Where should you inject Ozempic or Mounjaro?

The short answer

The stomach, thigh or upper arm, under the skin, say the US and EU instructions for Ozempic (semaglutide) and Mounjaro (tirzepatide). For Mounjaro, the back of the upper arm is for someone else to inject. They say to use a new spot each week. Mounjaro's KwikPen adds 5 cm (2 inches) from the belly button.

  • GLP‑1 medicines such as Ozempic and Mounjaro are prescription-only in India, not sold over the counter.9
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.9 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.10,11
  • This page reports what the instructions say. They also say a healthcare provider should show you how to inject before your first dose.1
  • The risks are in Guide 5: Side effects and risks.

Which parts of the body do the instructions name?

The US and EU instructions for both medicines name 3 areas, the stomach (abdomen), the thigh and the upper arm, always just under the skin.

They say not to inject into a muscle or a vein.1,3 Mounjaro's KwikPen instructions say a stomach injection should be at least 5 cm (2 inches) from the belly button.3,5

The Ozempic pen instructions give no distance in words, though their drawing leaves the area around the belly button unshaded.1 The EU Ozempic leaflet names "the front of your thighs, the front of your waist (abdomen), or your upper arm".4 The 5 cm (2 inches) rule appears in the Ozempic prefilled-syringe instructions, a US and EU pack.1,4

In India, Mounjaro launched in vials and later as a KwikPen, and Ozempic comes in a pen.6,7 We found no public Indian leaflet, so this page uses the US and EU instructions. Daily life on injections is in Guide 8.

The US Ozempic pen instructions say to use a new needle every time. After the counter shows 0, they say to keep pressing and count 6 seconds.1 The Mounjaro vial instructions say the needle stays in for at least 5 seconds. They add that your provider should tell you whether to pinch the skin.3 Every step is in How do you inject Mounjaro with the KwikPen, and use the Ozempic pen?

Can you inject into your own upper arm?

For Mounjaro, the US label, patient guide, pen and vial instructions all say another person should inject the back of your upper arm.2,3

The EU label says the same.5 You or someone else can use the thigh or stomach, they say.3,5 The US label says people with poor eyesight should not use the KwikPen on their own.2

The Ozempic pen leaflets set no such limit. The US patient guide lists the "stomach (abdomen), thigh, or upper arm".1 Only the Ozempic prefilled syringe says the back of the upper arm needs "help from someone else".1,4

Does the injection site change how much medicine you absorb?

No, according to both US labels, which report "similar exposure" whether the injection goes into the stomach, thigh or upper arm.

Exposure means how much medicine reaches the blood. About 89% of an Ozempic (semaglutide) dose reaches it, and about 80% of a Mounjaro (tirzepatide) dose.1,2 The EU labels report the same, and the EU Ozempic label says the site "can be changed without dose adjustment".4,5

We found nothing in the US or EU labels that ranks one site above another, for weight loss or anything else.1,2,4,5

How should you rotate sites, and what about insulin?

The instructions say to pick a new spot each week, in the same area if you like, but not last week's spot.1,3

The Mounjaro vial instructions say rotating lowers the risk of pits, thickened skin and lumps. They also say not to inject into skin that is pitted, lumpy, bruised, tender, hard or scarred.3 India's ESI 2025 guideline says injection technique and how to avoid thickened skin (lipohypertrophy) must be explained.8

With insulin, both US labels say to give separate injections, never mixed. The same area is fine, but not side by side.1,2 The EU Mounjaro label says to use a different site from insulin.5

Skin reactions at the injection site were reported in 3.2% of people on Mounjaro vs 0.4% on a dummy injection.2 Ozempic's label gives 0.2%, with no placebo figure.1 These come from separate trial pools, not a head-to-head comparison.
