Can you take Ozempic if you don't have diabetes?

The short answer

No, not as an approved use. India approved Ozempic (semaglutide) in September 2025 for adults with type 2 diabetes only. Wegovy (higher-dose semaglutide), Mounjaro (tirzepatide) and some generic semaglutide brands carry Indian approvals for weight management. Using Ozempic for weight would be off-label, and only a doctor can make that call.

Before you go further
  • GLP‑1 medicines are prescription-only in India, not sold over the counter.6
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.6 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.14,15

What is Ozempic approved for in India?

India's drug regulator approved Ozempic in September 2025 for adults with type 2 diabetes, alongside diet and exercise, and not for obesity.1

The approval was announced on 26 Sep 2025, for 0.25 mg, 0.5 mg and 1 mg strengths.1 Its India launch was reported on 12 Dec 2025.9 The approval report adds that Ozempic can cause some weight loss, but "it is not approved for obesity management".1 We found no public Indian label to check this against. Every use on the US label is also for adults with type 2 diabetes.2

Which GLP-1 brands are approved for weight in India?

Wegovy (semaglutide, the same medicine as Ozempic) and Mounjaro (tirzepatide) are among the brands with Indian approvals for long-term weight management.3,4

Mounjaro launched in India on 20 Mar 2025. It is approved for type 2 diabetes, and for weight at a BMI of 30 or more.3 The weight cut-off drops to 27 with a weight-related condition.3 Wegovy launched on 24 Jun 2025, for weight and to lower the risk of heart attack and stroke.4 It comes in 5 strengths up to 2.4 mg. Ozempic's Indian strengths stop at 1 mg.1,4

Wegovy and Mounjaro each have a second brand. Poviztra is Novo Nordisk's semaglutide for weight, sold by Emcure.11 Yurpeak is Lilly's tirzepatide, sold by Cipla for weight and diabetes.12 Novo Nordisk's main Indian semaglutide patent expired in March 2026, and many companies launched generic versions.10 Some are approved for weight; others for diabetes only.10 Guide 6: Seeing a doctor, and the medicines in India covers them all. What does "off-label" mean for Ozempic?

Off-label means a doctor prescribes an approved medicine for a use or dose its label doesn't cover, such as Ozempic for weight.5

The US drug regulator (FDA) says doctors "generally may prescribe" a drug off-label when they judge it "medically appropriate".5 We found no published Indian rule on off-label GLP‑1 use. The prescriber limits are in the box above.

When might a doctor consider a GLP-1 without diabetes?

India's ESI 2025 obesity guideline says a doctor may consider medicine at a BMI above 27, or above 25 with a weight-related condition.7

Those conditions include high blood pressure, high cholesterol and sleep apnoea.7 Meeting a number doesn't make a medicine right for you. The guideline says counselling on benefits, limits, side effects and cost must come first.7 Guide 4 covers who should not take these medicines.

In the STEP 1 trial, 1,961 adults without diabetes took semaglutide 2.4 mg or a dummy injection for 68 weeks. Both groups also got diet and activity advice.8 Their BMI was 30 or more, or 27 or more with a weight-related condition, higher than the ESI cut-offs.7,8 Weight fell by 14.9% (15.3 kg) on average, against 2.4% (2.6 kg) on the dummy.8 More people stopped because of stomach side effects: 4.5% vs 0.8%.8 STEP 1 used 2.4 mg, a Wegovy strength, not one Ozempic was approved in.1,4 A year after stopping, people had regained two-thirds of the weight they lost.13 Guide 9 covers stopping.

RisksThe risks, in brief
  • Stomach effects are common. Nausea and diarrhoea were the most common side effects in STEP 1.8
  • Pancreatitis (a swollen pancreas), sometimes fatal, has been seen on Ozempic (US label).2
  • Gallbladder disease, and kidney injury mostly in people dehydrated by vomiting or diarrhoea.2
  • Low blood sugar is more likely when it is taken with insulin or a sulfonylurea (US label).2
  • Thyroid warning. Ruled out by the US label for anyone with a personal or family history of medullary thyroid cancer or MEN 2; tell your doctor if this applies.2
  • Pregnancy. The US label says stop at least 2 months before a planned pregnancy.2
