Ozempic, GLP-1 medicines and thyroid cancer: what does the boxed warning mean?

Ozempic, Wegovy and Mounjaro carry a thyroid tumour warning in the US. Here is where it came from, who it rules out, and what the studies in people found, in plain numbers.

Written for India. Reading from the US? US versionComing soon

The short answer

Nobody has shown that GLP-1 medicines cause thyroid cancer in people. They caused thyroid C-cell tumours in mice and rats, so US labels carry a boxed warning. Those labels rule out anyone with medullary thyroid cancer in themselves or family, or MEN 2. Human studies mostly find no clear rise, though they can't rule out a small one.

Key takeaways

  • Nobody has shown that GLP-1 medicines cause thyroid cancer in people; the US labels say the human relevance of the rodent tumours "has not been determined".1,2
  • The US labels for Ozempic, Wegovy, Rybelsus and Mounjaro rule out use for anyone with a personal or family history of medullary thyroid cancer, or with MEN 2.1,2,3,4
  • In a Scandinavian study of 145,410 users, thyroid cancer occurred at 1.33 cases per 10,000 people a year, against 1.46 on another diabetes medicine.12
  • A 2025 review of 48 trials put the effect at between 1 fewer and 9 more thyroid cancers per 10,000 people treated, over trials its authors call short.15
  • Europe's medicines regulator found in October 2023 that the evidence does not support a causal link, after reviewing studies including a French one that found a higher risk.7,11
Before you go further
  • GLP-1 medicines are prescription-only in India. A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.19 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.20
  • The thyroid warning is one of several. Pancreatitis, gallbladder problems and the rest are in Guide 5, Side effects and risks.
  • The July 2026 recalls named select batches of 3 generic semaglutide brands: Obeda, Semalix and Usema.22
  • This post explains a label warning and the studies behind it. It can't tell anyone their own risk.

What does the boxed warning say?

The US labels say these medicines caused thyroid C-cell tumours in rodents, and that nobody yet knows whether they do this in people.1,2

The warning sits in a box at the top of the US label for Ozempic.1 The same box is on the US labels for Wegovy, Rybelsus and Mounjaro.2,3,4 Ozempic, Wegovy and Rybelsus all contain semaglutide, as our answer on whether semaglutide is the same as Ozempic explains. The box makes three points.

  • Rodents got tumours. Semaglutide caused C-cell tumours in mice and rats, with more tumours at higher doses and with longer use.1
  • People are an open question. Whether it causes these tumours in humans "has not been determined".1
  • Some people must not use it. The US label rules it out for people with medullary thyroid cancer (MTC) in their own or family history. It also rules it out for people with MEN 2.1

A box can rest on animal data. US rules let the FDA require a boxed warning based on "serious animal toxicity" when human data are missing.6 So this box records an animal finding, with no proof of harm in people.

The label also asks doctors to explain the possible MTC risk and its symptoms to each patient.1

What happened in the animal studies?

In 2-year studies, mice and rats given semaglutide grew more thyroid C-cell tumours at exposures similar to human doses. Non-cancerous growths (adenomas) rose in both; cancers rose clearly in male rats.1

In rats, the non-cancerous growths rose at every dose tested.1 Tirzepatide, the medicine in Mounjaro, did the same in a 2-year rat study.2 In standard lab tests, semaglutide did not damage genes.1

The EU label for Ozempic says rodents are "particularly sensitive" to this effect.5 It calls the relevance for humans "low, but cannot be completely excluded".5

In December 2025, 3 UK thyroid and hormone societies explained why. They said the finding has not been repeated in monkeys or people.8 Human C cells have "minimal" activity at the docking points these medicines act on (GLP-1 receptors).8 Our GLP-1 full form answer explains the hormone these medicines copy.

What do "C-cell" and "medullary" mean?

The thyroid has two main kinds of cell, and medullary thyroid cancer (MTC) is the cancer that starts in the C cells.9,10

Follicular cells use iodine from the blood to make thyroid hormone.10 C cells make calcitonin, a hormone that helps keep blood calcium at a healthy level.9

MTC makes up less than 5% of all thyroid cancers.10 About 1 in 4 cases runs in families, and about 3 in 4 do not.10

An inherited change in a gene called RET can cause MTC.9 MEN 2 (multiple endocrine neoplasia type 2) is an inherited condition that raises the risk of MTC.9 The US National Cancer Institute also lists being Asian among the risk factors for thyroid cancer in general.9

So the US label's ban covers a small group. It means people with MTC in their own or family history, and people with MEN 2.1

An underactive thyroid is a different thing. Hypothyroidism is not on the US label's list of reasons not to use these medicines.1 UK societies cite a 2022 review that found no rise in hypothyroidism, thyroid lumps (nodules) or goitre with them.8 Our answer on GLP-1 medicines and hypothyroidism will cover this.

