Surgery led to more weight loss: 25% after 10 years with gastric bypass in one Swedish study. Weekly GLP‑1 injections gave 15% to 21% in 68- to 72-week trials, and most of the weight returned after stopping. Surgery costs about ₹2 to 6 lakh once; a year of medicine at a weight dose costs about ₹1 to 3.4 lakh.
Key takeaways
- We found no reported randomised trial of surgery against GLP‑1 medicines; in separate studies, gastric bypass gave 25% loss at 10 years, and tirzepatide 15 mg gave 21% at 72 weeks.12,17,21
- In a large US study of first-time bypass and banding operations in 2005–07, 0.3% of people died within 30 days, and 4.3% had at least one major problem.15
- In the STEP 1 trial of semaglutide 2.4 mg (the Wegovy dose), 44% felt sick against 17% on a dummy injection.16
- Eight private hospitals publish bariatric surgery prices between about ₹2 lakh and ₹6 lakh, while Wegovy's first year on the label's dose steps costs ₹1,83,210 at MRP.22,28,30,31,32
- Some Indian health policies we checked still use IRDAI's 2019 wording, which pays for obesity surgery only at a BMI of 40, or 35 with one of four serious conditions.40,48,49

How do bariatric surgery and GLP-1 medicines work differently?
Surgery makes the stomach smaller or reroutes food past part of the gut, while GLP‑1 medicines copy a gut hormone that lowers appetite.1,2
In a sleeve gastrectomy, the surgeon removes about 80% of the stomach, leaving a narrow tube.1 In a gastric bypass, a small pouch at the top of the stomach is joined to the lower small intestine.1
India's surgeons' society counted 20,242 operations in 2018: 48% sleeve, 34% mini (one-anastomosis) bypass and 15–16% standard bypass.3
GLP‑1 is a gut hormone that helps control appetite and how much you eat.2 Semaglutide copies it. Tirzepatide acts on GLP‑1 and a second gut hormone, GIP.18 Both are weekly injections, taken for as long as prescribed (what GLP‑1 medicines do).
- GLP‑1 medicines are prescription-only in India.4
- Who may prescribe them: A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.4 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.5 More in Do you need a prescription for GLP‑1 medicines in India?
- Know the risks first: Guide 5: Side effects and risks.
- Check for recalls before using a generic GLP‑1 pack. Some batches were recalled in July 2026.6
- No promise of results. The numbers below are trial and study averages. Some people lose much less.
Who is bariatric surgery considered for in India?
Asian surgical guidelines, which India's surgeons follow, say surgery should be considered at a BMI of 35, or 30 with poorly controlled diabetes.8,9
In Indians, overweight starts at a BMI of 23 and obesity at 25.7 The Asia-Pacific surgeons' consensus (IFSO-APC, 2011) gives three levels for Asian patients:8
- BMI 35 or more: surgery should be considered, with or without other health problems.8
- BMI 30 or more: surgery should be considered for type 2 diabetes or metabolic syndrome that lifestyle and medicines haven't controlled.8
- BMI 27.5 or more: surgery may be considered as a second-line option for poorly controlled diabetes.8
The Obesity and Metabolic Surgery Society of India (OSSI) follows these levels. It adds that a large waist with weight-related problems may count: 90 cm (35.4 inches) or more in men, 80 cm (31.5 inches) in women.9 A 2022 statement from US and international surgeons says Asian people with a BMI above 27.5 should be offered surgery.10
The medicine thresholds are separate, in the Indian obesity guideline (ESI 2025). It says a doctor may consider a weight-loss medicine at a BMI above 27, or 25 with a related condition.11 Meeting a number doesn't make a treatment right for someone; a doctor or surgical team assesses each person.
How much weight do people lose after surgery, and on GLP-1 medicines?
