The short answer

Without insurance, Novo Nordisk’s US self-pay price for Ozempic (semaglutide) pens is $349 a month for 0.25, 0.5 or 1 mg and $499 for 2 mg. Ozempic tablets cost $149 to $299 a month, per Novo’s May 2026 price guide. A limited offer lets new patients pay $199 a month for their first 2 months, through December 31, 2026.

Before you go further
  • GLP‑1 medicines are prescription-only in the US. A licensed clinician has to decide they suit you before any price applies.
  • Prices here are list prices or the makers’ own published self-pay prices, each with its source and the date we checked it. We never quote a seller’s price, and we don’t name or rank pharmacies or telehealth companies.
  • Prices and offers change often. Check the date next to each one.
  • Nothing here promises any result, and nothing here is for sale.

What does each Ozempic dose cost without insurance?

Novo Nordisk’s May 2026 price guide sets the Ozempic pen at $349 a month for 0.25, 0.5 or 1 mg, and $499 for 2 mg.1

Ozempic form and doseSelf-pay price a month
Pen, 0.25, 0.5 or 1 mg$349
Pen, 2 mg$499
Tablets, 1.5 mg$149
Tablets, 4 mg$199
Tablets, 9 mg$299
Limited offer: pen, 0.25 or 0.5 mg, first 2 months for new patients, through Dec 31, 2026$199

Novo Nordisk’s self-pay prices from its Ozempic price guide (May 2026), checked October 14, 2026.1 A month is 1 pen (4 weekly doses) or 30 tablets.

The $199 offer covers 2 monthly fills, through December 31, 2026. Novo says it can change or cancel the program at any time.1 The government site TrumpRx.gov lists the same offer.4 You still need a prescription: see Can I get Ozempic over the counter?

What does a year of Ozempic cost, and what is the list price?

A year is 52 weekly doses, or 13 pens, so a year at $349 a pen comes to $4,537 before any discount.

That is our arithmetic from the price guide.1 A first year starting on the $199 offer comes to $4,237: 2 pens at $199, then 11 at $349. A year at 2 mg comes to $6,487.

The list price is different. It is what Novo charges wholesalers before discounts or rebates, and Novo says most people with insurance don’t pay it. Ozempic’s list price is $1,027.51 a pen at every dose.2 Novo says it falls to $675 a month on January 1, 2027, and that self-pay prices won’t change.3

What if you have insurance or Medicare?

With commercial insurance that covers Ozempic, Novo’s savings offer lets you pay as little as $25 a month, with savings capped at $100 a month.1

The offer excludes government beneficiaries, such as people with Medicare or Medicaid.1 Medicare Part D plans can cover GLP‑1s for type 2 diabetes, the use Ozempic is approved for.5,6 Medicare can’t pay for drugs used only for weight loss, and its $50 GLP‑1 Bridge doesn’t include Ozempic.6,7

If you’re insured but pay cash, TrumpRx’s terms say the payment can’t count toward your deductible or out-of-pocket limit.4 See getting a prescription in the US for how coverage works.

RisksThe risks, in brief
  • Stomach effects are common. In placebo-controlled trials, 16 to 20 in 100 adults on Ozempic 0.5 or 1 mg had nausea, against 6 in 100 on a placebo (dummy injection), on the US label.5
  • Pancreas and gallbladder. The US label warns of inflammation of the pancreas (pancreatitis) and gallbladder problems, including gallstones.5
  • Thyroid warning. Don’t use Ozempic if you or a family member has had medullary thyroid carcinoma (MTC, a rare thyroid cancer), or if you have MEN 2, an inherited condition that causes gland tumors.5
  • Pregnancy. The label says to stop Ozempic at least 2 months before a planned pregnancy.5
