What happens when you stop Ozempic or Mounjaro?
Side effects, weight gain, blood sugar, tapering and restarting: semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro).
Written for India. Reading from the US? US versionComing soon
Weight, blood sugar and blood pressure usually drift back up. In one tirzepatide (Mounjaro) trial, people switched to a dummy injection regained about half their lost weight within a year. The labels list no withdrawal syndrome and no taper plan. Stomach side effects became less common after stopping.
Key takeaways
- Weight usually comes back after stopping, and in SURMOUNT-4 people switched from tirzepatide 10–15 mg to a dummy injection gained back 14% of their body weight in a year.10
- The US and EU labels for Ozempic and Mounjaro list no withdrawal syndrome, but the EU leaflets warn that blood sugar can rise after stopping.1–4
- Stomach side effects were less common after switching to a dummy injection, with 3% feeling sick against 8% of people who kept taking tirzepatide in SURMOUNT-4.10
- Blood pressure climbed back in the STEP 1 extension: the average top reading was 129 at the start, 121 on semaglutide 2.4 mg (Wegovy), and 131 a year after stopping.8
- No label we read gives a taper schedule; only the Wegovy labels deal with restarting after several missed doses, with a lower restart dose (US) or one to consider (EU).1–7

What side effects can you get after stopping Ozempic?
The medicine's own side effects usually become less common. The main changes people notice are hunger coming back and, with diabetes, higher blood sugar.
- Semaglutide and tirzepatide are prescription-only in India.20
- A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.20 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.21,22
- Stopping, lowering or restarting a dose is your doctor's call. The figures below come from US and EU labels and trials, not an Indian label.
- Risks while you take these medicines: Guide 5.
- No dose or brand guarantees any result.
Ozempic is semaglutide. In India it is sold for type 2 diabetes in 0.25, 0.5 and 1 mg strengths.23 Wegovy is the same molecule at up to 2.4 mg, for weight.25 Mounjaro is tirzepatide, sold in India for both weight and type 2 diabetes.24
The medicine leaves slowly. The US Ozempic label says semaglutide stays in the blood for about 5 weeks after the last dose.1 For Wegovy 2.4 mg it says 5 to 7 weeks.5 Tirzepatide has a shorter half-life (the time for half of it to leave the blood) of about 5 days.3
We found no stopping trial at Ozempic's 0.25–1 mg diabetes doses. The best data come from two weight trials at higher doses. In both, some people switched to a dummy injection (placebo) while others carried on.9,10
| Side effect after the switch | STEP 4: kept Wegovy 2.4 mg | STEP 4: switched to dummy | SURMOUNT-4: kept tirzepatide | SURMOUNT-4: switched to dummy |
|---|---|---|---|---|
| Feeling sick (nausea) | 14% | 5% | 8% | 3% |
| Vomiting | 10% | 3% | 6% | 1% |
| Diarrhoea | 14% | 7% | 11% | 5% |
New events reported after the switch. STEP 4: semaglutide 2.4 mg, 48 weeks after a 20-week start, 803 people.9 SURMOUNT-4: tirzepatide 10 or 15 mg (the Zepbound label in the US), 52 weeks after a 36-week start, 670 people.10 Rounded to whole numbers. Both trials excluded people with diabetes.
Stomach effects did not vanish on the dummy injection, but they were 2 to 5 times less common. STEP 4 counted only problems that began after the switch, to allow for the medicine's long half-life.9
Your heart rate may settle. Tirzepatide raised the average resting pulse by 5 beats a minute in SURMOUNT-4. After the switch to a dummy injection, it fell by 5 again.7
Hunger comes back. In one study of 50 people who lost weight by dieting, hunger was still higher a year later. Their appetite hormones had not gone back to normal.14 That was a diet study, not a GLP-1 study.
Is there an Ozempic or Mounjaro withdrawal?
Not in the labels we read. None lists a withdrawal syndrome, but the EU leaflets warn that blood sugar can rise once you stop.