Bypass and sleeve gave 19% to 25% loss 5 to 10 years after surgery; GLP‑1 trials found 15% to 21% at 68 to 72 weeks.12,13,16,17
After surgery
The Swedish Obese Subjects (SOS) study followed 4,047 people, half with surgery, for about 11 years.12 Gastric bypass led to 32% loss at its peak, 1 to 2 years after the operation. After 10 years, it was 25%.12 For a 90 kg person, 25% is about 23 kg.
Two older operations, gastroplasty and banding, led to 16% and 14% after 10 years. The usual-care group stayed within 2% of its starting weight.12 Fewer people died in the surgery group: 101 vs 129 deaths over the study.12 SOS was not randomised, so the groups may have differed in other ways.
The STAMPEDE trial randomised 150 people with type 2 diabetes and a BMI of 27 to 43.13 After 5 years, weight was down 23% after bypass and 19% after sleeve. Medicines alone gave 5%.13
Surgery has its own risks: in a 10-year Finnish trial, 16% to 19% needed another operation.14 The surgery risks section gives more.
On GLP‑1 medicines
The STEP 1 trial tested semaglutide 2.4 mg, the Wegovy dose, in 1,961 adults without diabetes.16 After 68 weeks, they had lost 15% of their weight, against 2% on a dummy injection.16 For a 90 kg person, that's about 13 kg vs 2 kg. But 44% felt sick (nausea), against 17%.16
The SURMOUNT-1 trial tested tirzepatide, sold for weight as Zepbound in the US. On the 15 mg dose, people lost 21% after 72 weeks, against 3% on the dummy.17 In India, tirzepatide is sold as Mounjaro, for diabetes and obesity.33 About 11% of SURMOUNT-1's volunteers were Asian.17
Much of the weight returns when the medicine stops.19 Our post on weight lost on GLP‑1 medicines lists every trial.
- Stomach effects are common. In the Zepbound weight trials, 28% on 15 mg felt sick, against 8% on the dummy.17
- Pancreatitis and gallstones. Label bands and trial rates are in the medicine risks section.44,45
- Dehydration from vomiting or diarrhoea can lead to kidney injury.17
- Thyroid warning. Ruled out by India's ESI 2025 obesity guideline for people with a personal or family history of medullary thyroid cancer (MTC) or MEN 2; tell your doctor if this applies.11
- Not in pregnancy or breastfeeding.11
Has any trial compared surgery with GLP-1 medicines directly?
Not that we found: a US randomised trial of surgery vs semaglutide vs tirzepatide is under way, with main results due after July 2027.21
That trial has finished enrolling about 140 adults aged 18 to 70, with a BMI of 35 to 65. Its main measure is weight loss after 52 weeks.21 Until then, the numbers here come from separate studies in different people.
A 2026 US study looked back at 44,025 people with a BMI of 35 or more.20 After 2 years, weight was down 22% after sleeve and 28% after bypass. It was down 7% on semaglutide and 11% on tirzepatide.20 It was not a trial, and it counted people who stopped their medicine.20
| Weight lost in a main study | Main risks | Cost in India | |
|---|---|---|---|
| Sleeve gastrectomy | 19% at 5 years (STAMPEDE)13 | Operation risks; another operation in 16% by 10 years; reflux damage to the food pipe14,15 | About ₹2–5.5 lakh, once (7 hospitals)22,23,24,25,26,27,29 |
| Gastric bypass | 25% at 10 years (SOS); 23% at 5 years (STAMPEDE)12,13 | Operation risks; another operation in 19% by 10 years14,15 | About ₹2–6 lakh, once (6 hospitals)22,23,24,27,28,29 |
| Semaglutide 2.4 mg (Wegovy dose) | 15% vs 2% at 68 weeks (STEP 1)16 | Nausea in 44% vs 17%; pancreatitis and gallstones; weight returns after stopping16,19,44 | Wegovy ₹1,83,210 in year 1 at MRP, then ₹2,13,200 a year (our sum)30,31,32 |
| Tirzepatide 15 mg (Zepbound in the US, Mounjaro in India) | 21% vs 3% at 72 weeks (SURMOUNT-1)17 | Nausea in 28% vs 8% (Zepbound label, 2 weight trials pooled); pancreatitis and gallstones17,45 | Mounjaro ₹3,02,810 in year 1 at MRP, then ₹3,35,153 a year (our sum)34 |
Separate studies: SOS was not randomised; the others were. Hospital prices are published ranges, not quotes. Medicine costs are 52 weekly doses at MRP on the label's dose steps, checked 5 October 2026.
