How long do Mounjaro side effects last?

The short answer

Usually days to weeks. The US label for Mounjaro (tirzepatide) says most nausea, vomiting and diarrhoea come during dose increases, then ease. We found no published median duration for tirzepatide. In trials of semaglutide, a similar medicine, a typical bout of nausea lasted 8 days and constipation 47 days.

  • GLP‑1 medicines such as Mounjaro are prescription-only in India, not sold over the counter.11
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.11 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.12

How long does a bout of nausea or diarrhoea last on Mounjaro?

In an Indian tirzepatide study most nausea settled within 2 to 3 weeks; in semaglutide trials a typical bout of nausea lasted 8 days.7,5

A Lilly analysis of Mounjaro’s diabetes trials calls its nausea, vomiting and diarrhoea “transient”. It gives no duration, and we found no published median elsewhere.6 The full list is in Mounjaro side effects.

In 3 STEP weight trials of semaglutide (Ozempic, Wegovy), a typical bout of nausea lasted 8 days. Diarrhoea lasted 3 and vomiting 2.5 Bouts on a dummy injection (placebo) lasted about as long.5

In 7 Lucknow clinics, 32 of 71 people with diabetes felt sick (45%) after starting tirzepatide. Most cases were mild and settled within 2 to 3 weeks.7 But only the 71 of 225 starters who completed 3 months were analysed. Within 3 months, 50 of the 225 (22%) had stopped, most often because of stomach side effects.7 The study had no comparison group, and 6 authors reported Eli Lilly fees.7

Do side effects come back when the dose goes up?

They can, because the trials and labels tie most stomach side effects to the weeks when the dose is being stepped up.1,5

Mounjaro starts at 2.5 mg a week. If needed, the dose can rise by 2.5 mg after at least 4 weeks on each dose.1 Stepping up every 4 weeks, the 15 mg maximum comes at week 20 at the earliest.1 In the SURMOUNT-1 and -2 weight trials, stomach side effects were most common in the first 8 weeks, then fell.4

In STEP, the last semaglutide step came at week 16, and nausea, diarrhoea and vomiting peaked around week 20.5

If a maintenance dose isn’t tolerated, the Zepbound label (tirzepatide for weight) suggests a lower one.2 The Wegovy label (semaglutide) suggests waiting 4 more weeks before the next increase if a dose isn’t tolerated.8 After 2 or more missed doses in a row, it advises restarting lower to cut stomach side effects.8

Which side effects last longer?

Constipation tends to last longer than nausea, with a typical bout of 47 days in semaglutide trials, against 35 days on placebo.5

On the Zepbound label, 11% to 17% of people on tirzepatide reported constipation, against 5% on placebo. It gives no duration.2 What helps is in Guide 5.

In STEP, all 4 stomach effects stayed more common than on placebo for 68 weeks.5 Nausea fell the most.5

The EU Mounjaro label rates 74% of stomach side effects mild and 23% moderate in diabetes trials. In weight trials it was 61% and 35%.3

When do lasting symptoms need a doctor?

The Mounjaro medication guide says to tell your doctor right away about nausea, vomiting or diarrhoea that does not go away.9

Losing fluid this way can cause dehydration, which can harm the kidneys.9 The guide also says to report gallbladder signs right away.9 These include upper stomach pain, fever, yellow skin or eyes, and clay-coloured stools.9

The US label also lists post-approval reports of blocked bowels (ileus) and severe constipation.1 You can report a side effect to India's Pharmacovigilance Programme on 1800 180 3024, on working days.10

Get help now: call 112 for severe stomach pain that won't go away, sometimes spreading to your back, with or without vomiting.9 Do the same for swelling of the face, lips, tongue or throat, or trouble breathing.9 The medication guide says to stop Mounjaro in both cases.9
