What is the maintenance dose of Mounjaro, Wegovy and Ozempic?

The short answer

The maintenance dose is the weekly dose you stay on after the steps up. The US labels give 5, 10 or 15 mg for tirzepatide (Zepbound, same as Mounjaro), 1.7 or 2.4 mg for Wegovy, and 0.5, 1 or 2 mg for Ozempic. India sells Ozempic only up to 1 mg. A doctor picks it by results and side effects.

What is the maintenance dose on each label?

Each label names more than one dose to stay on, reached after starting low and stepping up at least 4 weeks apart.

  • Mounjaro (tirzepatide): 5, 10 or 15 mg a week on the US Zepbound label, the weight brand of the same tirzepatide. The 2.5 mg dose is only for starting.1 The US Mounjaro label, written for diabetes, allows 2.5 mg steps up to 15 mg.2
  • Wegovy (semaglutide): 2.4 mg a week, the dose the label recommends, or 1.7 mg.3 The US label also allows 7.2 mg after at least 4 weeks on 2.4 mg; India sells strengths up to 2.4 mg.3,6
  • Ozempic (semaglutide): 0.5, 1 or 2 mg a week, set by blood sugar.4 India sells 0.25, 0.5 and 1 mg.6
  • Rybelsus (semaglutide tablets): 7 mg a day, or 14 mg if blood sugar needs more control.5

How does a doctor choose the dose to stay on?

The Wegovy label says to weigh "treatment response and tolerability", and the Zepbound label allows a lower dose if one isn't tolerated.3,1

Higher doses brought more weight loss on average, and more side effects. In SURMOUNT-1, adults lost 15.0% of their weight on tirzepatide 5 mg and 20.9% on 15 mg over 72 weeks, against 3.1% on a dummy injection.7 In the Zepbound label's pool of SURMOUNT-1 and SURMOUNT-2, stomach side effects made 1.9 in 100 stop on 5 mg and 4.3 in 100 on 15 mg, against 0.5 in 100 on the dummy.1

RisksThe risks, in brief
  • Nausea is very common (more than 1 in 10) on the EU Wegovy and Mounjaro labels, mostly while the dose rises.13
  • Pancreatitis is uncommon (1 in 100 to 1 in 1,000) on both EU labels; severe stomach pain that won't go away is the warning sign.13,3
  • Thyroid warning. The US labels rule these medicines out for anyone with a personal or family history of medullary thyroid cancer or MEN 2.2,3

Is there a lower dose once you reach your goal weight?

None of the labels we read sets a separate, lower dose for after weight loss. The main trial of staying on kept people at their highest tolerated dose.

In SURMOUNT-4, 783 adults took tirzepatide 10 or 15 mg for 36 weeks, and the 670 who finished lost 20.9% on average.8 Over the next 52 weeks, those who stayed on it lost a further 5.5%, while those switched to a dummy injection regained 14.0%. Stomach side effects, mostly mild to moderate, were more common in the group that stayed on tirzepatide.8 Lilly staff were among the authors.8

An Italian expert panel in 2026 called for "flexible dose management" over the long term, but gave it as opinion, and several members had been paid by Novo Nordisk or Lilly.9 What happens if you stop is in What happens when you stop Ozempic or Mounjaro?

What does each maintenance dose cost in India?

Each pen holds 4 weekly doses, so a year is 13 pens at the dose you stay on.

  • Mounjaro: ₹16,406 a pen at 5 mg (₹2,13,278 a year), ₹20,625 at 10 mg (₹2,68,125), ₹25,781 at 15 mg (₹3,35,153).10
  • Wegovy: ₹12,750 a pen at 1.7 mg (₹1,65,750 a year), ₹16,400 at 2.4 mg (₹2,13,200).6,12
  • Ozempic: ₹8,100 a pen at 0.5 mg (₹1,05,300 a year), ₹9,100 at 1 mg (₹1,18,300).11,12
