Losing weight is the main treatment. Guidelines link 5% weight loss to less liver fat, 7–10% to less inflammation and 10% or more to less scarring. In July 2026, India gave Wegovy (semaglutide 2.4 mg) a conditional approval for MASH with moderate-to-advanced scarring. We found no Indian approval for resmetirom.
Key takeaways
- European liver guidelines aim for at least 5% weight loss to cut liver fat, 7–10% to calm inflammation, and 10% or more to improve scarring.1
- A 2022 review of Indian studies found fatty liver in about 39% of adults; India's liver society found 17% of Indian patients with it had a BMI under 23.2,3
- In the ESSENCE trial, Wegovy (semaglutide 2.4 mg) cleared liver inflammation in 63% vs 34% on placebo, but nausea affected 36% vs 13%.4,5
- India's drug regulator gave Wegovy a conditional approval in July 2026 for MASH with moderate-to-advanced scarring; keeping it depends on part 2 of the ESSENCE trial.6,7
- Resmetirom (Rezdiffra) is approved for MASH in the US and EU; we found no Indian marketing approval, and a CDSCO panel rejected one trial plan in February 2026.8,9,10
- Every medicine on this page is prescription-only. A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe GLP-1 medicines.11 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.12
- The liver approvals cover one stage of disease: liver inflammation with moderate-to-advanced scarring, without cirrhosis. They do not cover fat in the liver alone.13,6
- All results below are group averages from trials. They can't tell you how your own liver will respond. Each medicine's harms sit next to its results.

What are MASLD and MASH?
MASLD is fat in the liver of someone with at least one metabolic risk factor, such as high blood sugar. MASH is the form with inflammation and damaged liver cells.14,1
Until 2023, doctors called these non-alcoholic fatty liver disease (NAFLD) and non-alcoholic steatohepatitis (NASH). An international expert panel chose the new names in 2023.14 Of the experts surveyed, 66% found the word "fatty" stigmatising and 61% found "non-alcoholic" stigmatising.14
The full name is metabolic dysfunction-associated steatotic liver disease. "Steatotic" means fatty. The definition needs at least 1 of 5 heart and metabolic risk factors.14
India's liver society, INASL, has adopted the new name.15
Alcohol gets its own label. People with MASLD who drink more now fall into a group called MetALD. That means 140–350 g of alcohol a week for women, or 210–420 g for men.14
Our answer on alcohol on Mounjaro or Ozempic covers drinking on these medicines.
Scarring (fibrosis) is graded F0 to F4. F0 means no scarring, and F4 is cirrhosis.16 The liver approvals below are for F2 and F3, moderate to advanced scarring.13
Fatty liver does not usually cause symptoms, says the NHS. Some people feel tired, unwell, or have discomfort under the right ribs.17
How common is fatty liver in India?
A 2022 review of 50 Indian studies estimated that about 39% of adults have fatty liver. It was 28% in average-risk adults and 53% in high-risk groups.2
The review pooled 23,581 adults. High-risk groups included people with obesity, diabetes or heart disease.2 It found about 35% in children too, using the older NAFLD definition.2
A normal weight doesn't rule it out. INASL defines "lean" fatty liver in Indians as a BMI under 23.3 Its pooled analysis of 17 Indian studies found 17% of patients with fatty liver were lean.3
About 6.5% of the general community had lean fatty liver.3

How much weight do you need to lose for fatty liver?
European guidelines aim for at least 5% weight loss to cut liver fat. They aim for 7–10% to calm inflammation, and 10% or more to improve scarring.1
India's INASL recommends a 7–10% loss through diet and exercise as the main treatment for people with overweight or obesity.3 The US AASLD says 3–5% improves fat, but more than 10% is generally needed for inflammation and scarring.18
A worked example. For a 90 kg person, 5% is about 5 kg. 7–10% is 6–9 kg, and 10% is 9 kg.
A 2015 study followed 293 people with biopsy-proven NASH through a year of diet and exercise.19 Among those who lost 5% or more, NASH cleared in 58%.19
Among those who lost 10% or more, NASH cleared in 90%, and 45% had less scarring.19
Only 30% of that study's patients lost 5% or more.19 AASLD notes that 10% or fewer reach effective weight loss in structured programmes at 1 year.18
INASL suggests eating 500–1,000 fewer calories a day.3 It also suggests 30–45 minutes of moderate exercise on at least 5 days a week, whatever your weight.3 Our post How much weight do people lose on Ozempic or Mounjaro? gives the trial averages for weight.
