Mounjaro (tirzepatide) acts on two gut-hormone receptors, GIP and GLP-1; Ozempic (semaglutide) acts on GLP-1 only. In India, Mounjaro is approved for type 2 diabetes and weight management, Ozempic for type 2 diabetes only. In head-to-head trials, tirzepatide lowered weight and blood sugar more, with similar stomach side effects.
Key takeaways
- Mounjaro contains tirzepatide, which acts on both the GIP and GLP-1 receptors, while Ozempic contains semaglutide, which acts on the GLP-1 receptor only.1,2
- In India, Mounjaro is approved for type 2 diabetes and chronic weight management, while Ozempic's approval, reported in September 2025, covers type 2 diabetes only.3,5
- In the SURMOUNT-5 trial, adults without diabetes lost 20.2% of their weight on tirzepatide vs 13.7% on semaglutide, and 44% in each group had nausea.15,16
- Mounjaro's KwikPen prints an MRP of ₹13,125 at 2.5 mg and ₹25,781 at 15 mg; Ozempic's pen is ₹5,660 at 0.25 mg and ₹9,100 at 1 mg, each for 4 weekly doses.11,7,24
- Mounjaro's US label says it can make the contraceptive pill less reliable after starting and after each dose step; the US Ozempic label has no such advice.1,2

- Both are prescription-only medicines. A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.18 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.22,23

How do Mounjaro and Ozempic work differently?
Ozempic copies one gut hormone, GLP-1. Mounjaro acts on GLP-1 and a second gut hormone, GIP, so it has two targets.
GLP-1 (glucagon-like peptide-1) is a natural hormone. Semaglutide, the medicine in Ozempic, is 94% the same as human GLP-1.2 It switches on the GLP-1 receptor, the "lock" the hormone fits.2 Tirzepatide, the medicine in Mounjaro, switches on the receptors for both GIP and GLP-1.1
Both lower blood sugar the same two ways. They raise insulin and cut glucagon, but only when blood sugar is high.1,2 Both also slow how fast the stomach empties, and tirzepatide's label says it cuts how much people eat.1,2
Both last long enough for one injection a week. Tirzepatide's half-life is about 5 days; semaglutide's is about 1 week.1,2 Semaglutide stays in the blood for about 5 weeks after the last dose.2
Eli Lilly makes Mounjaro and Novo Nordisk makes Ozempic.3,6 Each medicine has a sister brand. Wegovy is the same semaglutide, prescribed in India for weight.7 Yurpeak is the same tirzepatide, made by Lilly and sold by Cipla.13 The hormone itself is covered in what GLP‑1 medicines do, and the newer medicine in What is tirzepatide? The semaglutide pair gets its own post: Wegovy vs Ozempic.
What is each one approved for in India?
Mounjaro is approved in India for type 2 diabetes and long-term weight management. Ozempic is approved for type 2 diabetes only.
Lilly launched Mounjaro on 20 March 2025, after marketing approval from CDSCO, India's drug regulator.3,4 The approval covers blood-sugar control in adults with type 2 diabetes.3 It also covers long-term weight management in adults with a BMI of 30 or more.3 At a BMI of 27 or more, a weight-related condition is also needed.3
Those BMI figures are the label's wording. What BMI and waist size mean for Indian bodies is in Guide 1: weight and health in Indians.
CDSCO's approval of Ozempic was reported on 26 September 2025, for type 2 diabetes alongside diet and exercise.5 The same report said Ozempic "is not approved for obesity management".5 Novo Nordisk launched it in India on 12 December 2025.6 Our short answers Is Mounjaro available in India? and Is Ozempic available in India? cover each brand's status. Business Standard noted it is widely prescribed "off-label" for weight loss, meaning outside its approved use.6

What did head-to-head trials find, harms included?
Two trials compared the medicines directly. Tirzepatide lowered weight more in both, and stomach side effects were common in both groups.
SURPASS-2, people with type 2 diabetes. It followed 1,879 adults for 40 weeks.14 It compared tirzepatide 5, 10 or 15 mg with semaglutide 1 mg, the top Ozempic strength sold in India.14,6
- Blood sugar. HbA1c, the long-term blood sugar test, fell 1.86 points on semaglutide. On tirzepatide it fell 2.01 to 2.30 points, depending on dose.14
- Weight. People on tirzepatide lost 1.9 to 5.5 kg more than those on semaglutide. The higher the dose, the bigger the gap.14
The harms in SURPASS-2 ran close. Nausea hit 17–22% on tirzepatide and 18% on semaglutide.14 Vomiting hit 6–10% and 8%.14 Serious adverse events were 5–7% vs 3%.14 Very low blood sugar (below 54 mg/dL) was 0.2–1.7% vs 0.4%.14
SURMOUNT-5, people with obesity but no diabetes. It followed 751 adults for 72 weeks.15 Semaglutide went up to 2.4 mg, the Wegovy dose, not Ozempic's 1 mg.15 Average weight change was −20.2% on tirzepatide and −13.7% on semaglutide.15 Waists shrank by 18.4 cm (7.2 inches) and 13.0 cm (5.1 inches).15
For a 90 kg person, those averages work out to about 18 kg and 12 kg. They are group averages, not a forecast for one person.
