Does Medicaid cover Wegovy in my state?

The short answer

It depends on your state. State Medicaid programs must cover Wegovy (semaglutide) to lower heart risk in adults with heart disease, but covering it for weight loss is each state’s choice. KFF counted 13 state programs covering GLP-1s for obesity in January 2026, down from 16 in October 2025.

Before you go further
  • GLP‑1 medicines are prescription-only in the US. Coverage decides who pays, not whether the medicine suits you.
  • State counts are from KFF, a health-policy research group, checked October 12, 2026. States change their rules, sometimes mid-year.

Why does Medicaid coverage differ from state to state?

Medicaid must cover nearly all FDA-approved drugs, but weight-loss drugs are one of a few groups states are allowed to leave out.1

That exception comes from federal law, so covering GLP‑1s for obesity is optional for each state.1

Other uses are not optional. KFF says coverage is required for diabetes, for heart disease (Wegovy, since March 2024) and for moderate-to-severe obstructive sleep apnea in adults with obesity (Zepbound, since December 2024).1 Wegovy’s heart use is for adults with established heart disease and either obesity or overweight.5

How many states cover it for weight loss?

KFF counted 13 state programs covering GLP‑1s for obesity under fee-for-service as of January 2026, down from 16 in October 2025.1

California, New Hampshire, Pennsylvania and South Carolina ended coverage. North Carolina dropped it in October 2025 and brought it back in December 2025.1 KFF’s article doesn’t list the 13 states by name, so we don’t either.

Where it is covered, KFF says access is “typically subject to utilization controls such as prior authorization.”1 That usually means your prescriber sends evidence that you meet the state’s criteria before Medicaid pays.

How do you find out what your state covers?

Ask your State Medicaid Agency; Medicaid.gov links to each state’s agency.2

Ask whether Wegovy is covered for weight, for heart risk or both, and what the prior authorization asks for. Bring your BMI and any heart, blood-pressure or blood-sugar results to your prescriber.

A new federal option may widen access. Under CMS’s BALANCE Model, state Medicaid programs could join from May 2026 through January 1, 2027, with prices negotiated by CMS.3,4 CMS hadn’t published which states joined when we checked on October 12, 2026.3 Coverage for older adults is in Does Medicare cover Zepbound or Wegovy?, and how prescriptions and prior authorization work is in Guide 6.