How do doctors check for scarring?
European guidelines start with a score worked out from routine blood tests (FIB-4). If scarring is still suspected, a liver stiffness scan (elastography) comes next.1
INASL recommends these non-invasive tests in everyday practice.3 A liver biopsy is still the most accurate test for MASH, but INASL says it isn't routinely needed.3
The MASH approvals are for F2–F3 scarring, so staging decides whether they apply.13,9 INASL says that without a biopsy, MASH medicines can be used if non-invasive tests show significant scarring without cirrhosis.3
Which medicines are approved for MASH?
Three medicines carry a MASH approval somewhere: semaglutide (Wegovy), resmetirom (Rezdiffra) and saroglitazar. We found saroglitazar's liver approval only in India. Which country approved what differs.6,8,20
| Medicine | India | US | EU |
|---|---|---|---|
| Wegovy (semaglutide 2.4 mg)Weekly injection | Conditionally approved for MASH: committee advice 23 Apr 2026, approval reported 17 Jul 20267,6. Emcure's brand Poviztra (same medicine) has the same approval21 | Approved 15 Aug 2025 (accelerated)22 | Approved 26 Mar 2026 (conditional), authorised for MASH under a separate brand name, Kayshild23 |
| Rezdiffra (resmetirom)Daily tablet | No marketing approval found10,24 | Approved 14 Mar 2024 (accelerated)8 | Approved 18 Aug 2025 (conditional)9 |
| SaroglitazarDaily tablet | Approved for NASH Mar 2020, NAFLD Dec 202020 | No approval found | No approval found |
| Tirzepatide (Mounjaro)Weekly injection | No liver approval found | No liver approval found25 | No liver approval found26 |
Wegovy and resmetirom approvals cover adults with MASH and moderate-to-advanced scarring (F2–F3), without cirrhosis, alongside diet and exercise. "Accelerated" and "conditional" approvals rest on early evidence and need confirmatory trials. India's Wegovy approval also depends on part 2 of ESSENCE. "No approval found" is our search on 5 Oct 2026, not an official statement.
A second brand carries the same approval. Emcure co-markets Novo Nordisk's semaglutide as Poviztra, with the same MASH use as Wegovy.21
Watch the names on generic packs. Generic semaglutide launched in India in March 2026. One maker, Alkem, sells it under a brand called Hepaglide.27
Alkem's launch release lists approval for type 2 diabetes and weight management, not for the liver.27 We found no report of a generic semaglutide approved for MASH in India.
Ozempic is not the same dose. In India, Ozempic comes in 0.25, 0.5 and 1 mg for diabetes.28 The liver trial used Wegovy's 2.4 mg dose.4
The US Ozempic label lists no liver use.29 Our short answer Does Mounjaro or Ozempic help fatty liver? will cover this.
What did the Wegovy liver trial find?
After 72 weeks, liver inflammation cleared in 63% of people on Wegovy (semaglutide 2.4 mg) vs 34% on placebo. Scarring improved in 37% vs 22%.4
Adults with MASH and F2–F3 scarring. First 800 people, at week 72.4
Inflammation cleared, scarring no worse4
Scarring improved, inflammation no worse4
Both at once4
Nausea, a side effect5
The ESSENCE trial enrolled 1,197 adults with MASH and F2–F3 scarring. These results cover the first 800 people, at week 72.4 Novo Nordisk, Wegovy's maker, funded it.4
People on Wegovy lost 10.5% of their body weight vs 2.0% on placebo.4
Stomach side effects were more common on Wegovy.4 Nausea affected 36% vs 13%, diarrhoea 27% vs 12%, constipation 22% vs 8% and vomiting 19% vs 6%.5 Side effects made 2.6% leave the trial, vs 3.3% on placebo.5
About 27% of people in the trial were Asian. Only 7% had a BMI under 25, and 3% had "lean" MASH.13 The US label says there is limited information in people with MASH and a BMI under 25.13
The approval is an early one. The US label says continued approval for MASH may depend on a confirmatory trial.13 Part 2 of ESSENCE, which looks at liver outcomes such as cirrhosis, is expected to report in 2029.22
India's expert committee set the same kind of condition. Continued approval here depends on part 2 of ESSENCE confirming safety and benefit.7
The US liver society updated its advice in November 2025. It covers semaglutide 2.4 mg for MASH with F2–F3 scarring, found by non-invasive tests rather than a biopsy.30 Diet and activity stay the cornerstone of treatment.30
It asks doctors to watch for kidney injury from dehydration, gallbladder disease and pancreatitis.30 It also lists thyroid C-cell tumours, worsening eye disease (retinopathy) and loss of lean (muscle) mass.30
The 2024 European guideline was written before ESSENCE. It did not yet recommend GLP-1 medicines as MASH treatments. It called them safe in MASH when used for diabetes or obesity.1
What about tirzepatide (Mounjaro)?