Other side effects in SURMOUNT-5 were close too. Diarrhoea hit 88 people in each group (24% on tirzepatide, 23% on semaglutide), and constipation hit 27% on tirzepatide and 28% on semaglutide.16 Serious adverse events were 5% on tirzepatide and 3% on semaglutide.16 Six people in each group left the trial because of an adverse event.16 Stopping the medicine is counted apart from leaving the trial. Stomach side effects made 2.7% stop tirzepatide and 5.6% stop semaglutide.21 Most stomach effects were mild to moderate and came during dose increases.15
- Stomach effects are common on both. Nausea, diarrhoea, vomiting, constipation and stomach pain lead both labels' lists. Losing fluid from vomiting or diarrhoea can injure the kidneys.1,2
- Pancreatitis. Both labels warn of it. The sign is stomach pain that is severe or won't go away, sometimes reaching the back.1,2
- Gallbladder problems, such as gallstones, are in both labels.1,2
- Low blood sugar. Both labels warn the risk rises when either is taken with insulin or a sulfonylurea, a type of diabetes tablet.1,2
- Eye disease in diabetes. Both labels say people who already have diabetic retinopathy, damage to the back of the eye, should be checked for it getting worse.1,2
- Thyroid warning. The US labels rule out both for people with a personal or family history of MTC, a thyroid cancer, or MEN 2; tell your doctor if this applies.1,2 Pregnancy: the Ozempic label says to stop it 2 months before a planned pregnancy.2 Mounjaro's pregnancy data are too limited to judge.1
Is there Indian data comparing the two?
One Indian study compares them. It looked back at the records of 150 patients at one Delhi hospital over 6 months.
The hospital was Max Super Speciality Hospital, Saket, in New Delhi.17
Median weight loss was 8.6% on tirzepatide and 5.6% on semaglutide.17 The groups were uneven: 117 people on tirzepatide, 33 on semaglutide.17 Semaglutide doses went up to 2.4 mg, so the group was not only Ozempic users.17
People without diabetes lost more than people with it: 11.2% vs 5.5%.17 Nausea affected 31%, diarrhoea and constipation 25% each, and vomiting 11%.17 No one needed to stop for a severe side effect.17
The authors list the limits: one centre and records looked at after the fact.17 There was no data on diet, exercise or missed doses.17 If you have diabetes, the type 2 diabetes path covers HbA1c and other medicines.
How do the dose steps differ?
Ozempic has three strengths in India and Mounjaro has six. Both start low and step up after at least 4 weeks.
More detail per brand is coming in Mounjaro doses explained.
What do Mounjaro and Ozempic cost in India?
Printed MRPs on 5 October 2026: Mounjaro ₹13,125 to ₹25,781 a pen, Ozempic ₹5,660 to ₹9,100. Each pen holds 4 weekly doses.
Ozempic. Novo Nordisk announced cuts to Ozempic's prices on 30 March 2026, as generic semaglutide reached the market.7 The 0.25 mg dose fell from ₹8,800 to ₹5,660 a month.7 Reuters reported on 1 April that the 1 mg weekly shot now costs ₹2,275, citing Novo Nordisk.24 That is ₹9,100 for a 4-dose pen, the top of the range. Pharmacy listings print ₹8,100 on the 0.5 mg pen.12
Mounjaro. The KwikPen launched in August 2025 at ₹14,000 (2.5 mg) to ₹27,500 (15 mg) for a month's pen.8 GST on most medicines then fell from 12% to 5%.9 NPPA, the drug price regulator, told makers to revise MRPs from 22 September 2025.10 Today's printed MRPs of ₹13,125 and ₹25,781 equal the launch MRPs with that tax cut applied (our arithmetic).8,11 We found no Lilly statement on it. Pharmacy listings print the full KwikPen ladder: ₹13,125 (2.5 mg), ₹16,406 (5 mg), ₹20,625 (7.5 and 10 mg) and ₹25,781 (12.5 and 15 mg).11
The 5 mg vial shows the same pattern: ₹4,375 at launch, ₹4,102 printed now.4,11 Cipla said Yurpeak would cost the same as Mounjaro, but published no figure.13
Per weekly dose, that is ₹3,281 to ₹6,445 for Mounjaro and ₹1,415 to ₹2,275 for Ozempic (our division).7,11,24 A year is 52 doses, or 13 pens. A full year at 15 mg of Mounjaro is ₹3,35,153 (13 × ₹25,781); at 1 mg of Ozempic, ₹1,18,300 (our arithmetic). These doses are not equal in strength, and your dose decides your bill.
