How long do people stay on Mounjaro or Ozempic, and what are the long-term effects?
What labels and guidelines say about how long to take Mounjaro (tirzepatide) and Ozempic or Wegovy (semaglutide), how long people stay on, why they stop, and the cost in ₹.

Neither the US labels nor India's ESI 2025 obesity guideline sets an end date. US diabetes guidance favours continuing after a goal, but also allows a lower dose or stopping. In a large US study, 65 in 100 without diabetes stopped within a year. The longest trials ran about 4 years, so longer effects are unknown.
Key takeaways
- No end date appears in the US Wegovy or Mounjaro labels or in India's ESI 2025 guideline, while the UK's NICE caps NHS-funded Wegovy for weight at 2 years.1,4,6,7
- In a US health-record study of 125,474 adults, 65 in 100 without diabetes and 47 in 100 with type 2 diabetes stopped their GLP-1 within a year.9
- In a 2025 US survey of 440 people who had stopped, the top reasons were cost (36 in 100), side effects (33 in 100) and no insurance cover (28 in 100).11
- In the SELECT trial, people on Wegovy 2.4 mg weighed 10.2% less after 4 years, against 1.5% less on placebo, but 17 in 100 stopped for side effects, against 8.15,16
- At Mounjaro's listed MRP of ₹16,406 for 4 doses of 5 mg, 5 years of the medicine alone costs ₹10,66,390, about ₹10.7 lakh.27
- GLP-1 medicines are prescription-only in India. A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.29 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.30
- Every number here is an average for a group in a trial or study. None of it can tell anyone how long they will need a medicine.
- The side effects and serious risks are in Guide 5, Side effects and risks.
- Prices are MRPs on the date shown. We list medicines by molecule and never rank them by price.

What do the labels and guidelines say about how long to take them?
None of the labels or guidelines we read sets an end date, but the UK's NICE caps NHS-funded Wegovy for weight at 2 years.1,4,6,7
The labels. The US Wegovy label says it is used to reduce weight and "maintain weight reduction long term".6 We found no maximum length of treatment in the US Wegovy or Mounjaro labels.6,7 Indian labels were not online, so we used the US ones.
India's ESI 2025 guideline. The Endocrine Society of India says "the need for long term weight therapy is well-known".1 It also allows a first 3-month trial, to see whether a medicine works and suits the person.1 It sets no end date. It does say cost must be part of the discussion before starting.1
The ADA, 2026. The American Diabetes Association writes the main US diabetes guidance. Its rule 8.19 says long-term obesity medicines "should be continued beyond reaching weight loss goals".2 It says stopping often brings back the weight and the heart and blood-sugar risk factors.2 It also calls for a shared decision on the long-term plan. Its examples are "the lowest effective dose", "intermittent therapy", or stopping with "close weight monitoring".2 Guide 9 covers these options.
WHO, December 2025. WHO calls obesity "a chronic, relapsing disease requiring lifelong care".3 It recommends long-term GLP-1 treatment together with intensive help to change habits (behavioural therapy).3 Both recommendations are only "conditional". Limited long-term data and cost are among the reasons it gives.3
NICE (UK), March 2023. NICE decides what England's NHS pays for. It funds Wegovy for weight "for a maximum of 2 years", inside specialist weight services.4 It also says to consider stopping if less than 5% of starting weight is lost after 6 months.4
Why 2 years? NICE said UK specialist weight services are time-limited.4 It noted the company's point that the longest trial, STEP 5, ran 2 years.4 It added: "There is no evidence for longer-term use of semaglutide available".4 The committee itself called the cap "not ideal for treating a chronic condition".4
Tirzepatide has no NHS time cap. NICE's December 2024 advice, updated in September 2025, sets no maximum length.5 It asks for a decision at 6 months if weight fell less than 5%, on the highest dose a person can manage.5
How long do people actually stay on Mounjaro, Ozempic or Wegovy?
In large US studies, about half to two-thirds of people stopped within a year, and about 1 in 12 were still on after 3 years.8,9
Most of this data comes from the US. We found no peer-reviewed Indian study of how long people stay on these medicines.
US health records, 2018 to 2023. A study in JAMA Network Open followed 125,474 adults with a BMI of 27 or more.9 They had started liraglutide, semaglutide or tirzepatide.9 Within a year, 65 in 100 without diabetes had stopped, and 47 in 100 with type 2 diabetes.9
US insurance claims, Prime Therapeutics. This US pharmacy-benefit company tracked 5,780 insured adults with obesity and no diabetes. All started a GLP-1 in 2021 or early 2022.8 After 3 years, 8 in 100 were still on one, about 1 in 12.8 For people who started on Wegovy, it was 14 in 100, about 1 in 7.8
"Still on" meant no gap of 60 days or more between refills.8
Staying on has improved. Of people who started Wegovy or Zepbound in 2021, 33 in 100 were still on a year later. For early-2024 starters, it was 63 in 100.8 Zepbound is tirzepatide, the same medicine as Mounjaro, sold for weight in the US.32 Prime says the end of US shortages likely explains the rise.8
Blue Cross Blue Shield, May 2024. A study of nearly 170,000 insured US members found 58 in 100 stopped before reaching "a clinically meaningful health benefit".10 30 in 100 stopped within the first month.10 People whose prescription came from an endocrinologist or obesity specialist were more likely to stay on.10
Why do people stop taking them?