Tirzepatide, sold in India as Mounjaro, has finished only a phase 2 liver trial.31,32 Its US and EU labels list no liver use, and we found no Indian liver approval.25,26
The SYNERGY-NASH trial gave 190 adults with MASH and F2–F3 scarring weekly tirzepatide or placebo for 52 weeks.32 MASH cleared in 44%, 56% and 62% on 5 mg, 10 mg and 15 mg, vs 10% on placebo.32 Scarring improved in 55%, 51% and 51%, vs 30%.32
The commonest side effects were stomach problems, mostly mild or moderate.32 Eli Lilly, tirzepatide's maker, funded the trial. The authors say larger and longer trials are needed.32
A larger trial is under way. SYNERGY-Outcomes is a phase 3 trial of tirzepatide and retatrutide in about 4,500 adults with MASLD.33 It lists 20 sites in India and was recruiting on 5 Oct 2026.33
Our answer What is retatrutide? explains that medicine.
What is resmetirom, and is it in India?
Resmetirom (Rezdiffra) is a daily tablet that acts on a thyroid-hormone receptor in the liver to cut fat build-up. The US and EU approve it; we found no Indian approval.8,9
In the MAESTRO-NASH trial, 966 adults took 80 mg, 100 mg or placebo for 52 weeks.16 NASH cleared in 26% and 30%, vs 10% on placebo. Scarring improved in 24% and 26%, vs 14%.16
Madrigal Pharmaceuticals, its maker, funded the trial.16
On the EU label, diarrhoea and nausea are very common (more than 1 in 10). Itching is common (1 in 10 to 1 in 100). An inflamed gallbladder is rare (1 in 1,000 to 1 in 10,000).9
The US FDA flags liver toxicity, gallbladder problems and interactions with some cholesterol medicines (statins).8
In India, a CDSCO expert panel reviewed one firm's request to make and sell resmetirom on 11 Feb 2026. It called the proposed Indian phase 3 trial "not fit for purpose" and did not approve it.10
In May 2026, Torrent Pharma said it was running phase 3 trials in India.24 It aims to launch after the patent expires.24
Where do vitamin E, pioglitazone and saroglitazar fit?
India's liver society recommends vitamin E, pioglitazone and saroglitazar for biopsy-proven MASH. The 2024 European guideline does not recommend vitamin E or pioglitazone as MASH treatments.3,1
Vitamin E. In the PIVENS trial, 247 adults without diabetes took vitamin E 800 IU, pioglitazone or placebo for 96 weeks.34 The liver improved in 43% on vitamin E vs 19% on placebo, but scarring did not.34
INASL recommends vitamin E for people without diabetes.3 AASLD notes concerns about bleeding (haemorrhagic) stroke, and conflicting data on prostate cancer, at this high dose.18
Pioglitazone, a diabetes tablet, missed PIVENS's main goal: 34% vs 19%.34 NASH cleared in 47% vs 21%, says AASLD.18 Its harms include weight gain, thinner bones in women after menopause, worse heart failure and a debated bladder-cancer risk.18
Saroglitazar is an Indian tablet, taken as 4 mg once a day.20 India approved it for NASH in March 2020.20 In December 2020 it was approved for NAFLD with obesity, diabetes or abnormal blood fats.20
The European guideline says it improves liver fat and liver enzymes, and trials with liver biopsies are ongoing.1
A 16-week US trial, sponsored by Zydus Therapeutics, tested it in 106 people.35 On 4 mg, a liver enzyme (ALT) fell by 46%, vs a 3% rise on placebo.35 People gained 1.5 kg on average vs 0.3 kg on placebo.35
INASL rates its own saroglitazar recommendation "weak".3
What the guidance rules out. AASLD says metformin, ursodeoxycholic acid and silymarin should not be used as NASH treatments.18
Many people search for an ayurvedic medicine for fatty liver. The European guideline says herbal products and supplements cannot be recommended, for lack of evidence on benefit and safety.1
In an Indian registry, traditional and complementary medicines caused 14% of 1,288 cases of drug-induced liver injury.36 Our post Do ayurvedic, Patanjali or homeopathic weight-loss medicines work? covers the evidence.
