How do I get a GLP-1 safely in India? Doctors, brands, generics and cost
First the doctor: who may prescribe, what a real consultation looks like, and the warning signs. Then the medicines: what's approved, generics, recalls, fakes and what a year costs.
GLP-1 medicines need a prescription in India. A government statement says only endocrinologists, internal-medicine specialists and cardiologists should prescribe them. A proper visit includes your history, weight and waist, blood tests and a follow-up plan. Many brands and generics exist, a few generic batches were recalled in July 2026, and insurance rarely pays.

The order this guide follows. The doctor comes first; the medicine comes second.
Who is allowed to prescribe a GLP-1 in India?
A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.1,59
An endocrinologist treats hormone problems, including diabetes. An internal-medicine specialist (MD Medicine) treats adult illness in general. A cardiologist treats the heart.
The same statement said these medicines can't be bought over the counter.1 It also said they must only be taken under a "registered medical practitioner/specialists".1
The national drug regulator, the Central Drugs Standard Control Organisation (CDSCO), issued an advisory on 10 March 2026 that says the same thing more broadly: prescription only, by registered doctors, for approved uses.2 Since then, officials have inspected 49 places, including online pharmacy warehouses and weight-loss clinics.1
What about my GP, an online doctor or an AYUSH doctor?
A GP (MBBS) is a registered doctor, but not one of the three specialties the government statement named. Online visits are allowed, but a first visit needs at least a video call for anything beyond simple medicines.
GP or family doctor. Your GP can check you, order tests and refer you. For the prescription itself, the statement named specialists.1 The safest route is a GP referral to one of them.
Online (telehealth). India's 2020 telemedicine rules sort medicines into lists.4 Simple ones, such as paracetamol, can be prescribed by chat or phone. Most others need at least a video call. Some need an in-person visit first.4 The rules don't say which list a GLP-1 is on.4
Under the same rules, the doctor can stop a video visit and ask to see you in person.4 Prescribing without a diagnosis counts as professional misconduct.4 An AI chatbot is not allowed to prescribe.4
AYUSH doctors. Ayurveda, homoeopathy, Unani and Siddha doctors are not among the specialists named.1 They work under their own councils and their own telemedicine rules.5
What should a proper consultation include?
Expect questions about your health and medicines, your weight and waist measured, blood tests, and a plan for follow-up visits.
There's no single official checklist for a GLP-1 consultation in India. The list below brings together what India's obesity guideline, from the Endocrine Society of India (ESI, 2025), and the medicines' labels say a doctor should check before prescribing.45,8,9
- Your history. The US labels rule these medicines out after medullary thyroid cancer in you or your family, or if you have MEN 2 (multiple endocrine neoplasia type 2, a rare inherited condition). Tell your doctor if this applies.8,9 Past pancreatitis or gallstones matter too.8
- Your other medicines. Insulin, or sugar pills like glimepiride, can cause low sugar (hypos) when combined with a GLP-1.8 The dose of those may need to drop.
- Weight and waist. A 2025 proposal to redefine obesity in India uses BMI plus waist size and any linked illness.7 Guide 1 explains the numbers.
- Blood tests. Which ones is the doctor's call. India's endocrinologists list fasting sugar, HbA1c (your 3-month sugar average), creatinine (a kidney test), cholesterol, liver and thyroid (TSH) tests. They say not everyone needs all of them.45 Kidney tests also matter because of kidney warnings on the label.8
- Eyes, if you have diabetes. The label warns about worse eye disease (retinopathy) in people who already have it.8
- Pregnancy plans. Semaglutide should be stopped at least 2 months before a planned pregnancy.8 Tirzepatide can make the pill less reliable, so a backup method is advised after starting and after each dose step.9
- Any surgery or procedure coming up. The doctor should ask if you have an operation, an endoscopy or anything needing sedation planned. These medicines slow how fast the stomach empties, so food can still be there under anaesthesia and be breathed into the lungs. The label says to tell the team doing the procedure that you take one.8
- A follow-up plan. Who you call with side effects, and when you're seen again as the dose goes up. India's guideline suggests a 3-month trial first, to see whether the medicine works and suits you.45
What are the red flags with a clinic or app?
Walk away if you can't see who the doctor is, if there are no tests, or if anyone promises kilos. Each of these breaks a rule or a basic standard.
