Which coverage rules apply to your GLP‑1?
Work plan, Marketplace, Medicare, Medicaid in your state, TRICARE, the VA or FEHB: answer 4 questions and see which set of rules decides coverage for your use, what that route usually asks for, and what to do next.
Your route
What this means
What this doesn't mean
- It isn't a check of your plan. Two people on the same kind of coverage can have different drug lists. Your plan's own documents decide.
- It isn't a yes for treatment. Whether a GLP‑1 suits you is your prescriber's decision.
- Rules change. States and employers change coverage, sometimes mid-year. Dates on this page show when we last checked.
Why we say this
The rule behind each route, with its source. All checked Oct 14, 2026.
Tools for the next step
How this works
US coverage for GLP‑1 medicines runs on separate tracks, each with its own rules: employer plans, the Marketplace, Medicaid (one set of rules per state), Medicare, TRICARE, the VA, CHAMPVA, and federal employee plans. Within each track, the use written on the prescription decides what can be covered.9
The tool matches your answers to a track and a use, then shows what that route usually asks for, from dated sources. For Medicaid, it reads a table of all 50 states and DC kept on our Medicaid by state page, which was checked against each state's own notices on October 14, 2026.1 For Medicare, it uses the federal weight-loss exclusion and the CMS rules for the Medicare GLP‑1 Bridge.3,2
What this can't tell you
- Whether your own plan covers a medicine, at which tier, or what you'll pay.
- Whether you meet a plan's criteria. The prior authorization checklist lists what criteria ask for.
- Rules for a state's Medicaid health plan that go beyond the state's own list.
Questions people ask
Why does the use on the prescription matter so much?
US plans pay by the reason on the prescription. Medicare drug plans can't cover a drug used for weight loss alone, but can cover type 2 diabetes, heart-risk and sleep apnea uses.3,8 Many employers and states choose not to cover weight loss but still cover diabetes. The same pen can be covered for one use and refused for another.9
Does Medicaid cover Wegovy or Zepbound for weight loss?
It depends on the state. As of October 14, 2026, 10 state Medicaid programs covered GLP‑1s for weight loss alone: Delaware, Kansas, Michigan, Minnesota, Mississippi, Missouri, North Carolina, Tennessee, Virginia and Wisconsin, each with approval first or narrow criteria. Medicaid must still cover GLP‑1s for diabetes, heart risk and sleep apnea.1
I'm on Medicare. Which route applies?
For weight, the Medicare GLP‑1 Bridge: $50 a month from July 1, 2026 to December 31, 2027, if its criteria are met.2,11 For type 2 diabetes, heart risk, sleep apnea or MASH, your Part D drug plan (or Medicare Advantage plan with drug coverage), under its own drug list and rules.8
Does TRICARE For Life cover Wegovy or Zepbound?
Not for weight: TRICARE stopped covering weight-loss medicines for anyone outside TRICARE Prime and Select on August 31, 2025. Diabetes medicines such as Ozempic and Mounjaro remain covered with prior authorization.4
Does this check my plan?
No. It explains which set of rules applies and what that route usually asks for. Only your plan's drug list and documents show what your plan covers.
