How do you inject Mounjaro with the KwikPen, and use the Ozempic pen?

Which pens and vials are sold in India, and is there an Indian leaflet?

India has the Mounjaro KwikPen, Mounjaro single-dose vials, the Ozempic pen and Wegovy FlexTouch pens. We found no Indian instructions leaflet for any of them online.^9,11,12,15

Lilly launched Mounjaro in India in March 2025 as 2.5 mg and 5 mg single-dose vials.^9,10 The KwikPen followed in August 2025, in six strengths from 2.5 mg to 15 mg.⁹ Pharmacy listings in October 2026 still show single-dose vials next to the KwikPen.¹¹

Novo Nordisk sells Ozempic in India at 0.25 mg, 0.5 mg and 1 mg, in a pen.^14,15 Wegovy came in June 2025 in five strengths, also in a pen.¹³ A 2025 CDSCO alert names Indian Wegovy 0.25 mg, 0.5 mg and 1 mg pens as "Wegovy FlexTouch".¹²

The US also sells Mounjaro single-dose pens. Indian pharmacy listings don't show them, so we use their leaflet only where it answers a common question. Checked 5 Oct 2026.

Generic semaglutide makers sell their own reusable pens and vials.¹⁴ Each has its own leaflet, and we did not review them. The leaflet in your own box is the one that counts. Our post on semaglutide generics lists who makes what.

What do the Mounjaro KwikPen instructions say, step by step?

The instructions say: check the pen, fit a new needle, prime it, turn the dose knob until it stops, then push and count to 5. Here is each step as the US leaflet gives it.¹

1. Wash your hands with soap and water.¹
2. Pull the cap straight off and check the pen. Don't use it if the name or strength doesn't match your prescription, it has expired, or it looks damaged.¹ The medicine should be colourless to slightly yellow, never cloudy, frozen or with particles.¹
3. Wipe the red inner seal with an alcohol swab.¹
4. Fit a new pen needle. Push it straight on and twist until tight. Keep the outer needle shield; throw away the inner one.¹
5. Prime the pen. The next section has the detail.¹
6. Choose a site: thigh or belly, at least 5 cm (2 inches) from the navel. Someone else can use the back of your upper arm.^1,7
7. Set the dose. Turn the dose knob all the way until it stops and the dose symbol shows in the window. "Do not count clicks," the leaflet says.¹
8. Inject. Put the needle in, push the knob in until it stops, and slowly count to 5 while holding it.¹
9. Check. Pull the needle out. If the dose symbol isn't showing, put the needle back in and finish. Don't redial, and don't start again.¹
10. Tidy up. If you bleed, press lightly with cotton; don't rub. Put the outer shield back on, unscrew the needle into a sharps container, and recap the pen.¹

A drop of medicine on the needle tip afterwards is normal and "will not affect your dose".¹ If the knob is hard to push, the leaflet says to push more slowly. If the needle may be blocked, it says to fit a new one and prime again.¹

Four doses, then a new pen. Each KwikPen holds 4 weekly doses of 0.6 mL, plus a little extra for priming.^1,4 After the fourth dose, the pen won't let you dial a full dose. The leaflet says to throw it away "including the unused medicine".¹

The leaflet also says "Do not inject the leftover medicine" and "Do not transfer MOUNJARO from your Pen into a syringe".¹ The leaflet says never to share a pen, even with a new needle, because that can pass on a serious infection.¹ Lilly's label says a person who can't see well needs a trained helper.¹

Needles don't come with the KwikPen in the US or the EU.^1,7 We couldn't confirm what's inside an Indian box, so check yours.

Why does the KwikPen need priming every week, and what about air bubbles?