-
No named doctor or registration numberDoctors must show it on prescriptions, websites and WhatsApp messages, and apps must list it.4The doctor's name and registration number are on the site, the app and your prescription. You can check them yourself (see below).
-
"No prescription needed"These medicines are prescription-only.1A doctor assesses you first, and the medicine is only supplied against their prescription.
-
A chat or call, no video, no testsA first online visit needs video for anything beyond simple medicines.4A video visit first, with questions about your health and medicines, and blood tests where needed.
-
"Lose 10 kg guaranteed"The regulator treats guaranteed weight-loss claims as a violation.2Honest ranges: results vary, and some people lose little.
-
You find and choose the clinic yourself. Nobody messages you to sell it.
-
The medicine is sold before you're assessedPaying a consultation fee up front is normal. A package that already includes the medicine means the answer was decided before a doctor saw you.You pay to see the doctor. The medicine is offered only if it's prescribed, and you're told what happens if the answer is no.

How to check a doctor's registration
Use the National Medical Commission's Indian Medical Register. You can search by name, registration number, year and state council.6 The same page has a list of blacklisted doctors.6 Check that the name, qualification and council match the prescription.
Questions to ask a provider
Take these to the visit. A good doctor will be glad you asked.
- What is your specialty and your registration number?
- Do I meet the medical criteria for this medicine, and why?
- What else should I try first or alongside it?
- Which tests do I need before I start, and when again?
- Is this medicine approved in India for my condition, or is it off-label?
- Which side effects should make me call you, and how do I reach you?
- How will my dose go up, and who decides each step?
- Does it affect my other medicines, my contraception or a pregnancy plan?
- What will it cost per month at the full dose, plus visits and tests?
- What happens when I stop, and how long would I take it?
Before you go further
- Prescription only. Never start one without a doctor.1
- Know the risks first. Guide 5: Side effects and risks.
- Check for recalls before using a generic pack. See July 2026 recalls.
- No promise of results. Weight loss varies a lot from person to person, and some people lose little.
We list brands and prices as facts. We don't recommend any brand, company or seller, and we earn nothing from them.
Which GLP-1 medicines are approved in India, and for what?
Four medicines are on sale. Semaglutide and tirzepatide have brands for weight; liraglutide and dulaglutide are sold here for type 2 diabetes.
Diabetes approved for type 2 diabetes Weight approved for weight management
- Weekly injectionSame medicine as Wegovy. Using it for weight without diabetes is "off-label", meaning outside its approved use in India.10
- Weekly injectionWegovy also gained two newer approvals: teens aged 12 and up who weigh over 60 kg (May 2026), and adults with fatty liver with scarring, called MASH (July 2026).38,39

Brand or generic: what's the difference?
A generic is the same medicine made by another company once the patent ends. Semaglutide's main Indian patent expired on 20 March 2026. Natco and Eris launched generics that day, and other makers followed on 21 March.11,14,15,62
Each generic needs its own approval from the Drug Controller General of India (DCGI) before it can be sold.16 Approved uses differ by brand. One company says its approval came after a large final-stage (Phase 3) trial run in India.16
Semaglutide is a peptide, a small protein. It is harder to make than an ordinary pill. We could not find a public summary of the exact tests the regulator asked each generic to pass. The July recalls (below) show that quality problems can happen.18
Companies that launched semaglutide in March 2026 include the following. The list is alphabetical, not a ranking.
| Company | Brand names | Form | Approved for |
|---|---|---|---|
| Alkem | Semasize, Obesema, Hepaglide | Pen (disposable and reusable) | Diabetes and weight16 |
| Dr Reddy's | Obeda | Pen | Diabetes14 · Batches recalled |
| Glenmark | GLIPIQ | Vial and pen | Diabetes14 |
| Lupin | Semanext, Livarise (made by Zydus) | Pen | Same as Zydus's brands14 |
| Natco | Semanat, Semafull | Multi-dose vial | Diabetes15 |
| Sun Pharma | Noveltreat; Sematrinity | Pen | Weight (Noveltreat); diabetes (Sematrinity)14 |
| Torrent | Sembolic, Semalix | Tablets and pens | Diabetes17,18 · Some pen batches recalled |
| USV | Usema | Injection | Not confirmed · Batches recalled18 |
| Zydus | Semaglyn, Mashema, Alterme | Reusable pen | Diabetes and weight14 |
Not a full list: 40+ brands had launched by end of April 2026. Liraglutide also has a generic, Lirafit (Glenmark), since January 2024.27
Switching between brands is a doctor's decision. Pens and vials of different brands come in different strengths, so the dose you dial or draw can change.