In US studies, cost, side effects and losing insurance cover top the list, followed by reaching a weight goal and supply problems.8,11
A national survey in June 2025 asked 440 US adults why they had stopped. All had started a GLP-1 since 2022.11 More than half (55 in 100) had stopped within 6 months.11 The reasons they gave, and people could give more than one:11
- Cost: 36 in 100.
- Side effects: 33 in 100.
- No insurance cover: 28 in 100.
- Reached their weight goal: 26 in 100.
People with diabetes named side effects far more often: 50 in 100, against 28 in 100 without diabetes.11 In the health-record study, moderate or severe stomach side effects made stopping more likely.9 Losing more weight made it less likely, and so did a higher income for people with diabetes.9 Our answer on how long side effects last covers the usual timeline.
Supply. In India, Dr Reddy's paused new batches of its semaglutide in July 2026 after finding an impurity.13 It expected supply in India to be disrupted until at least late October 2026.13 Three makers also recalled select batches of Obeda, Semalix and Usema.13
What Indian doctors report. Doctors quoted by Outlook Business in June 2026 named stomach side effects, unrealistic expectations and out-of-pocket costs.12 Their views did not come from a study.
What does staying on it for years cost in India?
At current MRPs, one year of medicine alone runs from about ₹55,000 to ₹3.35 lakh, depending on the medicine and the dose.23,25,27
We count a year as 52 weekly doses, so a month is about 4.3 doses. A pack holds 4 weekly doses, so a year is 13 packs. The table assumes one steady dose. The first months cost less, because starting doses are lower.
| Medicine and dose | MRP for 4 weekly doses | 1 year | 2 years | 5 years |
|---|---|---|---|---|
| Ozempic 1 mg (semaglutide, type 2 diabetes) | ₹9,10023 | ₹1,18,300 | ₹2,36,600 | ₹5,91,500 |
| Wegovy 2.4 mg (semaglutide, weight) | ₹16,40023 | ₹2,13,200 | ₹4,26,400 | ₹10,66,000 |
| Poviztra 2.4 mg (Novo Nordisk semaglutide, sold by Emcure) | ₹8,99924 | ₹1,16,987 | ₹2,33,974 | ₹5,84,935 |
| Generic semaglutide pens, top doses where published | ₹4,200 to ₹8,00025,26 | ₹54,600 to ₹1,04,000 | ₹1,09,200 to ₹2,08,000 | ₹2,73,000 to ₹5,20,000 |
| Mounjaro 5 mg KwikPen (tirzepatide) | ₹16,40627 | ₹2,13,278 | ₹4,26,556 | ₹10,66,390 |
| Mounjaro 10 mg KwikPen (tirzepatide) | ₹20,62527 | ₹2,68,125 | ₹5,36,250 | ₹13,40,625 |
Medicine only, at one steady dose; our arithmetic (13 packs a year). Listed by molecule, not by price. Ozempic, Wegovy and Poviztra MRPs are from dated news reports of the makers' prices. Generic pen prices are launch prices from March 2026; vials are priced per vial and left out. Mounjaro has no newer maker price. Its row uses the printed MRP on two pharmacy listings, checked 5 Oct 2026.
Higher Mounjaro doses cost more. The 12.5 and 15 mg pens are listed at ₹25,781 for 4 doses, about ₹3.35 lakh a year.27
Doctor visits and tests add more. An endocrinologist visit costs a median ₹1,100 across 6 cities on Practo.28 A basic blood-test panel at private lab chains costs a median ₹2,440.28 At 4 visits and 2 rounds of tests a year, that adds about ₹9,280 a year. Those counts are our assumption.
A worked example. Take 3 years of Mounjaro 5 mg. The medicine is 39 packs, ₹6,39,834.27 Twelve visits add ₹13,200 and six rounds of tests add ₹14,640.28 That is about ₹6.7 lakh in all.
Guide 6 lists every brand's MRP by dose and works out the first year step by step. Our posts on the all-in cost of a year and on health insurance cover go further.

What do the longest trials show?