Priming pushes out air and proves the pen works. Lilly's KwikPen leaflet says to do it "before each weekly injection", not only the first time.¹

• Turn the dose knob slowly until you hear 2 clicks and see the prime line in the window.¹
• Hold the pen needle-up and tap the cartridge holder gently "to collect air bubbles at the top".¹
• Push the knob in, count slowly to 5, and look for a drop at the needle tip. A drop means the pen is primed.¹
• No drop? The leaflet says to repeat up to 2 more times. Still none? It says to change the needle and try once more, then call the maker.¹

The EU KwikPen leaflet has the same priming steps.⁷ Priming goes into the air, never into your body.¹

Air bubbles elsewhere. Lilly's single-dose pen leaflet says "Air bubbles are normal."² The US Ozempic prefilled syringe, a device we found no Indian listing for, has a leaflet saying bubbles "do not affect your dose" and not to try to remove them.⁵ With a vial and syringe, you tap bubbles to the top and push them out (next section).³

Warming the pen. Some how-to pages we checked say to take the pen out 30 minutes early. Lilly's single-dose pen leaflet says "It is not necessary to warm the Pen to room temperature."² The KwikPen leaflet doesn't mention warming.¹

How do the instructions say to inject from a Mounjaro vial?

The US single-dose vial leaflet says to draw 0.5 mL into a fresh syringe, push out the air bubbles, inject, and hold the needle in for at least 5 seconds.³

1. Check the vial. Right medicine, right strength, not expired, not frozen, cloudy or with particles.³
2. Prepare. Wash your hands. Flip off the plastic cap, but leave the rubber stopper in. Wipe the stopper with an alcohol swab.³
3. Fill the syringe with air. With a new needle on, pull the plunger back to the 0.5 mL line. Push the needle through the stopper and inject that air into the vial.³
4. Draw up the dose. Turn the vial and syringe upside down, keep the needle tip in the liquid, and pull past 0.5 mL.³
5. Remove bubbles. Tap the syringe so bubbles rise. Push the plunger up to exactly the 0.5 mL line, then pull the syringe out.³
6. Inject. Put the needle into your belly or thigh and push the plunger down. Keep it in for at least 5 seconds.³
7. Finish. "Do not recap the needle." It goes straight into a sharps container.³

The US label says to use a syringe that can measure the dose, such as a 1 mL syringe.⁴ In the US, the syringe and needle are sold separately.³ The US label says to use a new syringe and needle every time.⁴

One vial is one dose. The leaflet says to throw it away after use, "even if there is medicine left in the vial".³ If you switch from vials to the KwikPen, the label asks for training on the new device.⁴

What do the Ozempic pen instructions say, step by step?

The US Ozempic leaflet says to fit a new needle, remove both caps, check the flow with each new pen, dial your dose, then hold for 6 seconds.⁵

1. Check the pen. Read the name and coloured label, especially if you use more than one medicine. The liquid should be clear and colourless.⁵
2. Fit a new needle and remove both caps. "If you forget to remove both caps, you will not inject any medicine."⁵ Keep the outer cap.
3. Check the flow (new pen only). Turn to the flow-check symbol, hold the pen needle-up, and press until the counter shows 0. A drop should appear.⁵
4. Select your dose. Turn until the dose counter shows your dose, lined up with the pointer.⁵ "Do not set the dose by counting the number of clicks you hear."⁵
5. Inject. Wipe the skin and let it dry. Put the needle in, keeping the counter visible, and press until it shows 0.⁵
6. Hold for 6 seconds. Keep pressing and count slowly to 6. Pull out early and medicine may stream out, so you miss part of the dose.⁵
7. Remove the needle. Unscrew it into a sharps container without recapping it, and put the pen cap back on. With no container, the leaflet describes a one-handed recap into the outer cap. Never put the inner cap back on.⁵

The flow check. If no drop appears, the leaflet says to repeat it up to 6 times. Then change the needle and try once more. Still nothing means don't use the pen.⁵

The dose counter. It also shows what's left: turn the selector until it stops.⁵ If it stops short of your dose, there isn't enough for a full dose, and the leaflet says to use a new pen.⁵

If the counter never reaches 0 while you press, the needle may be blocked. The leaflet warns you have then "not received any" medicine, even though the counter moved.⁵ The leaflet says to change the needle and repeat from step 1.⁵

The US pen comes with NovoFine Plus 32G 4 mm needles.⁵ Never use a syringe to draw medicine out of the pen, and don't leave it in a hot car.⁵ We couldn't confirm how many doses each Indian Ozempic pen holds; the counter tells you.

How is the Wegovy FlexTouch pen different?