What happened with the July 2026 recalls?
In July 2026, three companies recalled some batches of their semaglutide injections. It was not a ban on semaglutide, and not every pack of these brands.18
| Brand | Company | What was recalled | Reason given |
|---|---|---|---|
| Obeda | Dr Reddy's | Selected batches; reported 10 July 202618 | An impurity linked to the active ingredient, found "out of specification"18 |
| Semalix pens, 2 mg and 4 mg | Torrent (made by Dr Reddy's) | Select batches only19 | Precaution after Dr Reddy's flagged the issue19 |
| Usema | USV | Select batches18 | Few details published18 |
Not affected, per Torrent: Semalix and Sembolic tablets, Semalix 8 mg injection (made by MSN), and the Sembolic reusable pen (made by Zydus).18,19
The companies said there was no risk to people already using the medicine.19 Dr Reddy's paused supply and expects to restart in late October or early November 2026.20 Its chief executive said what had already reached the market was safe.21
- Don't throw it away, and don't just stop. Find the batch number and expiry on the pack.
- Ask the pharmacy that supplied it whether that batch is recalled. Batch numbers weren't published in the reports we found.18
- Or contact the company directly. Torrent asked patients with questions to do this.19
- Tell your doctor before you switch brand, so they can match the dose.
- Report any side effect through the government's "ADR PvPI" app,36 or call the PvPI helpline on 1800 180 3024 (toll-free, working days, 9:00 am to 5:30 pm).64
Pen, vial or tablet: does it matter?
Yes. A pen measures the dose for you. A vial means drawing it up with a syringe, which is where dangerous dosing mistakes happen.
VialYou draw the doseCheaper, but you measure each dose with a syringe. Vials of semaglutide arrived in India in March 2026.15
TabletDaily, strict routineSemaglutide tablets go on an empty stomach with a little water. Wait 30 minutes before food.8Why vials carry a risk. In the US, the FDA reported overdoses from vials of compounded semaglutide.30 People mixed up millilitres, milligrams and syringe "units". Some injected 5 to 20 times the intended dose.30
In one example, 25 units were given instead of 5.30 Some people ended up in hospital with vomiting, fainting, dehydration, pancreatitis or gallstones.30
If you're given a vial, ask the doctor or pharmacist to show you the dose in units on your own syringe. Ask for the right size of syringe too.30
How do I spot a fake, and store it right?
Fake semaglutide has been found abroad. It may contain no medicine, or contain insulin instead. Only use packs from a licensed pharmacy against your prescription.31
The WHO warned in June 2024 about fake semaglutide found in Brazil, the UK and the US.31 Insulin in a fake pen can push blood sugar dangerously low.31
In India, CDSCO warned on 21 August 2025 that Novo Nordisk stock stolen in transit included Wegovy pens (0.25 mg batch RP5S232, 0.5 mg RP5S233, 1 mg RP5S210). Their quality may be compromised if they weren't kept at 2–8°C. It told patients to buy only from authorised sources, with a proper invoice.61
Never use a GLP-1 that came from a social-media seller, a WhatsApp group or an unverified website.31
Check the pack before you leave the counter
No QR code? That alone doesn't prove a fake. Since August 2023, 300 named top-selling brands must carry a QR or barcode,32 but that list came before most GLP-1 brands launched here. A 2026 update adds vaccines, antibiotics, cancer and narcotic medicines, not GLP-1s.33
What do GLP-1 medicines cost in India?
From about ₹1,800 to ₹25,800 for 4 weekly doses of the medicine alone, with Natco's vials cheaper, priced per month. The price rises as your dose goes up, so the starting price is not what you'll keep paying.
Prices below are the MRP (maximum retail price): the maker's printed list price for 4 weekly doses (one pen), or 30 days of a daily medicine, grouped by medicine. We count a year as 52 weekly doses, so a month is about 4.3 doses. Pharmacies may sell below it, but not above it. Where the maker hasn't published a price for a dose, we say so instead of guessing.