The longest trials ran 2 to 4 years, and weight stayed down while people kept taking the medicine, though more stopped for side effects.14,15,16,17
Ozempic (semaglutide 0.5 or 1 mg), SUSTAIN-6, 2 years. 3,297 adults with type 2 diabetes took Ozempic or a dummy injection (placebo) for 104 weeks.19 More people stopped Ozempic because of side effects, mainly stomach ones.19 Our post on the heart trials gives its full results.
The weight trials below used Wegovy, which is semaglutide at 2.4 mg. That is a higher dose than any Ozempic sold in India, which comes in 0.25, 0.5 and 1 mg.31
Wegovy 2.4 mg, STEP 5, 2 years. 304 adults without diabetes lost 15.2% of their weight on Wegovy, against 2.6% on placebo.14 Stomach side effects hit 82 in 100 on Wegovy and 54 in 100 on placebo, mostly mild to moderate.14 About 6 in 100 stopped Wegovy for side effects, against 5 on placebo.14
Wegovy 2.4 mg, SELECT, about 4 years. 17,604 adults with existing heart or blood-vessel disease and no diabetes were followed for an average of 3.3 years, with weight data to 4 years.15 At 208 weeks, weight was 10.2% lower on Wegovy and 1.5% lower on placebo.16 About 17 in 100 stopped Wegovy for side effects, against 8 on placebo.15
Tirzepatide (Mounjaro), SURMOUNT-1, 3 years. 1,032 adults with obesity and prediabetes took 5, 10 or 15 mg a week, or placebo, for 176 weeks.17 Weight fell 12.3%, 18.7% and 19.7% by dose, against 1.3% on placebo.17 Type 2 diabetes developed in 1.3% on tirzepatide and 13.3% on placebo.17 Our post on prediabetes and GLP-1 medicines covers this group.
Stopping for side effects in SURMOUNT-1 was 7.3%, 9.5% and 12.3% by dose, against 5.9% on placebo.17 Eli Lilly, which makes Mounjaro, paid for the trial.17
Tirzepatide, SURMOUNT-4, 88 weeks. Everyone first took 10 or 15 mg for 36 weeks.18 Then half were switched to placebo without being told.18 Over the next year, weight rose 14.0% in the switched group. It fell a further 5.5% in those who stayed on.18
Mounjaro in type 2 diabetes, SURPASS-CVOT, about 4 years. 13,299 adults with diabetes and heart disease took Mounjaro up to 15 mg or dulaglutide, an older weekly injection.7 Over a median of 210 weeks, heart events were 12.1% against 13.0%. Mounjaro was shown to be no worse (non-inferior), but not proven better.7 13.3% stopped Mounjaro for side effects, against 10.2% on dulaglutide.20
For what these results mean for one person, our post on how much weight people lose gives the ranges.
- Stomach effects. Feeling sick (nausea) and diarrhoea are very common on the EU Mounjaro label (more than 1 in 10).21
- Inflamed pancreas (pancreatitis). Uncommon on the EU Mounjaro and EU Wegovy labels (1 in 100 to 1 in 1,000).21,22
- Gallstones. Common on the EU Wegovy label (1 in 10 to 1 in 100). Uncommon on the EU Mounjaro label (1 in 100 to 1 in 1,000).22,21
- Dehydration. The EU Mounjaro leaflet warns that fluid lost through vomiting or diarrhoea can reduce kidney function.21
- Low blood sugar. The risk rises when taken with insulin or a sulphonylurea, a type of diabetes tablet.21
- Thyroid warning. The US Mounjaro label carries a boxed warning based on thyroid tumours in rats. It rules out use after medullary thyroid cancer or with MEN 2.7
- Pregnancy. The US Wegovy label says to stop at least 2 months before a planned pregnancy.6
What side effects showed up over 2 to 4 years?
Tirzepatide's 3-year trial reported no new safety concerns, but Ozempic's 2-year trial found more serious diabetic eye problems than placebo.17,19
Eye problems on Ozempic (0.5 or 1 mg), SUSTAIN-6. Over 2 years, serious complications of diabetic eye disease (retinopathy) hit 3.0% on Ozempic and 1.8% on placebo.19,33 These included bleeding inside the eye, blindness, or needing laser or eye-injection treatment.19 The gap was largest in people who already had eye disease: 8.2% against 5.2%. Without it, 0.7% against 0.4%.33
The US Ozempic label links a fast drop in blood sugar with a temporary worsening of this eye disease.33 The EU Ozempic leaflet lists these eye complications as common (1 in 10 to 1 in 100).34
Sudden vision loss (NAION). Damage to the eye's nerve can cause painless loss of sight in one eye. It is very rare on the EU Wegovy and Ozempic leaflets (fewer than 1 in 10,000).22,34 Large studies in type 2 diabetes suggest about 1 extra case per 10,000 people taking semaglutide for a year.34
Tirzepatide, SURMOUNT-1, 3 years. The authors found "no new safety signals".17 Stomach side effects were the most common, mostly mild to moderate and mostly in the first 20 weeks, while the dose was being raised.17
Wegovy 2.4 mg, SELECT, about 4 years. Serious side effects were less frequent on Wegovy than on placebo in every BMI group. More people stopped Wegovy, though, and stopping rose as BMI fell.16 Our post on Ozempic side effects covers the rest.
