What is Optum Rx, and does its list decide your coverage?
Optum Rx is a pharmacy benefit manager in UnitedHealth Group; it publishes standard drug lists, but your plan decides what it covers.9,5,1
A formulary is a list of covered prescription drugs, sorted into cost levels called tiers. Optum Rx’s is chosen by a committee of doctors and pharmacists, and your employer or plan sponsor sets what each tier costs.1 Optum Rx says drugs may move to a higher tier or be excluded on January 1 or July 1 each year.1 It runs more than one standard list, including Premium and Select.1,3
Where a list and your plan differ, “the benefit plan documents rule.”2 An excluded drug “may not be covered or need prior authorization.”1
Where do Wegovy and Zepbound sit on Optum Rx’s list?
Optum Rx’s Premium Standard formulary from July 1, 2026 lists Wegovy, Wegovy HD and the single-dose Zepbound pen, each with prior authorization and a quantity limit.1
Foundayo, Saxenda and generic liraglutide for weight management are listed the same way.1 The July 1, 2026 exclusion list excludes Zepbound KwikPens and vials. It names Saxenda, Wegovy and single-dose Zepbound pens as alternatives.2
| Medicine | Premium Standard formulary (July 1, 2026) |
|---|---|
| Wegovy, Wegovy HD | Listed; prior authorization and quantity limit1 |
| Zepbound single-dose pen (auto-injector) | Listed; prior authorization and quantity limit1 |
| Zepbound KwikPens and vials | Excluded2 |
| Foundayo, Saxenda, liraglutide | Listed; prior authorization and quantity limit1 |
| Contrave | Excluded; Wegovy and Zepbound pens among the alternatives2 |
| Ozempic, Mounjaro, Rybelsus, Trulicity | Listed (diabetes medicines); prior authorization and quantity limit1 |
On Optum Rx’s Select list, Wegovy and Zepbound also need prior authorization, with a limit of 4 syringes per 28 days.3
What has Optum Rx changed, and when?
Optum Rx’s 2026 documents keep Wegovy and single-dose Zepbound pens on its lists, while excluding Zepbound KwikPens and vials from July 1, 2026.1,2
- Jan 1, 2026Optum Rx’s Select formulary prior authorization list includes Wegovy and Zepbound, each limited to 4 syringes per 28 days.3
- Jul 1, 2026The Premium exclusion list excludes Zepbound KwikPens and vials.2 The Premium Standard formulary lists Wegovy, Wegovy HD, Foundayo and the single-dose Zepbound pen.1
- Oct 1, 2026UnitedHealthcare’s updated weight-loss program for its own plans took effect, covering Foundayo, Wegovy injection and tablets, Wegovy HD and Zepbound.4
We found no Optum Rx announcement about Wegovy or Zepbound. The dates above come from Optum Rx’s own lists.
What approval rules has Optum Rx published?
We found no public copy of Optum Rx’s own weight-loss approval criteria for commercial plans; UnitedHealthcare, a sister company in UnitedHealth Group, publishes the rules it uses for its plans.5,4
Prior authorization means your doctor must tell Optum Rx “why you are taking a medication” so it can decide on coverage.3 Optum Rx’s lists mark Wegovy and Zepbound as needing it.1,3
UnitedHealthcare calls its program “an optional program” for clients that “have elected to cover weight loss products.”4 For weight, it asks for:4
- Lifestyle changes alongside the drug, such as diet, exercise or behavior support.4
- A BMI of 30 or more (body mass index, your weight compared with your height), or 27 or more with a weight-related condition. Examples given: high cholesterol, high blood pressure, type 2 diabetes, sleep apnea.4
- First approval: 5 months for Wegovy, 6 months for Zepbound.4
- Renewal: at least 5% of starting weight lost, plus continued lifestyle changes; renewals last 12 months.4 A one-time 6-month renewal can cover the time it takes to reach a maintenance dose.4
For someone who started at 220 lb (100 kg), 5% is 11 lb (5 kg). The same program covers Wegovy for heart risk and MASH, a liver disease, and Zepbound for sleep apnea.4 It also says Wegovy tablets “are typically excluded from coverage.”4
These are UnitedHealthcare’s rules, not Optum Rx’s rules for every plan. Optum Rx’s lists say that where a list and your plan differ, “the benefit plan documents rule.”2 More on non-weight uses: Wegovy for heart and fatty liver and Zepbound for sleep apnea.

