Why is Mounjaro so expensive in the UK?
Who sets the price, why it jumped in 2025, why there is no cheap generic yet, and why the NHS is rolling it out slowly. Official figures only: no seller prices, no seller names.
Written for the UK.
Eli Lilly sets Mounjaro’s UK list price: £133 to £330 for a 4-week pen, depending on dose, after a 2025 rise. No generic copy of tirzepatide is licensed in the UK yet. On the NHS you pay £9.90 an item in England or nothing elsewhere, but NHS access is being phased in, partly because of the cost.
Key takeaways
- The October 2026 Drug Tariff lists a 4-week Mounjaro pen at £133 at 2.5 mg, rising to £330 at 12.5 and 15 mg, excluding VAT.1
- Lilly raised its UK list price from 1 September 2025, saying the launch price had been set below the European average to avoid delaying NHS access.3
- The government said private pricing “is a matter for Lilly and for private providers”, while the NHS pays a confidential discounted price.3,2
- Generic liraglutide shows what copies can do: the Drug Tariff price is £50.49 for 2 pens, against £196.20 for 5 Saxenda pens.1,8
- NICE estimates 3.4 million people in England are eligible for NHS Mounjaro, and expects at least 220,000 to be funded in the first 3 years.5
- GLP‑1 medicines are prescription-only medicines in the UK, never sold over the counter.19

What does Mounjaro cost at list price?
A 4-week Mounjaro pen lists at £133 to £330 before VAT, so a year held at one dose costs £1,729 to £4,290.1
Each KwikPen holds 4 weekly doses, so a year is 13 pens.9 The Drug Tariff lists £133 at 2.5 mg, £180 at 5 mg, £255 at 7.5 and 10 mg, and £330 at 12.5 and 15 mg.1 The year totals are our arithmetic: pen price × 13.
On an NHS prescription you pay the charge, not the list price: £9.90 an item in England.12 Prescriptions are free in Scotland, Wales and Northern Ireland.13,14,15 Every strength is in How much is Mounjaro in the UK?
Who sets the price of Mounjaro?
Eli Lilly sets the list price; the NHS buys at a confidential discount, and each private service sets what it charges you.2,3
NICE publishes the list price “excluding VAT; company communication”, meaning Lilly supplied it.2 The NHS gets Mounjaro under a commercial arrangement, and the discount is confidential.2
When Lilly raised the price, the Department of Health said: “Pricing in the private market is a matter for Lilly and for private providers.”3 The GPhC, the pharmacy regulator, says access to weight-loss medicines “is usually via private services”.16
Why did the price jump in 2025?
Lilly raised its UK list price from 1 September 2025, from £92–£122 to £133–£330 a pen, citing price gaps with other rich countries.3
Lilly said the rise aimed to “address pricing inconsistencies compared to other developed countries, including in Europe”.3 It said the UK launch price had been “significantly below the European average to prevent delays in NHS availability”.3
The government said NHS commissioning “is unaffected by the change in list price”.3 Complaints followed: 78% of the pricing concerns the GPhC received came in August and September 2025.16 More in Why did Mounjaro’s price go up?

Why is there no cheap generic Mounjaro?
No generic tirzepatide is licensed in the UK: the October 2026 Drug Tariff lists tirzepatide only as Mounjaro KwikPen pens.1
A generic is a copy of a medicine made by another company. Copies can enter the market once the original’s patents and related protection end.6 We found no published UK end date for tirzepatide’s protection. Liraglutide shows what copies can do to price.
Biocon launched generic liraglutide in the UK on 28 February 2025, as Liraglutide Biocon and Biolide.7 The October 2026 Drug Tariff price for generic liraglutide is £50.49 for 2 pens.1 Saxenda, the original weight brand, lists at £196.20 for 5.8 Per pen that is £25.25 against £39.24, by our arithmetic.
Semaglutide, in Wegovy and Ozempic, is still protected. UK patent attorneys expect Novo Nordisk’s exclusivity in Europe to end in 2031.6 See Generic liraglutide vs Saxenda.
Why doesn’t the NHS give it to everyone who qualifies?
Because of the cost and the services needed: NICE estimates 3.4 million people in England are eligible, so NHS England is phasing it in.5
NICE judges value using a QALY, a year of life in full health. It treated £20,000 per QALY gained as the acceptable limit for Mounjaro.4 It noted a “high decision risk given the potential population size”.4
NHS England asked for more time. Its costs would exceed NICE’s £20 million-a-year budget test in each of the first 3 years.5 It also cited too few GP-based weight services and staff.5
NICE accepted that rollout may take up to 12 years, while saying it could well be shorter. At least 220,000 people are to be funded in the first 3 years.5 Who comes first is in Mounjaro on the NHS.
Why does the price go up with the dose?
Each strength has its own pen and list price, so the cost climbs as the dose steps up, from £133 at 2.5 mg to £330 at 15 mg.1
The UK label starts at 2.5 mg for 4 weeks, then 5 mg. Rises after that are 2.5 mg at a time, at least 4 weeks apart.9 A year climbing to 15 mg comes to about £3,793 at list price.1
That is our arithmetic: £133 + £180 + £255 + £255 + £330, then 8 more pens at £330. A prescriber may stop at a lower dose, which costs less. Our cost calculator adds up any plan.
- Stomach effects. In the SURMOUNT-1 and -2 weight trials pooled on the UK label, 25 to 29 in 100 people on 5 to 15 mg felt sick, against 9 in 100 on a dummy injection.9
- Gallstones are common (1 in 10 to 1 in 100) and pancreatitis uncommon (1 in 100 to 1 in 1,000) on the UK Mounjaro label.9,11
- The pill. Mounjaro can make it less reliable: the MHRA advises a barrier method for 4 weeks after starting and after each dose rise.10
- Pregnancy and surgery. Stop Mounjaro at least 1 month before a planned pregnancy, and tell the team before any anaesthetic.10