When is a liver problem an emergency?
Fatty liver itself is usually silent. Advanced scarring (cirrhosis) can cause bleeding or confusion, and the NHS treats those signs as emergencies.17,37
Other signs of cirrhosis include yellow skin or eyes, bleeding or bruising easily, and itchy skin.37 Earlier signs can be tiredness, losing weight without trying, and red, blotchy palms.37
- Stomach effects are very common, more than 1 in 10 people on the EU Wegovy label. Nausea affected 44% vs 16% in Wegovy's US weight trials.38,13
- On the EU Wegovy label, pancreatitis is uncommon (1 in 100 to 1 in 1,000). Gallstones are common (1 in 10 to 1 in 100).38
- Dehydration can hurt the kidneys. The US Wegovy label reports kidney injury, mostly after vomiting or diarrhoea.13
- Thyroid warning. Ruled out by the US label for people with a personal or family history of medullary thyroid cancer (MTC) or MEN 2; tell your doctor if this applies.13 Pregnancy: the US label says to stop at least 2 months before a planned pregnancy.13
Our post on pancreatitis and gallbladder problems covers those two risks in depth.
Questions people ask
Can fatty liver be reversed by losing weight?
Often, at least partly. In a 2015 study, NASH cleared in 58% of people who lost 5% or more. It cleared in 90% of those who lost 10% or more. Only 30% reached 5% in a year.19
Is Wegovy approved for fatty liver in India?
For one stage of it, and conditionally. In July 2026, CDSCO approved Wegovy for adults with MASH and moderate-to-advanced scarring, without cirrhosis. Keeping the approval depends on part 2 of the ESSENCE trial. Emcure's Poviztra, the same medicine, has the same approval. It does not cover fat in the liver without that scarring.6,7,21
Does Ozempic help fatty liver?
Ozempic contains semaglutide, but in India it comes in 0.25, 0.5 and 1 mg doses for diabetes. The liver trial used Wegovy's 2.4 mg dose. The US Ozempic label lists no liver use.28,4,29
Is resmetirom available in India?
We found no Indian marketing approval as of 5 October 2026. A CDSCO expert panel rejected one firm's phase 3 trial plan in February 2026. Torrent Pharma said in May 2026 that it was running trials in India.10,24
Can thin people get fatty liver?
Yes. India's liver society found 17% of Indian patients with fatty liver had a BMI under 23. The US Wegovy label says there is limited information in people with MASH and a BMI under 25.3,13
How we researched this
On 5 October 2026 we read the liver guidelines from Europe (2024), the US (2023) and India (2023). We also read the November 2025 US update on semaglutide. We read the ESSENCE, SYNERGY-NASH, MAESTRO-NASH and PIVENS trials, a saroglitazar trial and the 2015 weight-loss study.
We checked approvals with the FDA, the EMA, CDSCO committee minutes, company releases and named news reports.
We found no Indian prescribing information for Wegovy's liver use online, so label figures are US and EU. The Indian approval comes from CDSCO committee minutes (23 Apr 2026) and Business Standard reports.
Several guideline and trial authors declare industry ties, and the sources list notes them.
Before you move on
- Losing 5% of your weight cuts liver fat; guidelines aim for 10% or more to improve scarring.
- In India, Wegovy (and Poviztra) has a conditional approval for MASH with F2–F3 scarring. We found no Indian approval for resmetirom; saroglitazar's liver approval, as far as we found, is India-only.
- Every medicine here has harms: stomach effects for Wegovy and tirzepatide, diarrhoea for resmetirom, weight gain for pioglitazone.
- Vomiting blood, black stools or sudden confusion in someone with cirrhosis means call 112.
