Why are Mounjaro and Ozempic so expensive in India?
Patents, the 2026 generics, the price cuts that followed, what the medicines cost to make, and why no price ceiling or insurance policy brings the MRP down.

Mostly because of patents and missing competition. Tirzepatide (Mounjaro) is still under patent in India, expected to 2036, so only Eli Lilly makes it. Semaglutide (Ozempic, Wegovy) got cheaper after its patent expired in March 2026 and Indian generics arrived. No government price ceiling applies, and insurance rarely pays.
Key takeaways
- Semaglutide prices in India fell after its main patent expired on 20 March 2026, when Indian drugmakers launched versions up to 70% cheaper.1
- Novo Nordisk then cut Ozempic and Wegovy prices, by 23.8% and 27% on average, and Ozempic 0.25 mg fell from ₹8,800 to ₹5,660 for a pen of 4 doses.3
- Tirzepatide, the medicine in Mounjaro, is still under patent in India, with the main patent expected to expire in 2036.5
- A 2024 study estimated that GLP-1 medicines could be made and sold at a profit for $0.75 to $72.49 a month, well below prices in 13 countries.10
- Semaglutide and tirzepatide are not on India's 2022 essential medicines list, and standard health insurance pays only for hospital stays.11,14
On this page
- All GLP-1 medicines are prescription-only. A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.16
- Prices here are MRPs, the printed maximum retail price, never a seller's discount. We list brands to explain prices, not to rank them.
- A cheaper medicine is not automatically the right one: that is a medical choice. See Guide 6.
What do GLP-1 medicines cost in India now?
A pen of 4 weekly doses has a printed MRP from ₹5,660 for Ozempic or Wegovy at the starting dose, to ₹25,781 for Mounjaro at 15 mg.3,9
- Mounjaro (tirzepatide): ₹13,125 to ₹25,781 a KwikPen.9 A year at 15 mg is ₹3,35,153 (13 pens, our arithmetic).
- Wegovy (semaglutide): ₹5,660 to ₹16,400 a pen.3 A year at 2.4 mg is ₹2,13,200.
- Ozempic (semaglutide): ₹5,660 to ₹9,100 a pen.3
- Indian generic semaglutide: pens from ₹1,800 for 4 starting doses (Alkem), and vials from ₹1,290 a month (Natco).4,2
Doctor visits and blood tests come on top. What a year costs, all in adds them up for each medicine and dose.
Why did semaglutide get cheaper in 2026?
Its main Indian patent expired on 20 March 2026. Indian companies launched their own versions, and Novo Nordisk cut its prices within days.
Once semaglutide's patent ran out, at least half a dozen Indian drugmakers launched versions, some up to 70% cheaper, Reuters reported.1 Natco opened with vials from ₹1,290 a month on the day the patent expired.2
Novo Nordisk answered within days. Ozempic and Wegovy at 0.25 mg fell to ₹5,660 for a pen of 4 doses, from ₹8,800 and ₹10,848.3 The average cut was 23.8% for Ozempic and 27% for Wegovy.3 Emcure cut Poviztra, Novo's second semaglutide brand, by 55% at the starting dose.15
Reuters described the cuts as a response to competition from cheaper generics.1 What changed, brand by brand, is in what changed when the patent expired.
Why does Mounjaro still cost more?
Tirzepatide is still under patent in India, so Eli Lilly faces no generic competition. The main patent is expected to run until 2036.5
The Medicines Patent Pool, a public-health body, says the main tirzepatide patent is granted in India.5 Lilly's second brand, Yurpeak, is sold by Cipla, but Lilly makes it and Cipla said its price would match Mounjaro's.6 Our answer Is there a generic Mounjaro in India? covers what is sold online as "generic tirzepatide".
Mounjaro's MRP has fallen once, through tax. The KwikPen launched in August 2025 at ₹14,000 for 2.5 mg.7 GST on most medicines then fell from 12% to 5% from 22 September 2025.8 Today's ₹13,125 is the launch price with that tax change applied (₹14,000 × 105 ÷ 112, our arithmetic).9 We found no Lilly price cut beyond it.

What does it cost to make these medicines?
Far less than they sell for, by one estimate. A 2024 study put sustainable prices for GLP-1 medicines at $0.75 to $72.49 a month, including a profit.10
The researchers added up the cost of the active ingredient, making the finished product, running costs, tax and a profit margin, in 2023 US dollars.10 Those prices were "substantially lower" than what the medicines sold for in the 13 countries they surveyed.10 They concluded that strong generic competition could bring prices down.10
A model is not a price list. It shows the estimated gap between making a medicine, with a profit, and what it sells for.10
Does the government control GLP-1 prices?
Not with a ceiling. The government fixes ceiling prices for medicines on India's National List of Essential Medicines, and no GLP-1 medicine is on it.13,11
- The essential list. The 2022 list's diabetes section has glimepiride, insulins, metformin and teneligliptin, and no GLP-1 medicine.11 Semaglutide is not on it, and no government health scheme covers it, BusinessToday reported.12
- Price rises. For non-scheduled medicines, the price regulator NPPA limits MRP rises to 10% a year.13
- Tax. GST on most medicines has been 5% since 22 September 2025, down from 12%.8
Why doesn't insurance bring the cost down?
Standard Indian health insurance pays when you are admitted to hospital, and GLP-1 medicines are taken at home every week.14
Some policies add an outpatient (OPD) benefit; what it pays for depends on the policy.14 Does health insurance cover GLP-1 medicines in India? goes through policy types, and the insurance hub collects what we know.
Questions people ask
Why is semaglutide cheaper than tirzepatide in India?
Semaglutide's main Indian patent expired on 20 March 2026, and Indian companies launched their own versions. Tirzepatide's main patent is expected to run to 2036, and only Eli Lilly makes it, sold as Mounjaro and as Yurpeak through Cipla.1,5,6
Will Mounjaro get cheaper in India?
We found no announced price cut. Its MRP fell once, in September 2025, when GST on most medicines dropped from 12% to 5%. The main tirzepatide patent is expected to last until 2036.7,8,5
Why does Wegovy cost less than when it launched?
Wegovy launched in India in June 2025 at ₹17,345 to ₹26,050 a month. By March 2026 its pens were ₹10,850 to ₹16,400. After the patent expired and generics arrived, Novo Nordisk cut the lower strengths, to ₹5,660 to ₹16,400.17,3
Does the government fix the price of Ozempic or Mounjaro?
No. Neither is on India's National List of Essential Medicines, last revised in 2022. For medicines outside price ceilings, the government caps price rises at 10% a year.11,12,13
Does health insurance pay for Mounjaro or Ozempic?
Usually not. Standard Indian health insurance pays when you are admitted to hospital, and these are outpatient medicines.14
How we researched this
On 14 October 2026 we read dated reports of the 2026 semaglutide launches and Novo Nordisk's price cuts, the makers' releases, the Medicines Patent Pool's tirzepatide page, the 2022 essential medicines list, the GST Council's 2025 decision and statements on India's price-control rules. The manufacturing-cost estimate is from a peer-reviewed 2024 study.
Prices are MRPs from our price index. We found no Lilly statement on Mounjaro prices after the 2025 tax change, and no published Indian figure for what any maker spends to produce a pen.
Before you move on
- Semaglutide got cheaper after its Indian patent expired in March 2026.
- Tirzepatide's main Indian patent is expected to last until 2036, and Lilly is its only maker.
- Ceiling prices cover medicines on the essential list, and GLP-1s are not on it; their price rises are capped at 10% a year.
- Standard health insurance pays for hospital stays, not weekly medicines at home.
