Semaglutide, tirzepatide and every other GLP‑1 medicine in the US compared
Every GLP‑1 medicine sold in the US side by side: the brands, how you take each one, what their main trials found, the side effects and the makers’ prices. Not a ranking.
Written for the United States. Other country versions are coming.
Five GLP‑1 medicines are sold in the US: tirzepatide, semaglutide, orforglipron, dulaglutide and liraglutide, each under one or more brands. They differ in how often you take them, whether they come as a pen, vial or pill, what they’re approved for, how much weight people lost in their trials, and price. A doctor decides which, if any, fits you.

Not a ranking: rows are most searched first. The trials were separate, with different people and lengths, so their numbers can’t be compared directly. Weight change is the average share of body weight lost, except dulaglutide, whose label gives pounds. Prices are the makers’ regular self-pay and list prices, checked Oct 12, 2026, the same as in our cost calculator; insured people usually pay less.
Key takeaways
- The medicine is what works in your body; the brand is the name on the box. Semaglutide is sold as Wegovy, Ozempic and Rybelsus; tirzepatide as Zepbound and Mounjaro.5,8
- Liraglutide is taken daily; tirzepatide, semaglutide and dulaglutide weekly. The Wegovy pill and Foundayo are daily pills for weight.2,1,4,5
- In SURMOUNT-5, a head-to-head trial, people lost 20.2% of their weight on Zepbound and 13.7% on Wegovy over 72 weeks. Other numbers come from separate trials.14
- Stomach effects, mainly nausea, are the most common side effects of all of them.1,2,4,5,8
- Medicare’s $50 GLP‑1 Bridge covers three brands for weight: the Zepbound KwikPen, Wegovy (shot and pill) and Foundayo.37
- This page compares FDA labels and trials. It doesn’t say which medicine suits you, and we don’t rank them: a prescriber decides.
- Prices are the makers’ published prices, never a seller’s.