What did Europe's medicines regulator conclude?

In October 2023, Europe's medicines regulator found that "the available evidence does not support a causal association" with thyroid cancer.7

Its safety committee, called PRAC, read published studies plus trial and safety-report data from the makers.7 It covered exenatide, liraglutide, dulaglutide, semaglutide and lixisenatide.7 Tirzepatide was not on that list. The committee said no label change was needed, and told makers to keep watching.7

The EU and US labels differ here. The EU Ozempic label lists only allergy to the medicine as a reason not to use it.5 Its thyroid finding sits in the animal-study section.5 The US label bans use with MTC or MEN 2.1

The UK societies take a middle line. They see no sign of a clinically meaningful short-term rise in risk.8 They still advise avoiding these medicines after MTC, with a known RET gene change, or with MEN 2 in the family.8

What do studies in people show?

Most large studies find no clear rise in thyroid cancer. One French study found a higher risk, and some pooled trial data show a small, uncertain signal.11,12,13,14,15,16,17

Two kinds of number appear below. Relative risk compares two groups ("58% higher"). Absolute risk counts how many people it happened to ("1 in 10,000 a year").

StudyWho was studiedWhat it foundIn absolute numbers
France, 20232,562 people with thyroid cancer and 45,184 without, all with type 2 diabetes11After 1–3 years of use, 58% higher relative risk of thyroid cancer, and 78% higher for MTC11The paper reports relative risk only11
Scandinavia, 2024145,410 users vs 291,667 people on another diabetes medicine (a DPP-4 inhibitor)12No rise in thyroid cancer121.33 vs 1.46 cases per 10,000 people a year; MTC 4 cases vs 1112
International, 2025 (US data)460,032 users vs people on 3 other kinds of diabetes medicine13No rise in thyroid tumours against any comparison13About 9 to 10 thyroid tumours, cancerous or not, per 10,000 users a year13
64 trials, 2024Trials of GLP-1 medicines for diabetes or weight, each at least a year long1452% higher relative risk, resting on few cases141 extra case for every 1,349 people treated for 5 years14
48 trials, 202594,245 people in placebo-controlled trials, including trials of semaglutide and tirzepatide15Probably little or no effect (moderate certainty)151 fewer to 9 more cases per 10,000 people treated, over the trials' short follow-up (not per year)15
15 trials, 202684,237 people in trials of GLP-1 or dual-action medicines, including comparison groups16No significant rise; very low certainty16Not given as a rate. Pooled odds 52% higher, but anywhere from 14% lower to 2.7 times higher16
78 studies, 2026Real-world studies of GLP-1 users, pooled179% higher relative risk, not significant17Not reported in the summary17

"Not significant" means the result could easily be chance. The 2026 trial pool gave no rate per person, so we show its pooled result. Checked 5 Oct 2026.

Two rates that can't be compared. The international study counted all thyroid tumours, including non-cancerous ones, and only its US data passed the authors' checks.13 So its 9 to 10 per 10,000 can't be set beside the Scandinavian cancer rate.

What 1 in 10,000 looks like. In the Scandinavian study, about 1 in every 10,000 users got thyroid cancer each year.12 That was close to the rate on the comparison medicine.12

The French 58% sounds large, but it applies to a rare event. If it were applied to the Scandinavian comparison rate of 1.46 per 10,000 a year, it would mean about 2.3. That is our illustration, mixing two different groups of people, not a figure from either paper.11,12

The 2024 trial signal was fragile. Its authors gave it a "fragility index" of 1, meaning one case counted differently would have erased the result.14

Why don't the studies agree?

Thyroid cancer is rare, people on a new medicine get checked more often, and most studies follow users for only a few years.8,12,15

  • Looking harder finds more. The Scandinavian authors say the French result may reflect "detection bias", meaning cancers found sooner because users saw doctors more.12 The French rise appeared early and did not grow with longer use.12
  • The conditions themselves raise risk. Obesity and diabetes each raise thyroid cancer risk on their own, the UK societies note. That muddies studies of real-world users.8
  • Follow-up is short. In the Scandinavian study, semaglutide users were followed for 1.1 years on average.12 The trials in the 2025 review were not designed to look for cancer.15
  • MTC is too rare to measure well. The Scandinavian study saw just 4 MTC cases among GLP-1 users.12 Its MTC estimate ranged from about a third of the comparison risk to nearly 4 times it.12
  • Older medicines dominate the data. Liraglutide made up 57% of GLP-1 users in the Scandinavian study, and semaglutide 33%.12 Tirzepatide was not among the medicines studied.12

The Scandinavian authors put it plainly: their study "cannot exclude a small increase in risk".12 The 2026 trial review says the same about a clinically relevant rise.16

What does the Indian label say?