We read the US labels for Ozempic, Wegovy, Mounjaro and Zepbound, and the EU labels for Ozempic, Wegovy and Mounjaro. We searched each one for withdrawal, stopping and tapering. We found no withdrawal syndrome and no taper schedule in any of them.1–7
The EU leaflets do say what to expect. For Ozempic: "Do not stop using this medicine without talking to your doctor. If you stop using it, your blood sugar levels may increase."2 For Mounjaro, the leaflet says blood sugar can increase if you have type 2 diabetes.4 The Wegovy leaflet says only not to stop without talking to your doctor.6
One 2025 review called the return of weight, blood sugar and blood pressure "metabolic rebound".13
In the STEP 1 extension, researchers did not collect side effects in a systematic way after people stopped.8
How much weight comes back after stopping Mounjaro or Ozempic?
In the trials, about half to two-thirds of the lost weight came back within a year. On average, people stayed somewhat below where they started.
Tirzepatide (Mounjaro). In SURMOUNT-4, 783 adults without diabetes started tirzepatide, building up to 10 or 15 mg. The 670 still in the trial at 36 weeks had lost 20.9% of their weight on average.10 Then half switched to a dummy injection for a year. They gained back 14.0% of their body weight. Those who kept taking tirzepatide lost another 5.5%.10
The 670 adults still in the trial at 36 weeks. After that, half switched to a dummy injection for a year.10
Weight lost, compared with the start
After switching, people gained back 14.0% of their body weight in a year.
That is about half the loss back: 9.9% net against 20.9% lost (our arithmetic). In kilos, the dummy-injection group weighed about 86 kg when they stopped.7 A 14% gain takes that to about 98 kg (our arithmetic). They had started near 107 kg.7
Results varied a lot. Of 308 people who had lost at least 10% before the switch, 83% regained at least a quarter of their loss. About 1 in 4 regained three-quarters or more. About 1 in 6 regained less than a quarter.11
Semaglutide (the Wegovy dose). The STEP 1 extension followed 327 people for a year after they stopped semaglutide 2.4 mg or a dummy injection. After 68 weeks, the semaglutide group had lost 17.3% on average. A year later they had regained 11.6 percentage points, ending 5.6% below their start.8 The authors call that two-thirds of the loss.8 For a 90 kg person, that is about 16 kg lost, then about 10 kg back.
In STEP 4, people switched to a dummy injection after 20 weeks of semaglutide 2.4 mg. They gained 6.9% over the next 48 weeks.9 These are Wegovy-dose results; Ozempic in India goes up to 1 mg.23
How fast? A 2026 BMJ review pooled 37 studies of 9,341 people. After semaglutide or tirzepatide, weight came back at about 0.8 kg a month.12 It projected a return to starting weight about 1.5 years after stopping. That is an estimate, because no study of these two medicines followed people for longer than a year.12
Getting to the weight lost also had a cost. In SURMOUNT-4, 14% of people stopped tirzepatide before week 36, most often because of side effects.7 In STEP 4, 71% had a stomach or gut problem in the first 20 weeks.9 More on results in how much weight people lose on Ozempic or Mounjaro.
What happens to blood sugar and HbA1c after stopping?
It rises again. The rise was small in people without diabetes, and larger in diabetes trials, though those used older GLP-1 medicines.
HbA1c is a blood test showing your average sugar over about 3 months. Our HbA1c explainer covers what the numbers mean.
Without diabetes. In the STEP 1 extension, average HbA1c started at 5.7%. It was 5.2% after 68 weeks on semaglutide 2.4 mg, and 5.6% a year after stopping.8 In SURMOUNT-4, HbA1c rose 0.3 points on the dummy injection, against no change on tirzepatide.7
The more weight people regained, the more HbA1c rose. In SURMOUNT-4, it went up by 0.14 points in those who regained least and 0.35 in those who regained most.11
With type 2 diabetes. A 2025 review pooled diabetes trials where a GLP-1 medicine was stopped. HbA1c rose by 0.65 points on average, from only 4 small trials of 219 people, with widely varying results.13 For someone at 7.0%, that is roughly 7.7%. Those trials used older medicines such as liraglutide and exenatide, not Ozempic or Mounjaro.13
What happens to blood pressure after stopping?
