Can you get Wegovy on the NHS? Specialist services, the 2-year limit and the heart route
NHS Wegovy for weight sits in specialist services, not GP surgeries. A second route, for people with heart disease, opened in 2026.
Written for the UK.
Sometimes. In England, Wegovy for weight is only prescribed in specialist weight management services, for up to 2 years, to adults with a BMI of 35 or more and a weight-related condition. Since May 2026, NICE also backs it to cut heart risk in people with heart disease and a BMI of 27 or more.
Key takeaways
- In England, NICE recommends Wegovy for weight only within a specialist weight management service, and for a maximum of 2 years.1
- The NHS threshold is a BMI of 35 or more with a weight-related condition, or 30 to 34.9 with specialist referral criteria; 2.5 lower for some ethnic groups.1
- GP practices can’t prescribe Wegovy for weight under these rules; Mounjaro is the weight medicine NICE allows in primary care.2,3
- Since NICE’s May 2026 guidance, Wegovy up to 2.4 mg can be used to cut heart-attack and stroke risk in adults with heart disease and a BMI of 27 or more.4
- In the trial behind that route, major heart events happened to about 65 in 1,000 people on Wegovy, against 80 in 1,000 on a dummy injection.5
- GLP‑1 medicines are prescription-only medicines in the UK, never sold over the counter.6
- Ozempic and Rybelsus are licensed for type 2 diabetes only, not for weight loss.7
- A prescriber decides whether a medicine fits, which one and at what dose. Nothing here is a recommendation.
- The risks are the same however you get it: see our side effects guide.

Who can get Wegovy for weight on the NHS in England?
Adults with a BMI of 35 or more and a weight-related condition, treated in a specialist weight management service for up to 2 years.1
NICE recommends semaglutide (Wegovy) only if all of these apply:1
- it is used for a maximum of 2 years;
- it is given in a specialist weight management service with a mixed team (often called tier 3 or 4);
- the person has at least one weight-related condition and a BMI of 35 or more;
- or a BMI of 30 to 34.9 plus the criteria for specialist referral.
Thresholds are usually 2.5 lower for people of South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean background.1 So 35 becomes 32.5. Our ethnicity and BMI post explains why.
NICE ties Wegovy to these services on purpose. It found “no evidence of effectiveness” when semaglutide is used as a stand-alone treatment.1
Can my GP prescribe Wegovy?
Not for weight on the NHS: NICE has not approved Wegovy for GP or community settings, so NHS patients get it through specialist services.2,1
Cheshire and Merseyside ICB tells patients plainly that NICE “has not approved these drugs for prescribing in GP/community settings”.2 Your GP can still refer you to a specialist service if you meet its criteria.
NICE says Mounjaro, by contrast, “can be used in primary care or specialist weight management services”.3 That is why most NHS weight-medicine news is about Mounjaro: see Mounjaro on the NHS.
A private prescription is a separate route, with its own checks. Our post on getting it from an online pharmacy covers the rules.
Why does NHS Wegovy stop after 2 years?
Because NICE recommends Wegovy for a maximum of 2 years, inside specialist weight services that people usually use for up to 2 years.1,8
NICE’s obesity guideline says specialist services “are usually accessed for up to 2 years”.8 It also says to consider stopping Wegovy if less than 5% of starting weight is lost after 6 months.1 Our post on the NICE 5% rule covers that review.
Weight often returns after stopping. The STEP 1 extension followed 327 people from both trial groups after all treatment stopped at 68 weeks. Those on Wegovy had lost an average of 17.3% of their body weight by then.9 A year later they had regained about two-thirds of that loss, leaving an average net loss of 5.6%, against 0.1% for the dummy-injection group.9
NICE says to offer support to help keep weight off when weight medicines stop.8 Our stopping guide explains what tends to happen next.

What is the heart-disease route (NICE TA1152)?
