.2,4

What happens after people stop?

In both big trials that tested stopping, most people regained a large share of their lost weight within about a year.

The STEP 1 extension followed 327 people for a year after the 68 weeks of treatment ended.15 The semaglutide group had lost 17%, then regained 12 percentage points.15 In the authors' words, they "regained two-thirds of their prior weight loss".15

By week 120, 48% were still at least 5% below their starting weight, down from 86% at week 68.15 Most gains in blood pressure, blood sugar and blood fats also slipped back toward the start.15

SURMOUNT-4 gave everyone tirzepatide for 36 weeks, by which point they had lost 21%.16 Then half were switched to a dummy without knowing it.16 Over the next year, the switched group gained 14% while those still on tirzepatide lost another 6%.16

Only 17% of the switched group kept at least 80% of their loss, against 90% who stayed on tirzepatide.16 Before the switch, 14% had already stopped, most often because of side effects.4

What stopping involves, and how people plan for it, is in Guide 9: stopping and the long term. A deeper post on regain is What happens when you stop Ozempic or Mounjaro?.

Do any weight-loss pills actually work as well?

In trials, orlistat beat a dummy pill by about 2 to 3 kg, and the diabetes tablet Rybelsus by about 1 to 2 kg.23,31,32

Orlistat is the only pill among the obesity medicines available in India.22 Rybelsus is approved here for type 2 diabetes.21 A semaglutide tablet sold only in the US gave 14%, against 2% on a dummy.2

Each result below sits beside its own dummy group and its side effects. The trials differ in people, doses and length, so read each pill on its own.

Orlistat 120 mg (prescription)

Orlistat works in the gut. It blocks the enzymes that digest fat, so about 30% of the fat in a meal is not absorbed.30 It is one of four obesity medicines available in India.22

In an Indian trial of 80 adults, orlistat gave 4.65 kg over 24 weeks, against 2.5 kg on a dummy.23 In XENDOS, a 4-year trial of 3,305 people, it gave 5.8 kg against 3.0 kg.31 Only 52% on orlistat finished the 4 years.31

The side effects come from the fat that passes through. In year 1 of the US-label trials, 27% had oily spotting and 8% leaked stool. On the dummy, each was about 1%.30

In those trials, 8.8% stopped because of side effects, against 5.0%.30 Rare cases of severe liver injury have been reported.30

In the US, a 60 mg dose is sold without a prescription.33 The Indian pharmacy listings we checked mark the 60 mg capsule as a prescription drug.36 The full comparison is in Orlistat vs GLP-1 medicines.

Rybelsus (semaglutide tablets, for diabetes)

In India, Rybelsus is approved for type 2 diabetes.21 A 2026 Indian review says oral semaglutide is not formally approved for obesity.22

A 26-week US-label trial tested it in people with type 2 diabetes. The 7 mg dose gave 2.3 kg and 14 mg gave 3.7 kg.32 The dummy group lost 1.4 kg.32 The 7 mg gap over the dummy, 0.9 kg, was too small to be sure it was real.32

Of that trial's 703 people, 17% were Asian.32

On 14 mg, 20% had nausea, against 6% on the dummy.32 And 8% stopped because of stomach effects, against 1%.32

The 3 mg first-month dose does not control blood sugar.32 More on that dose is in What is Rybelsus 3 mg used for?

Wegovy tablets (25 mg, US)

A 25 mg semaglutide tablet is sold in the US as Wegovy tablets. In its 64-week trial, 307 adults lost 14%, against 2% on a dummy pill.2 We found no Indian approval for it for weight.22

About 1 in 4 lost under 5%, and 6.9% stopped for side effects, against 5.9%.2 Only 1% of that trial's people were Asian.2 The tablet question has its own post: Is there an Ozempic tablet?

Why are supplements and "fat burners" not on this list?

We left them out because none of our sources tested them against a dummy. In the US, the FDA says many weight-loss products sold as supplements have contained hidden prescription drugs.34

One, first flagged in 2019, still contained sibutramine when the FDA checked again in July 2026. Sibutramine can raise blood pressure and heart rate.35 Ayurvedic and homeopathic products have their own post: Do ayurvedic, Patanjali or homeopathic weight-loss medicines work?

Questions people ask

Are Ozempic before-and-after photos realistic?

A photo shows one person. In STEP 1, about half on semaglutide 2.4 mg lost 15% or more of their weight. About 1 in 6 lost under 5%.2

How much weight do people lose on Mounjaro?

In SURMOUNT-1, adults without diabetes lost 15% to 21% over 72 weeks, depending on the dose. The dummy group lost 3%. People with type 2 diabetes lost 13% to 15% in SURMOUNT-2.3,6

Do people with diabetes lose less weight on semaglutide?

Yes. In STEP 2, adults with type 2 diabetes lost 10% on semaglutide 2.4 mg. In STEP 1, adults without diabetes lost 15%.5,1

Do weight-loss pills work as well as the injections?

Not in the trials we found. Orlistat gave 4.65 kg against 2.5 kg on a dummy in an Indian trial. Rybelsus, a diabetes tablet, gave 2.3 to 3.7 kg, against 1.4 kg on a dummy. A 25 mg semaglutide tablet sold in the US gave 14%.23,32,2

What happens if someone loses less than 5%?

UK guidance says to consider stopping semaglutide if under 5% is lost after 6 months. India's ESI 2025 guideline suggests a 3-month trial to judge whether a medicine works and is tolerated.28,27

Does the weight come back after stopping Ozempic or Mounjaro?

Mostly, yes. A year after stopping semaglutide in the STEP 1 extension, people had regained two-thirds of the weight they lost. In SURMOUNT-4, people switched off tirzepatide regained 14% of their weight in a year.15,16

How we researched this

On 5 October 2026 we opened every trial summary on PubMed or Europe PMC. We read the full text where it was free. Label figures come from the US Wegovy, Zepbound and Ozempic labels, not Indian ones.

Zepbound is the US weight brand of tirzepatide, the medicine in Mounjaro. For "lost under 5%", we used the labels' tables that count everyone who was randomised, including people who stopped. We checked trial site lists on ClinicalTrials.gov to see which trials ran in India.

We could not open the full STEP 1 and STEP 2 papers. So STEP 2's rate of stopping for side effects is missing from the table. We found no published results for Indian participants alone.

For the pills, we read the US Xenical, Alli and Rybelsus labels, two orlistat trials and two FDA notices. We used no before-and-after photos or personal stories.

Before you move on

  • Trial averages were about 15% on semaglutide 2.4 mg, 15% to 21% on tirzepatide, and 2% to 3% on a dummy.
  • People with type 2 diabetes lost less, and between 1 in 10 and 1 in 3 people lost under 5%.
  • Side effects, mostly in the stomach, came with the benefit, and 4% to 7% on tirzepatide stopped because of them.
  • Most of the weight came back within a year of stopping.