- Each price is for 4 weekly doses (one pen) or 30 days of a daily medicine. Where a source gave a price per pen, strip or week, we did the multiplication.
- MRP is the printed maximum price. Pharmacies may sell below it; selling above it isn't allowed.
- Pharmacy prices are read from the listings named under each brand and rechecked every week. Where no pharmacy lists a dose yet, we use the maker's announcement or a dated news report.
- Generic pens: the starting range runs from Alkem's pen of four doses at ₹1,800 to Dr Reddy's at ₹4,200; top-dose pens from ₹4,200 (Dr Reddy's 4 mg pen, four 1 mg doses, for diabetes) to ₹8,000 (Sun Pharma 2.4 mg, for weight). We found no newer maker or news figure since March 2026.16,53,52
- Liraglutide comes in a 3 mL pen of 18 mg, so 30 days takes 1 pen at 0.6 mg, 2 at 1.2 mg and 3 at 1.8 mg. Lirafit launched in January 2024 at about ₹100 a day at 1.2 mg; its listed MRP now works out to about ₹139 a day.27,58
GST on most medicines fell from 12% to 5% on 22 September 2025, and the National Pharmaceutical Pricing Authority (NPPA), which regulates medicine prices, told makers to pass the cut on as lower MRPs.55,56 Pharmacies list ₹13,125 for Mounjaro at 2.5 mg. That matches the August 2025 launch price of ₹14,000 after the September 2025 GST cut. Likewise, the 15 mg pen launched at ₹27,500 and is now listed at ₹25,781. Lilly has not published the new figures itself.29,54,55 One pharmacy still shows Victoza's old MRP of ₹5,324 next to the new ₹4,991, the same cut.26 All other prices above are from after the cut.
Semaglutide prices fell twice after generics arrived. Novo Nordisk announced cuts to Ozempic and Wegovy on 30 March 2026, and Poviztra was cut from 3 April.10,13 The biggest cuts were at the lowest dose; Wegovy's 2.4 mg price did not change.10

What does a year cost, all in?
The medicine is most of the bill, but visits and tests add up. Work out your own below, or see the worked example after it, with a dose that rises every 4 weeks and then holds. We count 6 months as 26 weekly doses and 12 months as 52. The figures use MRPs checked on 5 Oct 2026, so treat them as a dated example.
Work out your own cost
Choose a medicine. The cost calculator adds up the dose steps, doctor visits and tests for 6 or 12 months, at current MRPs, and you can change the plan.
Opens the cost calculator in a new tab. Nothing you choose leaves your device.A worked example. We used Wegovy because an MRP is listed for every dose step.10,57,23 This is an example for the sum, not a suggestion. The dose ladder follows the label: 4 weeks at each of 0.25, 0.5, 1 and 1.7 mg, then 2.4 mg.8 Your doctor may keep you lower.
| Item | 6 months | 12 months |
|---|---|---|
| Medicine (26 weekly doses, then 52) | ₹76,610 | ₹1,83,210 |
| Doctor visits (₹1,100 each, median fee on the doctor-booking site Practo; 6 visits, then 8)41 | ₹6,600 | ₹8,800 |
| Blood tests (₹2,440 a round, the median of 7 private lab chains; 2 rounds, then 3)43,44,47,48,49,50,51 | ₹4,880 | ₹7,320 |
| Total (MRPs checked 5 Oct 2026) | ₹88,090 | ₹1,99,330 |
Medicine, using MRPs checked on 5 Oct 2026: one 4-dose pen at each step, ₹5,660 + ₹8,100 + ₹9,100 + ₹12,750 = ₹35,610 for the first 16 weeks. Then 2.4 mg at ₹16,400 a pen, which is ₹4,100 a dose: 10 more doses by 6 months (₹41,000) and 36 by 12 months (₹1,47,600). The 0.25 and 2.4 mg prices come from the maker via Business Standard; 0.5 and 1 mg are the maker's weekly prices reported by Reuters, times 4; 1.7 mg is the MRP as listed by two pharmacies, checked 5 Oct 2026.10,57,23,24
Visits: one at the start and one at each of the 4 dose steps on the label (every 4 weeks), then a review at 6 months, when UK guidance says to judge whether to carry on.8,46 For 12 months we added reviews at 9 and 12 months; that every-3-months pace is our assumption. ₹1,100 is the median listed fee across 59 endocrinologists on the doctor-booking site Practo in 6 cities, checked 5 Oct 2026; fees ranged from ₹300 to ₹3,500. The median city fee ran from ₹850 in Hyderabad to ₹2,000 in Mumbai. General physicians had a median of ₹1,000 across 60 doctors.41
Tests: fasting sugar, HbA1c, creatinine, cholesterol (lipid profile), liver tests and TSH, from the Indian endocrinologists' list.45 We assumed a round at the start and at each 6-month review. One round costs ₹1,604–2,850 at 7 private lab chains, using the prices on their websites (Delhi for six, Mumbai for one), checked 5 Oct 2026.43,44,47,48,49,50,51 We used the middle (median) chain price, ₹2,440. Some chains show a higher crossed-out MRP; we used the price they charge, before coupons. A full kidney panel instead of creatinine alone can add up to ₹980 a round.44 The government scheme for its staff (CGHS) pays approved labs at most ₹1,386 for the same tests; that is not a walk-in price.42 Fees vary by city and clinic; swap in your own.