Since May 2026, NICE backs Wegovy up to 2.4 mg to lower heart-attack and stroke risk in adults with heart disease and BMI 27 or more.4
NICE technology appraisal TA1152 was published on 7 May 2026.4 It covers adults with established cardiovascular disease, meaning at least one of:4
- a previous heart attack;
- a previous stroke, from a clot or a bleed;
- symptomatic peripheral arterial disease (narrowed leg arteries causing pain, or past surgery or amputation for it).
They also need a BMI of at least 27.4 NICE said the NHS in England must fund it within 90 days. It did not say which service should give it.4 We could not find an NHS England page saying how each area delivers it.
The MHRA licensed this use on 23 July 2024.5 The SELECT trial had over 17,600 people. Major heart events (heart attack, stroke or death from heart disease) happened to about 65 in 1,000 people on Wegovy, against 80 in 1,000 on a dummy injection.5 That is about 15 fewer events per 1,000 people over the trial.
- Stomach effects. In the Wegovy 2.4 mg trials, about 44 in 100 people on Wegovy felt sick (nausea) over 68 weeks, against about 16 in 100 on a dummy injection.10
- Gallstones were reported in about 16 in 1,000 people on Wegovy 2.4 mg in its main trials.10
- Pancreatitis (an inflamed pancreas): severe stomach pain that won’t go away and may spread to the back. The MHRA updated its guidance on it in January 2026.11
- Dehydration from vomiting or diarrhoea can harm the kidneys. The Mounjaro label carries the same warning.10,12
- Surgery and pregnancy. Tell the team before any anaesthetic. The MHRA advises stopping semaglutide at least 2 months before a planned pregnancy, tirzepatide 1 month.7
What about Scotland, Wales and Northern Ireland?
Scotland also keeps Wegovy for weight in specialist services, Wales does the same, and Northern Ireland had no such service in May 2025.14,15,16
Scotland. The SMC accepted Wegovy for weight in adults with a BMI of 30 or more and a weight-related condition. They must be treated in a specialist weight management service.14 It separately accepted Wegovy to cut heart risk in adults with heart disease and a BMI of 27 or more.17
Wales. The All-Wales Weight Management Pathway prescribes weight medicines only within a specialist service.15 Northern Ireland had no specialist weight management service in May 2025, and planned one from early 2026.16
Questions people ask
Can I get Wegovy on the NHS without going to a specialist service?
For weight, not in England under NICE’s guidance. The heart-disease route (TA1152) doesn’t name a setting, so it isn’t limited to weight services.1,4
What BMI counts for Wegovy on the NHS?
For weight, NICE describes 35 or more with a weight-related condition, or 30 to 34.9 with referral criteria. It is usually 2.5 lower for some ethnic groups. For the heart route, it is 27 or more.1,4
Is the 2-year limit fixed?
NICE recommends Wegovy for weight for “a maximum of 2 years” within a specialist service. TA1152 doesn’t carry that limit.1,4
Can I get Wegovy tablets on the NHS?
Not yet. The MHRA approved them in June 2026 and said NHS use depends on a NICE evaluation.21
Is Wegovy 7.2 mg available on the NHS?
NICE’s Wegovy guidance for weight is for semaglutide 2.4 mg, and the heart route covers up to 2.4 mg. We found no NICE decision on 7.2 mg.1,4
How we researched this
On 12 October 2026 we read NICE TA875, TA1152 and NG246, and the SMC decisions. We also read the Welsh and Northern Ireland statements and the MHRA’s 2024 licence notice. Trial figures come from the MHRA release and the STEP 1 extension paper.
NHS England’s pages blocked our checks, so we could not confirm how TA1152 is delivered locally. We name no services or providers.
Before you move on
- Check whether your area has a specialist weight management service and how referral works.
- Know your BMI and which weight-related conditions are on your record.
- If you have had a heart attack, stroke or leg artery disease, ask about TA1152.
- Plan for what happens after 2 years.
- Keep the 6-month review in mind: the 5% rule.
