How much do Wegovy, Zepbound and the GLP‑1 pills cost without insurance in the US?

Every maker’s published cash price, a year added up, and why the list price is a different number. No coupon sites, no telehealth prices, no seller names.

Written for the United States. Reading from India? India version

The short answer

Paying cash, the makers’ own prices are $349 a month for a Wegovy pen ($199 for the first 2 fills for new patients), $299 to $449 for Zepbound, and $149 to $299 for the Wegovy pill or Foundayo. List prices are far higher: $1,349 for Wegovy and $1,087 for Zepbound.

Key takeaways

  • Without insurance, Novo Nordisk charges $349 a month for a Wegovy pen at any dose up to 2.4 mg, and $399 for Wegovy HD 7.2 mg.1
  • Eli Lilly’s self-pay price for Zepbound vials and KwikPens is $299 at 2.5 mg, $399 at 5 mg and $449 at higher doses if each refill comes within 45 days.2
  • The two daily GLP‑1 pills for weight, the Wegovy pill and Foundayo, start at $149 a month and reach $299 at higher doses.1,3
  • A year of Zepbound climbing to 15 mg comes to about $5,637 at cash prices; a year of Wegovy pens for a new patient, about $4,237.2,1
  • Novo Nordisk will cut Wegovy’s list price to $675 a month on January 1, 2027, but says its self-pay prices won’t change.4
Before you go further
  • The prices below are the makers’ own published self-pay prices. We don’t quote coupon sites, pharmacies or telehealth services, and we don’t link to any seller.
  • Every one of these medicines needs a prescription. The makers’ cash offers say so too.3
  • Ozempic and Mounjaro are approved for type 2 diabetes, not for weight loss.5,6
  • A prescriber decides whether a medicine fits, which one and at what dose. Nothing here is a recommendation.
  • The risks are the same whatever you pay: see our side effects guide.

What do the makers charge if you pay cash?

Novo Nordisk and Eli Lilly publish monthly self-pay prices for every weight-loss GLP‑1: $149 to $449 a month, depending on the medicine and the dose.1,2,3

A self-pay or cash price is what you pay with no insurance involved. You still need a prescription. The price usually rises as your dose goes up.

Medicine and formMaker’s self-pay price per month
Wegovy pen Semaglutide, weekly injection$349 at every dose from 0.25 to 2.4 mg. New patients: $199 for the first 2 fills of 0.25 or 0.5 mg, through Dec 31, 20261
Wegovy HD 7.2 mg Semaglutide, weekly injection$3991
Wegovy pill Semaglutide, daily tablet$149 (1.5 mg), $199 (4 mg), $299 (9 and 25 mg)1
Zepbound vial or KwikPen Tirzepatide, weekly injection$299 (2.5 mg), $399 (5 mg), $449 (7.5 to 15 mg) if each refill comes within 45 days; otherwise $499 (7.5 mg) or $699 (10 to 15 mg)2
Foundayo Orforglipron, daily tablet$149 (0.8 and 2.5 mg), $199 (5.5 mg), $299 (9, 14.5 and 17.2 mg). The 2.5 and 5.5 mg prices are cut until Dec 31, 2026; the top two doses need a refill within 45 days3

Grouped by medicine, not ranked. A month is 4 weekly doses (28 days) or 30 tablets. Prices as published by Novo Nordisk and Eli Lilly, checked Oct 12, 2026.1,2

The 45-day rule matters. Lilly’s higher-dose Zepbound price of $449 depends on refilling within 45 days of your last delivery. Miss it and the regular $699 price applies at 10 mg and above.2

Why is the list price so much higher?

The list price is what a maker charges wholesalers before insurer discounts: $1,349 a month for Wegovy, $1,087 for Zepbound and $1,028 for Ozempic.7

The government price site TrumpRx shows these as each drug’s “Original Price”, next to the lower cash price.7 Few people pay the list price itself. Insurers negotiate rebates, and cash buyers use the makers’ lower self-pay prices.

The list price still matters if you have insurance with a deductible or coinsurance, because your share can be worked out from it. That is the reason Novo Nordisk gave for its cut. From January 1, 2027, Wegovy, Ozempic and Rybelsus will list at $675 a month, about 50% less for Wegovy and 35% less for Ozempic.4

Novo Nordisk said the change does “not have an impact on direct-to-patient, self-pay prices”.4 Our post on where GLP‑1 cash prices really come from explains the makers’ own programs and the government price site.

What does a year cost at cash prices?

Paying cash, a year of Wegovy pens costs about $4,237 for a new patient, and a year of Zepbound climbing to 15 mg about $5,637.1,2

A year of weekly doses is 13 four-week fills, not 12. A year of daily pills is about 12 bottles of 30. Wegovy pens step up every 4 weeks to 2.4 mg by week 17.8 Zepbound starts at 2.5 mg and goes up 2.5 mg at a time, after at least 4 weeks on each dose.9

$4,237Wegovy pens, new patient: 2 fills at $199, then 11 at $3491
$5,087Zepbound, held at 5 mg after the first fill2
$5,637Zepbound, stepping up to 15 mg2
$3,338Wegovy pill, stepping up to 25 mg, 12 bottles1

Our arithmetic at the prices checked on Oct 12, 2026. Zepbound to 15 mg = $299 + $399 + 11 × $449, with every refill inside 45 days. Wegovy pill = $149 + $199 + 10 × $299. Doctor visits, lab tests and shipping are left out.

Two of the offers end on December 31, 2026: Wegovy’s $199 starter fills and Foundayo’s lower 2.5 and 5.5 mg prices.1,3 A year that starts now runs past that date, so check the price again before each new step.