At generic pen launch prices of ₹1,800–4,200 for 4 weekly doses, 12 months (13 pens) of medicine would be ₹23,400–54,600 at starting doses.16,53 At the top-dose pen prices makers published, ₹4,200–8,000, it would be ₹54,600–1,04,000.53,52 Natco's vials are priced separately, from ₹1,290 a month for lower strengths to about ₹1,750 for the top strength.15 Budget in years, not months: Guide 9 covers what happens when you stop.
Will health insurance pay for it?
Usually not. Standard health insurance in India pays when you're admitted to hospital. Monthly medicines bought outside hospital are usually not covered.34
Some policies sell an outpatient (OPD) add-on, which may cover some medicines and tests.34,35 The insurance regulator's standard exclusion list covers weight-loss surgery, with exceptions such as a BMI of 40 or more.35 We found no rule requiring insurers to pay for GLP-1 medicines.
Read your policy wording, or ask your insurer in writing before you count on it.
The risks, in brief
- Feeling sick, vomiting, diarrhoea and constipation are common, mostly when the dose goes up.8
- Inflamed pancreas (pancreatitis): severe stomach pain that won't settle.8
- Gallbladder problems, including gallstones.8
- Losing too much fluid from vomiting or diarrhoea can harm the kidneys.8
- Ruled out by the US label for people with a personal or family history of medullary thyroid cancer or MEN 2; tell your doctor if this applies.8
- Not in pregnancy. Stop semaglutide at least 2 months before trying.8
Questions people ask
Is my GP's prescription valid?
Your GP is a registered doctor, but not one of the specialists named in the government statement of 1 April 2026.1 The rules are not fully clear; the section on who may prescribe explains. Ask your GP to refer you, so a specialist checks the decision.
Is an online prescription legal?
Online consultations are legal under the 2020 telemedicine rules.4 A first visit must be on video for anything beyond simple medicines. The doctor must show their registration number.4 A chat-only form or an AI bot is not a valid route.4
Are generics as good as the brand?
They contain the same medicine and need their own approval before sale.16 Some batches from three companies were recalled in July 2026 over quality.18 We could not find public data comparing each generic with the original. Ask your doctor what they know about the brand they prescribe.
Is Ozempic sold in India?
Yes, since December 2025, approved for type 2 diabetes.12 Its MRP is ₹5,660 to ₹9,100 for 4 weekly doses, depending on dose.10
Can I keep the pen out of the fridge?
Unopened pens go in the fridge. After first use, some pens can be kept out, but only below 30°C and for a limited number of days.8,9 Check your own pack's leaflet.
Does insurance cover Mounjaro or Wegovy?
Usually not, because standard policies pay for hospital stays.34 An OPD add-on might cover some of it; check your wording.35
Before you move on
- See an endocrinologist, internal-medicine specialist or cardiologist, with tests.
- Online is fine if it's video, with a named, registered doctor.
- Check the registration on the NMC register.
- Know what your medicine is approved for, and check generic packs against the recalls.
- With a vial, learn your dose in units on your own syringe.
- Budget for the full dose and for years, not the starting price.