Which medicines and brands are there?
Five medicines, sold under nine brand names, plus generic liraglutide. The medicine is what works in your body; the brand is the name on the box, the way acetaminophen is sold as Tylenol.
GLP‑1 is a hormone your gut releases after you eat. These medicines copy it: they slow how fast the stomach empties and reduce hunger. Tirzepatide also copies a second gut hormone, GIP.5,8
The full family tree, with each brand’s form and approved uses, is in GLP‑1 types, brands and how they work.
Why are there so many?
Two companies, Novo Nordisk and Eli Lilly, make all the brand-name versions, and the newer medicines differ from the older ones in how often you take them, their form, or the hormones they copy.1,2,4,5,8
- 2005The first GLP‑1 medicine, exenatide (Byetta), was approved as a twice-daily diabetes injection. It isn’t in the table above: the FDA now lists every exenatide product (Byetta, the weekly Bydureon BCise and a generic approved in November 2024) as discontinued by its maker, so none is sold in the US.25,42
- 2010Liraglutide (Victoza): once a day, for type 2 diabetes.3 A higher dose, Saxenda, was approved for weight on December 23, 2014.21
- 2014Dulaglutide (Trulicity): once a week, for type 2 diabetes.1
- 2017Semaglutide (Ozempic): once a week, for type 2 diabetes.6 Rybelsus, the first GLP‑1 pill, followed on September 20, 2019,22 and Wegovy, for weight, on June 4, 2021.19
- 2022Tirzepatide (Mounjaro): once a week, for type 2 diabetes, and it copies two gut hormones, GIP and GLP‑1.9 Zepbound, for weight, followed on November 8, 2023.20
- 2025The Wegovy pill (semaglutide) was approved for weight on December 22, 2025.23
- 2026Orforglipron (Foundayo): a daily pill with no food rules, approved for weight on April 1, 2026.4,24
Generics. A generic is an FDA-approved copy made by another company once patents end. Liraglutide is the only one of these medicines with US generics: a copy of Victoza since December 2024 and of Saxenda since August 2025.16,17 There is no FDA-approved generic semaglutide or tirzepatide; when their patents run out is in Is there a generic Wegovy or Zepbound? Compounded versions, mixed by pharmacies, are not generics and are not FDA-approved.18
Why are some pills and some injections, some weekly and some daily?
It comes down to how each molecule is built: how well the gut absorbs it, and how long it stays in your blood.7,4
- Weekly or daily. A medicine’s half-life is the time for half of it to leave your blood. It is about 13 hours for liraglutide, which is injected daily.2 It is about 5 days for dulaglutide and tirzepatide and about 1 week for semaglutide, so one injection a week is enough.1,8,5
- Semaglutide pills (Wegovy pill, Ozempic pill, Rybelsus). Semaglutide is a peptide, a small protein, and very little of a tablet reaches the blood: about 0.4% to 1% for Rybelsus. An added ingredient, SNAC, helps it get absorbed in the stomach.7 That’s why the pill is taken first thing on an empty stomach with up to 4 ounces of water, then nothing to eat or drink for at least 30 minutes.5,7
- Foundayo (orforglipron) is a small molecule, not a peptide, that switches on the same GLP‑1 receptor.13 Food made no clinically relevant difference to how much got into the blood, so it’s taken once a day with or without food.4
The two weight pills and the Wegovy shot are compared in detail in Wegovy pill vs Foundayo vs Wegovy injection, and the Zepbound forms in Zepbound vials vs KwikPen vs pen.
Why does the same medicine have two brands?
Because each brand is approved for a different use, sometimes at different doses.5,6,8,9
- Ozempic and Wegovy are both semaglutide. Ozempic is for type 2 diabetes, up to 2 mg a week;6 Wegovy is for weight and heart risk, usually at 2.4 mg, with a 7.2 mg Wegovy HD dose.5 See Ozempic vs Wegovy.
- Mounjaro and Zepbound are both tirzepatide, with the same doses up to 15 mg. Mounjaro is for type 2 diabetes; Zepbound is for weight and sleep apnea.9,8 See Mounjaro vs Zepbound.
- Victoza and Saxenda are both liraglutide: Victoza up to 1.8 mg a day for diabetes, Saxenda 3 mg a day for weight.3,2
The approved use matters for coverage: a plan may pay for a medicine for diabetes but not for weight, as explained below.
How do the results compare?
Each medicine has its own main trial, and the trials were separate, with different people, doses and lengths, so their numbers can’t be compared directly.10,11,12,13
In each trial, people were split at random into groups. One got the medicine, the other a dummy injection or dummy pill (placebo), and both got the same diet and activity advice. The gap between the groups is what the medicine added.
For someone starting at 220 lb (100 kg), those averages are about 16 lb (7 kg) on Saxenda, 24 lb (11 kg) on Foundayo 17.2 mg, 33 lb (15 kg) on Wegovy and 46 lb (21 kg) on Zepbound 15 mg. Some people lost much more and some very little. Each trial was paid for by the medicine’s maker.10,11,12,13 The Wegovy pill gave 13.6% in its own trial (OASIS 4), against 2.4% on placebo.5,15 Every dose and side effect is in the trial results tool.
A head-to-head trial: SURMOUNT-5. It gave 751 adults with obesity, without diabetes, either Zepbound or Wegovy at the highest dose each could take. Over 72 weeks, average weight loss was 20.2% on Zepbound and 13.7% on Wegovy. Everyone knew which medicine they had, and Eli Lilly, Zepbound’s maker, paid for it.14 More in Zepbound vs Wegovy. For type 2 diabetes, Mounjaro and Ozempic were compared head to head in SURPASS-2: see Mounjaro vs Ozempic.
Side effects are similar in kind across all of them: mostly nausea, diarrhea, vomiting and constipation, worst while the dose goes up.5,8 Each label counts them in its own trials, so the rates in the table can’t be compared directly either. Serious risks are in our side effects guide.