What don't we know yet about the long-term effects of Mounjaro and Ozempic?
We found no trial longer than about 4 years, so the effects of 10 or 20 years of use have not been measured.3,7,16
Tirzepatide is new. Mounjaro was first approved in the US in 2022.7 Semaglutide was first approved there in 2017.6 So real-world use of tirzepatide spans only a few years.
The thyroid question is open. Tirzepatide caused thyroid C-cell tumours in rats.7 The US label says it is "unknown" whether it does so in humans.7 Our post on the thyroid cancer warning explains what that means.
Few South Asians were studied. STEP 5 had 2 Asian people among 304.14 SURMOUNT-4 ran in Argentina, Brazil, Taiwan and the US.18 In SURMOUNT-1's 3-year group, about 10 in 100 were Asian.17 We found no long-term trial run in India.
What happens if you stop after a long time?
In trials, half to two-thirds of the weight lost came back within a year of stopping, and some heart and blood-sugar gains faded too.2
In SURMOUNT-4, weight rose 14.0% in the year after people were switched from tirzepatide to placebo.18 ESI 2025 cites STEP 1, where two-thirds of the weight lost came back a year after stopping semaglutide.1
Guide 9, Stopping and the long term covers regain, lower doses and planning a stop. Our post on what happens when you stop Ozempic or Mounjaro goes further. If you are already on one, the already-on-it path collects the pages in order.
Questions people ask
Is Mounjaro meant to be taken for life?
No label or guideline we read sets an end date. The ADA's 2026 guidance says long-term obesity medicines should continue after a weight goal is reached, because stopping often brings the weight back. It also lists the lowest effective dose, intermittent use or stopping with close weight checks as options.2,7
What are the long-term side effects of Mounjaro?
The longest tirzepatide trials ran about 3 to 4 years.17,7 The 3-year SURMOUNT-1 report found no new safety signals. Stomach side effects were the most common, mostly in the first 20 weeks.17 Whether the thyroid tumours seen in rats happen in humans is unknown.7 For semaglutide, Ozempic's 2-year trial found more serious diabetic eye complications: 3.0% against 1.8% on placebo.33
Can you take Ozempic for 5 years?
Ozempic's main heart trial, SUSTAIN-6, ran 2 years, and the Wegovy 2.4 mg trial SELECT about 4 years.19,15 We found no trial of either that followed people for 5 years.
Why does the NHS stop Wegovy after 2 years?
NICE limited NHS-funded Wegovy for weight to 2 years in 2023. It said UK specialist weight services are time-limited and that there was no evidence beyond 2 years then.4 NICE's tirzepatide advice has no such cap.5
Do people restart after stopping?
Many do. In a US study, 36 in 100 people without diabetes restarted within a year of stopping. Among people with diabetes, it was 47 in 100.9
How much does 5 years of Mounjaro cost in India?
At the listed MRP of ₹16,406 for 4 doses, 5 years at 5 mg costs ₹10,66,390. That is about ₹10.7 lakh for the medicine alone. At 10 mg (₹20,625 for 4 doses) it is ₹13,40,625.27
How we researched this
On 5 October 2026 we read India's ESI 2025 obesity guideline and the ADA 2026 Standards. We also read WHO's guideline summary and NICE's appraisals TA875 and TA1026. We read the US Wegovy, Ozempic and Mounjaro labels and the EU Wegovy, Ozempic and Mounjaro labels. Indian labels were not online.
We read the Prime Therapeutics, Blue Cross Blue Shield, JAMA Network Open and US survey studies. We read the STEP 5, SELECT, SUSTAIN-6, SURMOUNT-1, SURMOUNT-4 and SURPASS-CVOT results. The SURMOUNT-1 3-year paper is paywalled; we read its tables from a full-text copy.
We checked each MRP on 5 Oct 2026 against the maker's statement or a dated news report. Mounjaro has no newer maker price, so we used the printed MRP on two pharmacy listings. All yearly costs, "in 100" figures and the worked example are our arithmetic. We found no peer-reviewed Indian study of how long people stay on these medicines.
Before you move on
- No label or guideline we read sets an end date. NICE's 2-year cap on Wegovy is about NHS funding.
- In US data, most people stopped within a year, mainly because of cost and side effects.
- The longest trials ran about 4 years, with more people stopping for side effects than on placebo.
- Five years of Mounjaro 5 mg is about ₹10.7 lakh at today's MRP, before visits and tests.