How do you find your own plan’s version?
Call the number on your member ID card, or log on to the Optum Rx app, for your plan’s current list and prices.1
- 1Call the number on your member ID card for the most current list of covered drugs.1
- 2Log on. Use your plan’s website or the Optum Rx app to look up lower-cost alternatives and compare prices.1
- 3Ask three things: is weight management covered, which Zepbound forms are covered, and what the prior authorization criteria are.
- 4Your prescriber sends the request, explaining why you need the drug.3
- 5If you need it before a decision, ask your pharmacy for 5 days or less. You pay the full cost, Optum Rx says.3
What can you do if your plan says no?
Ask for the reason in writing, check it against the plan’s criteria, and appeal: federal rules give you at least 180 days after the denial to file an internal appeal.6,7
If your coverage comes through a job, a federal rule says the denial must give you the guideline used. Or it must say one was used, and that you can have a free copy.7 You can also ask for free copies of the documents behind your claim.7
To keep taking an excluded drug, Optum Rx says you, your representative or your doctor can ask for coverage by calling the number on your member ID card.1 If it’s approved, you may pay a higher cost; if not, you may pay the full cost.1
If the plan still says no after its internal appeal, you can ask for an independent external review.6 Exceptions are explained in reading your drug list and asking for an exception. Appeals are in how GLP‑1 appeals work. If weight-loss drugs aren’t a benefit at all, see GLP‑1 costs without insurance.
Questions people ask
Does Optum Rx cover Ozempic or Mounjaro?
Optum Rx’s Premium Standard formulary from July 1, 2026 lists Mounjaro, Ozempic, Rybelsus and Trulicity, each with prior authorization and a quantity limit.1
Does Optum Rx cover Zepbound vials or KwikPens?
Not on its July 1, 2026 Premium standard list, which excludes both. It names single-dose Zepbound pens, Wegovy and Saxenda as alternatives.2
Can I get a few days’ supply while I wait for approval?
Optum Rx says you can ask your pharmacy for a short-term supply of 5 days or less. You pay the full cost at that time.3
Is Optum Rx the same as UnitedHealthcare?
No. Both are UnitedHealth Group businesses: UnitedHealthcare offers health benefits, and Optum Rx handles prescription medicines.5
Does this apply to Medicare plans?
This page covers Optum Rx’s commercial lists, not Medicare drug plans. By law, Medicare can’t cover drugs used only for weight loss; the temporary Medicare GLP‑1 Bridge, from July 1, 2026, to December 31, 2027, is the exception. Part D plans can cover GLP‑1s for type 2 diabetes, heart risk or sleep apnea.8 See how Medicare covers GLP‑1s.
How we researched this
We read Optum Rx’s own documents: its 2026 Premium Standard formulary, its July 2026 exclusion list and its Select prior authorization list.1,2,3 Two are copies hosted for client plans; each carries Optum Rx’s document number. We also read UnitedHealthcare’s published program.4
We searched for Optum Rx’s own prior authorization criteria for Wegovy and Zepbound and found no public copy for commercial plans; the Optum Rx forms we found were for state Medicaid programs. Appeal rights come from HealthCare.gov and the federal claims rule. All sources were checked on Oct 14, 2026.6,7
Before you move on
- Ask HR or the number on your card whether weight-loss drugs are covered at all.
- Ask which forms are covered: single-dose pens, vials or KwikPens.
- Ask for the prior authorization criteria for your plan, in writing.
- Collect proof of your starting BMI and any weight program you’ve done.
- If you’re denied, check the appeal deadline in the denial letter, and appeal in writing.