We could not find the Indian package insert for Ozempic, Wegovy, Rybelsus or Mounjaro online, so we can't confirm whether it carries the US warning.

Indian inserts do exist. In August 2025, CDSCO's expert committee recommended approval of an updated Rybelsus insert, aligned with the EU version.21 The news report did not publish the insert's text.21 The EU label has no MTC ban, so the Indian wording can't be guessed from the US one.5

India's regulator has asked about MTC in trials. On 20 Aug 2025, its expert committee reviewed trials of a generic semaglutide for diabetes and for weight.18 It asked for an ultrasound check for MTC before people join, and an MTC caution in the consent form.18

That was a condition for joining a trial. For patients, the US label calls routine calcitonin tests or thyroid ultrasound of "uncertain value". They can lead to unnecessary procedures.1

A government backgrounder of 1 Apr 2026 lists medullary thyroid cancer among the side effects of GLP-1 medicines.19 The US labels word it more carefully, saying it is unknown whether these medicines cause MTC in people.1 Our answer on whether Ozempic is available in India covers its approval here.

India note: US and EU labels used here. Every label quote on this page comes from the US or EU label, named each time. We found no Indian label text online. Guide 6 covers the brands approved in India.

Which symptoms does the label list?

The US Ozempic label lists a lump or swelling in the neck, hoarseness, trouble swallowing and shortness of breath as possible signs of thyroid cancer.1

This is the label's own list, and it tells patients to report these to their healthcare provider.1 Other conditions can cause the same signs, the US National Cancer Institute notes.9

The label also says a thyroid lump found on an exam or scan should be looked at further. The same goes for a high calcitonin result.1

Routine checks are a different matter. The US label calls routine calcitonin tests or ultrasound of uncertain value.1 The UK societies do not recommend them for people with no particular reason.8 Guide 4, Should I consider a GLP-1?, covers the other reasons these medicines may not suit someone.

Get help now: call 112. Swelling of the face, lips, tongue or throat, or trouble breathing or swallowing, can be a serious allergic reaction. The Ozempic label says to stop the medicine and get medical help right away.1

Questions people ask

Does Ozempic cause thyroid cancer?

Nobody has shown that it does in people. Semaglutide, the medicine in Ozempic, caused thyroid C-cell tumours in mice and rats. Large studies in people mostly find no clear rise, though they can't rule out a small one.1,12

Does Mounjaro carry the same warning?

Yes. The US Mounjaro label has the same boxed warning, based on thyroid C-cell tumours in rats. It also rules out use with a personal or family history of medullary thyroid cancer, or with MEN 2.2

Is the thyroid warning different for Rybelsus tablets?

No. The US label for Rybelsus and Ozempic tablets carries the same boxed thyroid warning as the injections. Our Rybelsus 3 mg answer covers the tablet itself.3

Does the warning apply to people with hypothyroidism?

An underactive thyroid (hypothyroidism) is not on the US label's list of reasons not to use these medicines. UK thyroid societies cite a 2022 review that found no rise in hypothyroidism with them.1,8

Is a thyroid scan or calcitonin test needed before treatment?

The US label says routine calcitonin tests or thyroid ultrasound are of uncertain value for finding medullary thyroid cancer early. UK thyroid societies do not recommend them for people with no particular reason. India's regulator asked for an ultrasound check before people joined one generic semaglutide trial.1,8,18

How we researched this

On 5 October 2026 we read the US labels for Ozempic, Wegovy, Rybelsus and Mounjaro, and the EU label for Ozempic. We searched each for every thyroid mention. We read the European Medicines Agency's 2023 review and the 2025 UK thyroid societies' statement.

We read the French and Scandinavian studies and a large international study that relied on US data. We also read 4 pooled reviews published up to March 2026, using the full text where it was open.

We searched for the Indian labels and checked CDSCO committee minutes and the 1 April 2026 government backgrounder. We could not find any Indian label text online. Where a study gave only relative risk, we say so. The one figure we worked out ourselves is labelled as our illustration.

Before you move on

  • The thyroid warning comes from rodent studies; whether it applies to people is unknown.
  • The US labels rule out use with a personal or family history of MTC, or with MEN 2.
  • In the Scandinavian study, about 1 in 10,000 users a year got thyroid cancer, close to the comparison medicine.
  • The label's symptoms are a neck lump or swelling, hoarseness, trouble swallowing and shortness of breath.
