Can you carry an Ozempic or Mounjaro pen on a flight?
Yes, in your hand luggage. India's airport security list does not name pens or needles as banned. It exempts medicine carried with a prescription from the 100 ml liquid limit. Avoid checked bags. Eli Lilly says the hold may be too hot or too cold. The US labels for Ozempic (semaglutide) and Mounjaro (tirzepatide) say never use a frozen pen.
- GLP‑1 medicines are prescription-only in India, not sold over the counter.13
- A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.13 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.14,15
- What a prescription involves is in Guide 6: Seeing a doctor, and the medicines in India.
- Nothing here says whether either medicine is right for you.
What does India's airport security list say about pens and needles?
India's list of items banned from hand baggage names knives, blades and pointed scissors, but no syringes, needles or medicine pens.1,2
The same list caps liquids at 100 ml per container but exempts “Medicine/inhaler accompanied by prescription”.1 An Ozempic pen holds 3 ml and a Mounjaro KwikPen 2.4 ml (US labels).7,8 Both are far below that limit.
We could not open India's official list on the Bureau of Civil Aviation Security (BCAS) website. It returned errors on 5 Oct 2026. We read BCAS's list through the Internet Archive instead (copies from 2019 and Sep 2024, same wording).1 Airports Authority of India pages repeat the same wording today. They call it an “indicative” list, so the officer at the checkpoint still decides.2
What do Air India and IndiGo allow?
Both allow medicines in the cabin, and Air India lists diabetes pens, vials and preloaded syringes as allowed in cabin and checked bags.3,4
Air India's list is written for insulin and does not name GLP‑1 medicines. Its items are allowed “only after inspection”.3 For prescribed medicine it asks for a doctor's note.3 The name on the label should match your boarding pass.3
IndiGo's table allows “Medicines (with proper prescription)” in both cabin and checked bags.4 It recommends that all medication goes in cabin baggage, and takes no responsibility for it in checked bags.5
The same IndiGo table bars “Sharp items” and “Medical Equipment” from the cabin, without saying whether pen needles count.4 Air India names pens and syringes; IndiGo doesn't, so the officer at the checkpoint decides.3,4
Why shouldn't the pen go in checked baggage?
Eli Lilly says the hold may be too hot or too cold for the Mounjaro KwikPen, and both labels say never use a frozen pen.6,7,8
Lilly says airport X-rays are not known to affect tirzepatide.6 If you use a cool bag, don't let the pen touch ice or frozen gel packs.6 The Ozempic label says to keep your pen with you.7 It also says to carry a spare pen and new needles in case of loss or damage.7 Diabetes UK suggests splitting your medicines between two bags.9
A pen you have started using doesn't need a fridge on a trip, within the label limits. The US label gives Ozempic 56 days at 15–30 °C after first use; the EU label gives the 4-dose pen 6 weeks below 30 °C.7,16 An opened Mounjaro KwikPen lasts 30 days at up to 30 °C.8 The full storage rules are in Guide 8: Life on a GLP‑1 and How long can Mounjaro stay out of the fridge?
What papers should you carry, including on flights to the US?
Carry your prescription and a doctor's note, because India's list exempts medicine with a prescription and Air India asks for a note.1,3
For flights abroad, Lilly says to check the destination country's rules with its embassy.6
In the US, the airport security agency (TSA) allows unused syringes with injectable medicine, and you must declare them.10 It allows larger amounts of medical liquids if you declare them.11 Used needles must travel in a sharps box or other hard container.12
