Ozempic’s $274 Medicare price in 2027: what will you actually pay?
What the negotiated price is, why a box of Wegovy works out at about $386, and what one real plan’s 2026 and 2027 figures show. From CMS’s own price files and Medicare’s Plan Finder.
Written for the United States, from US sources.
$274 is the price Medicare negotiated for a 30-day supply of Ozempic, Rybelsus or Wegovy from January 1, 2027. It isn’t your copay. You pay your plan’s share: the full price until your deductible (at most $700) is met, then a copay or coinsurance, up to a $2,400 yearly cap. Weight-loss-only use still isn’t covered.
Key takeaways
- Medicare’s negotiated price for Ozempic, Rybelsus and Wegovy is $274 for a 30-day supply from January 1, 2027, against a 2024 list price of $959.1
- CMS prices each pack: its fact sheet lists $276.78 for a 4 mg Ozempic pen and $385.63 for a box of four Wegovy 2.4 mg pens.1
- In one Chicago drug plan, a 2 mg Ozempic pen costs $277.29 before the deductible in 2027 Plan Finder estimates, against $1,004.89 in 2026.9
- Part D plans still can’t pay for Ozempic or Wegovy used for weight loss alone; the $50 Medicare GLP‑1 Bridge covers Wegovy for weight through 2027.14,10,2
- The yearly cap on what you pay for covered Part D drugs rises from $2,100 in 2026 to $2,400 in 2027.7
- Plans differ. The plan figures here are examples from Medicare’s Plan Finder, not your plan’s costs.
- Medicare and your plan decide coverage; we describe their published rules. We don’t name or rank plans.
- Ozempic and Wegovy need a prescription and carry real risks: see our side effects guide.

What is Ozempic’s $274 Medicare price, and who pays it?
It is Medicare’s negotiated price for a 30-day supply of Ozempic, Rybelsus or Wegovy from January 1, 2027, and it isn’t your copay.1,8
The law calls it the “maximum fair price”. CMS, which runs Medicare, set it with the maker under the Medicare Drug Price Negotiation Program.1 The 2024 list price was $959, so the cut is 71%.1
Semaglutide is the medicine; Ozempic, Rybelsus and Wegovy are brands of it, approved for different uses.11,12 Think of acetaminophen: it is the medicine, and Tylenol is a brand name on the box.
The maker must make the price available to the pharmacies that fill your prescription. Part D plans must put the drugs on their drug lists.1 Your own share still runs through your plan’s deductible and copays, explained below.
- Scale: 2,282,000 people with Part D used one of the three brands in 2024, at about $15.2 billion in total costs.1
- Savings estimate: CMS expects the 15 new 2027 prices to save Part D members about $685 million out of pocket.1
- After 2027: the price rises each year with inflation, measured by the consumer price index.1
Why won’t a box of Wegovy cost $274?
Because CMS sets a price for each pack, and a box of four Wegovy pens works out at about $386, not $274.1,3
| Pack | Negotiated price per pack |
|---|---|
| Ozempic pen, one pen2 mg, 4 mg or 8 mg pen | $276.67 to $276.79 |
| Ozempic or Rybelsus tabletsbottle of 30 | $276.78 |
| Wegovy pens, box of 4any strength, 0.25 mg to 7.2 mg | $385.63 to $385.64 |
| Wegovy tabletsbottle of 30 | $385.63 to $385.64 |
Every Wegovy pen box has the same price, from the starting dose to 7.2 mg.3 Ozempic tablets and the Wegovy pill were added to CMS’s list in 2026.3
Separately, Novo Nordisk is cutting its list price for Wegovy 2.4 mg injection and 25 mg tablets, Ozempic 0.5, 1 and 2 mg, and Rybelsus 7 and 14 mg to $675 a month.6 The cut starts on January 1, 2027.6 Medicare’s negotiated prices sit below that.
What will you pay for Ozempic in 2027, phase by phase?
You pay the plan’s full price until your deductible is met, then a copay or coinsurance, and nothing once your costs reach $2,400.5,7
- The deductible: at most $700 in 2027. In Medicare’s standard design you pay 100% of the drug’s price here; some plans have a lower deductible or none.7,5
- After the deductible: the standard design charges 25% for negotiated drugs. Plans can use other designs.7 A copay is a fixed amount; coinsurance is a percentage.5
- After the cap: once you’ve paid $2,400 for covered drugs, you pay $0 for the rest of the year.5,9
The lower price shows up most where your share is tied to the price: the deductible and coinsurance. A fixed copay is set by the plan, so it may not drop.5
We checked one drug plan in Medicare’s Plan Finder for both years. In 2026, a 2 mg Ozempic pen cost $1,004.89 before the deductible and $251.22 after it. In 2027, it is $277.29 and $69.32.9
Over 12 monthly fills that comes to $2,100 in 2026 and about $1,357 in 2027, by our arithmetic.9 It’s one plan, one dose and one pharmacy, so treat it as an example.
Other drugs can still take you to the $2,400 cap. People with Extra Help pay at most $12.65 a fill for brand-name drugs in 2026 and $14.40 in 2027.7 So their bill changes less.