It works like the Ozempic pen, with a flow check per new pen and a 6-second hold, but each pen gives one fixed strength.^6,8

In the US, FlexTouch comes only as a 2.4 mg pen holding 4 doses. Lower US strengths are single-dose pens.⁶ In the EU, each FlexTouch pen holds 4 doses at every strength, from 0.25 mg to 2.4 mg.⁸ India's pens at 0.25, 0.5 and 1 mg are also called FlexTouch.¹² We couldn't confirm how many doses each Indian Wegovy pen holds.

• Flow check: before the first injection with each new pen only. If no drop appears, the leaflets say to check again, but "only … twice". Then change the needle once.^6,8
• Dose: turn until the counter stops at your dose. If it stops short, there isn't enough left, and the leaflet says to use a new pen.⁶
• Inject: press until the counter shows 0, then keep pressing and count slowly to 6.⁶
• Count your doses: the US leaflet asks you to use the table inside the carton lid.⁶ The US leaflet says the 2.4 mg pen goes after 4 doses, even if some medicine remains.⁶ We couldn't confirm the count for Indian pens.

Where do the instructions say to inject, and how do you rotate?

All three labels name the belly, thigh and upper arm, just under the skin. Each says to use a new spot every week.^1,5,6

• Belly: at least 5 cm (2 inches) from the navel.^1,6
• Thigh: the upper leg.^1,6
• Upper arm: Lilly's leaflets say another person should inject the back of your upper arm.^1,2 Novo's labels list the upper arm without that condition.^5,6

Rotating. You can stay in the same body area, but choose a different spot within it each week.¹ Lilly says rotating lowers the risk of pits, thickened skin and lumps under the skin.³ India's ESI 2025 obesity guideline also says patients must be taught to avoid fatty lumps (lipohypertrophy).¹⁶

Lilly's vial leaflet says not to inject where the skin is tender, bruised, scaly, hard, scarred, pitted or lumpy.³ The Mounjaro Medication Guide says not to inject into a muscle or a vein.⁴ If you also use insulin, the Mounjaro label says to give it separately. It can go in the same area, but not right next to the Mounjaro.⁴

How often skin reacts. In Mounjaro's US trials, 3.2% had an injection-site reaction, vs 0.4% on a dummy injection (placebo).⁴ The EU Mounjaro leaflet lists itching or redness as "common" (up to 1 in 10 people).⁷ For Ozempic, the US figure was 0.2%.⁵

How do you get rid of used needles and pens in India?

The US leaflets say used needles go straight into a hard, lidded container. Under India's 2026 waste rules, household needles are "special care waste", handed over separately.^1,17

The US leaflets say never to throw loose needles in household rubbish.^1,3 Without a proper sharps bin, they describe a household container that is:^1,5

• made of heavy-duty plastic;
• closed with a tight, puncture-resistant lid that sharps can't come through;
• upright and stable, and leak-resistant;
• labelled to warn that it holds hazardous waste.

The leaflets also say don't recycle that container.¹ A KwikPen with its needle removed can go in household rubbish, the US leaflet says.¹ The Ozempic and Wegovy leaflets put the used pen in the sharps container too.^5,6

The Indian rule. The Solid Waste Management Rules, 2026 were notified on 27 January 2026 and took effect on 1 April 2026.^17,18 They list "used or waste needles and syringes" from homes as special care waste.¹⁷

Homes must sort rubbish into four streams: wet, dry, sanitary and special care. Each goes to authorised waste collectors.¹⁷ Special care waste is picked up by authorised agencies or dropped at designated collection centres.¹⁸

City bodies must set up one drop-off centre per 5 square kilometres and publish its timings.¹⁷ Your municipal office or waste collector can tell you where yours is. We found no separate CPCB guide for household sharps.

How long does a pen last once you start it?

A KwikPen lasts 30 days from first use or 4 doses, whichever comes first. The US Ozempic and Wegovy FlexTouch leaflets allow 56 days.^1,5,6

• Mounjaro KwikPen: after first use, up to 30°C; throw away 30 days after first use or after 4 doses.^1,4
• Ozempic pen: in use for 56 days at 15–30°C, or in the fridge.⁵
• Wegovy FlexTouch (US): 56 days at 20–25°C or in the fridge. The leaflet says to throw it away if it has been above 30°C.⁶