Medicine is not the only cost. An endocrinologist visit had a median listed fee of ₹1,100 across 59 doctors on Practo in 6 cities.20 Blood tests come on top. Generic semaglutide brands exist too; what changed when the patent expired covers them, without rankings.
Pick a medicine and dose steps; it adds MRPs, doctor visits and tests. No sign-up.
How do the pens and vials differ day to day?
Ozempic comes as a pen holding 4 doses. Mounjaro comes as a 4-dose KwikPen or a single-dose vial.12,4,8
Heat. Once in use, an Ozempic pen keeps for 56 days at 15–30 °C or in the fridge.2 An in-use KwikPen can stay at up to 30 °C. It is thrown away after 30 days or 4 doses.1 A Mounjaro single-dose vial can sit out of the fridge below 30 °C for up to 21 days.1 Both labels stop at 30 °C, which many Indian rooms pass in summer. Storing your pen in Indian heat goes into this.
Sharing. Never share a KwikPen with anyone, even with a new needle.1
Missed doses. An Ozempic dose can be taken within 5 days.2 A Mounjaro dose can be taken within 4 days; after that, skip it.1
The contraceptive pill. Mounjaro can make the pill less reliable.1 Its label advises a non-pill method, or adding a barrier method, for 4 weeks after starting and after each step.1 The US Ozempic label has no such advice.2 More in GLP-1 medicines, pregnancy and the pill.
Questions people ask
Is Mounjaro stronger than Ozempic?
In two head-to-head trials, tirzepatide lowered weight and HbA1c more than semaglutide. In SURMOUNT-5, average weight change was −20.2% vs −13.7% over 72 weeks. Both trials were funded by Eli Lilly, Mounjaro's maker. Nausea hit 44% in each group in SURMOUNT-5.14,15,16
Is Ozempic approved for weight loss in India?
No. CDSCO approved Ozempic for type 2 diabetes. Wegovy is the semaglutide brand prescribed in India for obesity and weight management.5,7
Are Mounjaro and Yurpeak the same medicine?
Yes. Both are tirzepatide made by Eli Lilly. Cipla sells Yurpeak and said its price would be the same as Mounjaro's.13
Ozempic vs Mounjaro: which has more side effects?
Stomach side effects were common on both. In SURMOUNT-5, 44% had nausea in each group, and vomiting hit 15% on tirzepatide vs 21% on semaglutide. Stomach effects made 2.7% stop tirzepatide and 5.6% stop semaglutide. Serious adverse events were 5% vs 3%.16,21
Why is Mounjaro's MRP lower than its ₹14,000 launch price?
GST on most medicines fell from 12% to 5% on 22 September 2025, and makers had to revise their MRPs. The 2.5 mg KwikPen now prints ₹13,125, which matches that tax change.9,10,11
Is Ozempic 2 mg sold in India?
Ozempic launched in India in 0.25 mg, 0.5 mg and 1 mg strengths. The 2 mg dose appears in the US label.6,2
How we researched this
We read the US labels for Mounjaro (August 2026) and Ozempic (May 2026); we found no Indian pack leaflet online. We read the SURPASS-2 and SURMOUNT-5 papers' abstracts, SURMOUNT-5's registry results, an American College of Cardiology summary of SURMOUNT-5, and the Indian study in full.
We checked each MRP on 5 October 2026. Ozempic's range and its 1 mg price come from dated news reports. The 0.5 mg price and Mounjaro's MRPs are printed MRPs on at least two pharmacy listings. We name those listings but do not link them. We found no maker statement after the GST change. We never use discounted selling prices.
We left out two things we could not confirm. The SURPASS-2 abstract gives no kilos lost per group. We found no source on why acting on GIP adds to the effect.
Before you move on
- Mounjaro (tirzepatide) acts on GIP and GLP-1; Ozempic (semaglutide) acts on GLP-1.
- In India, Mounjaro is approved for type 2 diabetes and weight; Ozempic for type 2 diabetes.
- Tirzepatide lowered weight more in two Lilly-funded trials; stomach side effects were common on both.
- MRPs are per 4-dose pen, and the dose a doctor sets decides the cost.