Add up your copay, the $50 Bridge copay or the makers’ self-pay prices, month by month. No sign-up.
Does the negotiated price help if you take Wegovy for your heart?
Yes, if your plan approves it: Part D can cover Wegovy to lower heart risk, at a negotiated price of about $386 a box.2,3
KFF says Part D plans can cover GLP‑1s for uses other than obesity, including lowering heart risk.2 Wegovy is approved to cut the risk of heart attack, stroke and heart death in adults with heart disease and obesity or overweight.12
In 2026, the plan we checked didn’t list Wegovy. In 2027, both plans we opened listed it, with prior authorization and a quantity limit.9 That fits CMS’s rule that plans must list negotiated drugs.1
The 2027 estimate was $386.13 before the deductible and $96.53 after it, about $1,683 over 12 fills by our arithmetic.9 More on these uses is in Wegovy for heart and liver. People with MASH, a fatty liver disease, can’t use the Bridge; Medicare says their plan may already cover a GLP‑1.10
What doesn’t change in 2027?
Weight-loss-only use is still excluded from Part D, the $50 Bridge still runs separately, and Zepbound and Mounjaro have no negotiated price.14,10,1
- Weight alone: Part D excludes weight-loss drugs “even if used for a non-cosmetic purpose”.14 Being on a plan’s drug list doesn’t change that.2
- The Bridge: $50 a month for Wegovy, Foundayo or the Zepbound KwikPen, for people who meet its criteria, through December 31, 2027.10,2 It sits outside Part D and doesn’t count toward the cap.10 For Bridge drugs, KFF reports the makers agreed a separate net price of $245 a month. That deal doesn’t change the $274 Part D price or your $50 copay.2 See the Bridge explained.
- After the Bridge: CMS said it won’t start its longer-term model (BALANCE) in Medicare in 2027. KFF says it is uncertain how people will keep coverage after 2027.2
- Tirzepatide: Mounjaro and Zepbound aren’t among the 15 drugs with 2027 negotiated prices.1
- Savings cards: the makers’ cards still exclude Medicare. See does the savings card work with Medicare?
What should you check before December 7?
Check your plan’s 2027 tier, prior authorization and price for your exact pack, then compare plans in Medicare’s Plan Finder before open enrollment closes.9
Open enrollment runs October 15 to December 7, 2026. Our step-by-step guide is how to check a Part D plan for Ozempic, Wegovy or Zepbound. The full set of rules is in how Medicare covers GLP‑1 medicines. Prices and prescriptions are in our prescriptions guide.
- Stomach effects. In Ozempic’s diabetes trials, about 16 in 100 people on 0.5 mg and 20 in 100 on 1 mg felt sick (nausea), against 6 in 100 on a dummy injection (placebo).11 At Wegovy’s higher 2.4 mg dose for weight, it was 44 in 100, against 16 in 100.12
- Gallstones were reported in 15 in 1,000 people on Ozempic 0.5 mg and 4 in 1,000 on 1 mg, and in none on placebo.11
- Low blood sugar is more likely when Ozempic is taken with insulin or medicines that make the body release insulin.11
- Pancreatitis (an inflamed pancreas): severe stomach pain that won’t go away and may spread to the back. The label says to stop the medicine if it is suspected.11
- Thyroid warning. Ozempic and Wegovy carry a boxed warning: they are not for people with a personal or family history of medullary thyroid cancer (MTC) or MEN 2, a rare inherited condition that causes gland tumors.11,12
Questions people ask
Will Ozempic cost $274 at the pharmacy in 2027?
Not as your copay. $274 is Medicare’s negotiated price for a 30-day supply. You pay your plan’s share of it: the deductible, then a copay or coinsurance, up to the $2,400 yearly cap.1,5
Does the $274 price apply to Wegovy for weight loss?
Part D plans still can’t pay for weight loss alone. For weight, the $50 Medicare GLP‑1 Bridge covers Wegovy for people who meet its criteria, through December 31, 2027.14,10,2
Why is Wegovy’s negotiated price higher than Ozempic’s?
CMS sets a price for each pack. A box of four Wegovy pens works out at about $386, and one Ozempic pen at about $277. The $274 is CMS’s figure for a 30-day supply.1,3
Is Mounjaro or Zepbound getting a Medicare negotiated price?
Not in 2027. Neither is among the 15 drugs whose negotiated prices start on January 1, 2027.1
Will the negotiated price change after 2027?
Yes. Each year it rises with inflation, measured by the consumer price index, unless it is renegotiated.1
How we researched this
Prices come from CMS’s 2027 negotiated-price fact sheet and its price file by drug code, matched to packs with the FDA’s drug code directory.1,3,4 Benefit rules come from CMS’s 2027 payment announcement and Medicare.gov.7,5
We ran Medicare’s Plan Finder on October 14, 2026, for ZIP code 60601, for 2026 and 2027.9 The yearly totals are our arithmetic from its per-fill figures. We don’t name or rank plans.