Which costs more, and what does coverage change?
Paying cash, the makers’ monthly prices run from $149 to $699, depending on the brand and dose; what you actually pay depends on your insurance.26,28,30
The makers’ regular self-pay prices a month, most searched medicine first (Google Trends):
- Tirzepatide: Zepbound vials and KwikPens $299 to $699, by dose; Mounjaro $499. The Zepbound single-dose pen is sold through insurance only.28,29
- Semaglutide: Wegovy pen $349 at doses up to 2.4 mg and $399 for Wegovy HD 7.2 mg; Wegovy pill $149 to $299; Ozempic pen $349 to $499; Ozempic pill $149 to $299.26,27
- Orforglipron: Foundayo $149 to $349, by dose.30
- Dulaglutide: Trulicity $389 at every dose.29
- Liraglutide: no self-pay price published. List prices are $271.76 to $815.27 for Victoza, by dose, $1,349.02 for Saxenda at 3 mg, and $1,165 for Teva’s generic Saxenda (from a news report; Teva hasn’t published it).32,17
List prices, which insured people’s costs can be based on, run from $649 (Foundayo) to $1,349.02 (Wegovy and Saxenda) a month.31,32 Novo Nordisk is cutting the list price of Wegovy, Ozempic and Rybelsus to $675 a month from January 1, 2027.34 Every brand and dose is on our GLP‑1 prices page.
What coverage changes.
- Medicare: by law, Part D can’t cover drugs used for weight loss,38 but it can cover these medicines for other approved uses, such as diabetes, heart risk or sleep apnea.41 From July 1, 2026, to December 31, 2027, the GLP‑1 Bridge covers the Zepbound KwikPen, Wegovy (shot and pill) and Foundayo for weight at $50 a month, for people who meet its criteria.37 See the Medicare GLP‑1 Bridge explained.
- Employer plans: in 2025, 19% of firms with 200 or more workers covered GLP‑1s mainly for weight loss.39 See prior authorization, step by step.
- Medicaid: 13 state programs covered GLP‑1s for obesity in January 2026.40 See Medicaid by state.
All the routes are in Who pays: insurance and Medicare.
Which one? What a doctor weighs
There is no single right choice for everyone: a prescriber looks at your health, your preferences and what you can get.
- Your health conditions. Each brand is approved for different things: type 2 diabetes, weight, heart risk, kidney risk in diabetes, sleep apnea or MASH liver disease (see the table).5,6,8
- Daily or weekly, and needles or pills, including whether an empty-stomach morning routine works for you.5,4
- Side effects you’ve had before, and things that rule a medicine out, such as a personal or family history of medullary thyroid cancer or MEN 2, a rare inherited condition that causes gland tumors.5,8
- Other medicines. The labels say not to take two GLP‑1 medicines together.5,8
- Availability and cost: what your plan covers, and the cash price at the dose you’d likely reach. Saxenda and Rybelsus are being discontinued.35,36

Questions people ask
What is the difference between all the GLP‑1 medicines?
They are five different medicines: tirzepatide, semaglutide, orforglipron, dulaglutide and liraglutide. They differ in how often you take them (daily or weekly), their form (pen, vial or pill), what each brand is approved for, their trial results and their price. Tirzepatide also acts on a second gut hormone, GIP.1,2,4,5,8
Which GLP‑1 medicines are pills?
Two pills are approved for weight: the Wegovy pill (semaglutide), taken on an empty stomach with up to 4 ounces of water and a 30-minute wait, and Foundayo (orforglipron), taken with or without food. The Ozempic pill and Rybelsus are semaglutide pills for type 2 diabetes.5,4,7
Are Ozempic and Wegovy the same medicine?
Yes, both are semaglutide, made by Novo Nordisk. Ozempic is approved for type 2 diabetes, up to 2 mg a week; Wegovy is approved for weight and heart risk, usually at 2.4 mg. In the same way, Mounjaro and Zepbound are both tirzepatide.6,5,9,8
Do the trials show which GLP‑1 loses more weight?
Mostly no, because each medicine was tested in a separate trial with different people and lengths. In one head-to-head trial, SURMOUNT-5, average weight loss over 72 weeks was 20.2% on Zepbound and 13.7% on Wegovy; Zepbound’s maker paid for it.14
How much do GLP‑1 medicines cost without insurance?
The makers’ self-pay prices run from $149 to $699 a month, depending on the brand and dose: for example Zepbound vials or KwikPens $299 to $699, Wegovy pens $349, Foundayo $149 to $349 and Trulicity $389. Liraglutide brands have no published self-pay price.30,29,26,28
Is there a generic GLP‑1 in the US?
Only liraglutide: a generic copy of Victoza since December 2024 and of Saxenda since August 2025. There is no FDA-approved generic semaglutide or tirzepatide, and compounded versions are not FDA-approved.16,17,18

Before you move on
- Note which uses your plan covers: diabetes, weight, heart or sleep apnea.
- Ask whether a weekly shot or a daily pill suits your routine.
- Compare cash prices at the dose you’d likely reach.
- Plan for stomach side effects while the dose rises.
- Never take two GLP‑1 medicines together.