It goes back up, roughly to where it started, within a year. People who regained more weight saw bigger rises.
In the STEP 1 extension, average top (systolic) blood pressure was 129 at the start. It fell to 121 on semaglutide 2.4 mg, then was 131 a year after stopping. The dummy-injection group also ended above its start (130, then 132), so not all of the rise came from stopping.8
In SURMOUNT-4, top blood pressure fell from 126 to 115 on tirzepatide. It then rose by 8 points on the dummy injection, against 2 points for those who carried on.7 Among people who stopped, the rise ran from 7 to 10 points, depending on how much weight came back.11
The BMJ review projected that heart and sugar markers return to their starting levels within about 1.4 years.12 That figure pooled all weight-loss medicines. The review had too little data to work it out for semaglutide and tirzepatide alone.12

Should you taper off Ozempic or Mounjaro, or stop at once?
The labels say nothing about tapering, and WHO made no recommendation on how to stop. One trial supports a lower tirzepatide dose; a semaglutide taper trial has not started.
In the trials above, people stopped all at once by switching to a dummy injection.9,10 No label we read gives a step-down plan.1–7
The WHO's December 2025 guideline looked at the evidence on stopping. It found it "insufficient to inform a clear recommendation or clinical management approach", including on maintenance doses.18 The authors of the 2025 rebound review wrote that guidelines give no specific advice on tapering.13
A lower dose of tirzepatide. SURMOUNT-MAINTAIN followed 378 US adults who had lost weight on tirzepatide 10 or 15 mg. For a year they took their full dose, 5 mg, or a dummy injection.16 By 112 weeks they were 21.9%, 16.6% and 9.9% below their starting weight.16 Eli Lilly, which makes tirzepatide, paid for the trial, and 67% of people were White.16
Some people regained at least half their loss and were given tirzepatide again. That happened to 67% on the dummy injection, 25% on 5 mg and 8% on the full dose.16
Semaglutide. A randomised trial of a semaglutide taper is registered in China. As of August 2026 it was not yet recruiting.17
When stopping is advised. NICE in the UK says to consider stopping semaglutide if under 5% of weight is lost after 6 months.19 The EU labels say to stop semaglutide at least 2 months before a planned pregnancy. For tirzepatide it is at least 1 month.2,4 See pregnancy and the pill on GLP-1s.
How do you restart Ozempic or Mounjaro after a gap?
Only the Wegovy labels cover a long gap. The US label restarts the dose steps lower after 2 or more missed doses in a row; the EU label says a lower restart dose should be considered.
- Ozempic (US label): take a missed dose within 5 days. After 5 days, skip it and take the next one on your usual day.1
- Mounjaro (US label): take a missed dose within 4 days. After 4 days, skip it.3
- Wegovy (US label): if 2 or more doses in a row are missed, "reinitiate dosage escalation at a lower dosage". The reason given is to lower the risk of stomach side effects.5
- Wegovy (EU label): if more doses are missed, a lower restart dose "should be considered".6
We found no rule for restarting after a long break in the US Ozempic, Mounjaro or Zepbound labels.1,3,7 Their starting doses are 0.25 mg (Ozempic) and 2.5 mg (Mounjaro).1,3 The restart dose is a prescription decision. The short-gap rules are in what to do if you miss a dose.
What helps keep weight off after stopping?
Staying on the medicine kept weight off best in trials, with its side effects. Evidence on exercise comes from one small liraglutide trial.