At the list price, 13 Wegovy fills would come to $17,537.7 Our stopping guide explains why many people budget in years, not months.

RisksThe risks, in brief
  • Stomach effects. In the main trials, about 44 in 100 people on Wegovy 2.4 mg injection felt sick (nausea), against about 16 in 100 on a dummy injection (placebo);8 stomach and gut side effects affected about 74 in 100 people on the Wegovy 25 mg pill, against about 42 in 100 on a dummy pill;10 about 25 to 29 in 100 people on Zepbound felt sick, against about 8 in 100 on a dummy injection;9 about 26 to 35 in 100 people on Foundayo felt sick, against about 10 in 100 on a dummy pill.11 It mostly comes while the dose goes up.
  • Gallstones were reported in 16 in 1,000 people on Wegovy injection, against 7 in 1,000 on placebo;8 11 in 1,000 on Zepbound, against 10 in 1,000 on placebo, with gallbladder inflammation in 7 in 1,000 against 2 in 1,000;9 10 in 1,000 on Foundayo, against 7 in 1,000 on placebo.11
  • Pancreatitis (an inflamed pancreas): severe stomach pain that won’t go away and may spread to the back. The labels say to stop the medicine if it is suspected.8,9,11
  • Thyroid warning. The labels carry a boxed warning: these medicines are not for people with a personal or family history of medullary thyroid cancer (MTC) or MEN 2, a rare inherited condition that causes gland tumors.8,9,11
  • Dehydration from vomiting or diarrhea can harm the kidneys. Tell the team before any surgery or sedation, because a slower-emptying stomach raises the risk of breathing in food or fluid.8,9,11
  • Pregnancy. The Wegovy label says to stop at least 2 months before a planned pregnancy;8 Zepbound and Foundayo can make birth-control pills less reliable after starting and after each dose step: for 4 weeks with Zepbound, 30 days with Foundayo.9,11

What about Ozempic and Mounjaro?

Ozempic and Mounjaro are the diabetes brands of semaglutide and tirzepatide; their self-pay prices are $199 to $499 a month.13,14

Ozempic contains the same medicine as Wegovy, semaglutide, at lower doses. The FDA approved it for type 2 diabetes, not for weight loss.5 Mounjaro contains tirzepatide, the medicine in Zepbound, and is also approved for type 2 diabetes.6

  • Ozempic pen: $199 a month for the first 2 fills of 0.25 or 0.5 mg for new patients, through Dec 31, 2026. Then $349 at 0.25 to 1 mg and $499 at 2 mg.13
  • Mounjaro pen: $499 a month at every dose.14

A doctor can prescribe a medicine for a use that isn’t on its label (off-label use). Makers can’t advertise those uses. Whether that suits you is a question for your prescriber. Our guide to the medicines sets out which brand is approved for what.

Is paying cash ever better than using insurance?

It depends on your plan: makers’ savings cards can bring an insured copay down to $25, but they exclude Medicare and Medicaid, and cash payments may not count toward your deductible.1,15,16

If your plan covers it, Novo Nordisk’s Wegovy savings card means you “pay as little as $25”, with savings capped at $100 a month.1 Lilly’s card offers “as little as $25” for a Zepbound pen fill, up to $1,300 a year.15 Getting the plan to agree usually needs a prior authorization: see how prior authorization works.

If you have Medicare or Medicaid, the savings cards don’t apply. Novo Nordisk’s terms say “Government beneficiaries excluded”.1 Medicare’s own GLP‑1 Bridge charges $50 a month to those who meet its rules.17

If you pay cash while insured, payments through TrumpRx don’t count toward your deductible, one expert told Managed Healthcare Executive.16 Ask your plan before you choose.

Compounded semaglutide and tirzepatide are sold for less by some sellers. They are not FDA-approved, and we don’t list their prices: see compounded GLP‑1s in 2026.18

Questions people ask

How much is Wegovy without insurance?

Novo Nordisk charges $349 a month for Wegovy pens at any dose up to 2.4 mg, and $399 for Wegovy HD 7.2 mg. New patients pay $199 for the first 2 fills of 0.25 or 0.5 mg, through Dec 31, 2026.1

How much is Zepbound without insurance?

Lilly lists Zepbound vials and KwikPens at $299 for 2.5 mg and $399 for 5 mg. Higher doses are $449 if you refill within 45 days; otherwise $499 for 7.5 mg and $699 for 10 to 15 mg.2

Is the Wegovy pill cheaper than the Wegovy shot?

At the start, yes: the 1.5 mg pill is $149 a month. At the top doses, the 25 mg pill is $299 against $349 for the pen.1

Can I use the Wegovy savings card with Medicare?

No. Novo Nordisk’s savings card terms say government beneficiaries are excluded. Medicare has its own $50-a-month GLP‑1 Bridge for people who meet its rules.1,17

Will Wegovy get cheaper in 2027?

Its list price drops to $675 a month on January 1, 2027. Novo Nordisk says the cut does not change its self-pay prices.4

How we researched this

We took every price from the makers’ own pages (Novo Nordisk’s NovoCare price guide and Eli Lilly’s self-pay pages) and the government’s TrumpRx listings, all checked on October 12, 2026. We left out coupon, pharmacy and telehealth prices, which change often and can depend on where you buy.1,2,7

Year totals are our arithmetic from those prices and the dose schedules in each US label.8,9

Before you move on

  • Price the whole dose ladder, not the first fill.
  • Count 13 fills a year for weekly pens.
  • Note which offers end on Dec 31, 2026.
  • Ask your plan whether cash payments count toward your deductible.
  • Check Medicare’s $50 Bridge if you’re on Medicare.