Exercise. A Danish trial gave people a year of supervised exercise, liraglutide 3 mg (an older daily GLP-1), both, or neither, during treatment. Then everything stopped. Over the next year, liraglutide alone was followed by 2.5 kg more regain than liraglutide plus exercise, a gap too small to rule out chance. Exercise alone (no medicine) beat liraglutide alone by 6.0 kg.15 Only 109 people came to that follow-up, and it was not a semaglutide or tirzepatide trial.15 Exercise on Mounjaro or Ozempic covers strength work.
Behavioural support. WHO pairs long-term GLP-1 treatment with intensive behavioural therapy (structured help with eating and habits). Both recommendations are conditional.18
Staying on it. In STEP 4 and SURMOUNT-4, those who kept taking the medicine kept losing weight.9,10 But they also had more stomach effects, as the table above shows. The BMJ review found regain was faster after medicines than after diet programmes, by 0.3 kg a month.12 See how long people stay on these medicines, and Guide 9 for the cost.
- Stomach effects are very common (more than 1 in 10) on the EU Ozempic, Wegovy and Mounjaro labels.2,4,6
- Pancreatitis is uncommon (1 in 100 to 1 in 1,000) on the EU Ozempic, Wegovy and Mounjaro labels. Severe stomach pain spreading to the back is the warning sign.2,4,6
- Gallstones are common (1 in 10 to 1 in 100) on the EU Ozempic and Wegovy labels. They are uncommon on the EU Mounjaro label.2,4,6
- Dehydration from vomiting or diarrhoea can harm the kidneys.1,3
- Thyroid warning: ruled out by the US labels for anyone with a personal or family history of medullary thyroid cancer (MTC) or MEN 2; tell your doctor if this applies. Not in pregnancy.1,3
Questions people ask
Are there side effects of Ozempic after stopping?
The labels we read list no withdrawal syndrome. In trials, stomach effects became less common after people switched to a dummy injection. Weight, blood sugar and blood pressure slowly rose again.9,10
Will I gain all the weight back after stopping Mounjaro?
Not always all of it. In SURMOUNT-4, people who stopped tirzepatide regained about half their loss in a year. On average they ended 9.9% below their start.10 About 1 in 6 regained less than a quarter.11
How long does Ozempic stay in your body after stopping?
About 5 weeks, says the US Ozempic label. The US Wegovy label says 5 to 7 weeks after a 2.4 mg dose.1,5
Can I stop Ozempic or Mounjaro suddenly?
In the trials, people stopped all at once, and no label we read gives a taper plan. The EU leaflets say not to stop without talking to your doctor, and warn that blood sugar can rise.2,4
Can I restart at my old dose after a break?
Only the Wegovy labels cover it. The US label restarts lower after 2 or more missed doses in a row; the EU label says to consider a lower dose. The Ozempic and Mounjaro labels give no long-gap rule.5,6
Shorter answers are on the way: Does the weight come back after stopping Ozempic? and Can you stop Ozempic or Mounjaro suddenly?
How we researched this
On 5 Oct 2026 we read the US labels for Ozempic, Wegovy, Mounjaro and Zepbound (revised May to August 2026). We also read the EU labels for Ozempic, Wegovy and Mounjaro. We searched each for withdrawal, stopping, tapering and missed doses. We found no Indian label for these brands, so the label facts here are US or EU.
Trial numbers come from the published papers and the US Zepbound label. We read the STEP 1 extension, STEP 4 and SURMOUNT-4 in full. Trial money came mostly from the makers: Novo Nordisk for STEP, Eli Lilly for SURMOUNT.8,16
We found no Indian study of what happens after stopping. We left out claims we saw elsewhere that a slow taper prevents regain, because we found no trial showing it.
Before you move on
- About half to two-thirds of the lost weight came back within a year in the trials.
- Blood sugar and blood pressure drift back up; with diabetes, the sugar change matters most.
- The labels list no withdrawal syndrome and no taper plan.
- After a long gap, only the Wegovy labels deal with restarting, at a lower dose.