What does bariatric surgery cost in India, compared with a year of medicine?
Eight private hospitals publish surgery prices of about ₹2 to 6 lakh, paid once; a year of medicine at a weight dose costs about ₹1 to 3.4 lakh.22,28,34,36
Most hospitals publish a range that varies by city, room type and operation. For laparoscopic sleeve gastrectomy, the published ranges are (hospitals in A to Z order):
- Apollo Hospitals (India-wide average): ₹2–4 lakh22
- Jammu Hospital, Jalandhar: ₹4 lakh29
- KIMS Hospitals (Hyderabad): ₹2.75–5 lakh27
- Lux Hospitals, Hyderabad: ₹2.5–3.7 lakh25
- Medicover Hospitals, Bengaluru: ₹3.5–5 lakh26
- PACE Hospitals, Hyderabad: ₹2.8–4.5 lakh24
- Yashoda Hospitals, Hyderabad: ₹3–5.5 lakh23
For gastric bypass, published figures run from ₹2 lakh to ₹6 lakh.22,23,24,27,28,29 Only Jammu Hospital lists what its price covers: the surgeon, anaesthesia, theatre, a standard stay and routine tests. Complications and an ICU stay cost more.29 Lifelong check-ups and vitamin supplements after surgery are extra costs too.43
Medicines cost every month they are taken. We count a year as 52 weekly doses, at MRPs checked on 5 October 2026:
- Wegovy (semaglutide), up to 2.4 mg. Novo Nordisk's MRPs run from ₹5,660 to ₹16,400 for 4 doses.30 On the label's 4-weekly dose steps, year 1 costs ₹1,83,210. Each further year at 2.4 mg costs ₹2,13,200 (our sums).30,31,32
- Mounjaro (tirzepatide), up to 15 mg. Pharmacy listings print ₹13,125 to ₹25,781 for a pen of 4 doses: the August 2025 launch MRP less the September 2025 GST cut.33,35 On the dose steps used in SURMOUNT-1, year 1 costs ₹3,02,810. Each further year at 15 mg costs ₹3,35,153 (our sums).34
- Generic semaglutide pens. Sun Pharma's 2.4 mg weight pen costs ₹2,000 a week at its top dose: ₹1,04,000 a year (our sum).36 Dr Reddy's diabetes pen costs about ₹4,200 for 4 doses of 1 mg, a diabetes dose rather than a weight dose.37
Natco's multi-dose vials are priced per vial: ₹1,290 to ₹1,750 a month.38 An endocrinologist visit adds a median ₹1,100 on Practo.39 Prices move too: Novo Nordisk cut Wegovy's prices in March 2026, as generics arrived.30
Guide 6 lists every brand's MRP. Our posts on what GLP‑1 medicines cost in India and semaglutide generics go further. A short Answer on how much bariatric surgery costs in India is planned.

Does health insurance pay for bariatric surgery or GLP-1 medicines in India?
Some policies we checked pay for obesity surgery only at a BMI of 40, or 35 with serious conditions. That wording comes from a 2019 IRDAI standard, withdrawn in 2024.40,48,49
IRDAI is India's insurance regulator. In 2019, it set standard wording for health-policy exclusions. Its obesity exclusion, "Excl06", pays for surgery only if all of these are true:40
- a doctor advised the surgery, and it follows clinical protocols;40
- the person is 18 or older;40
- the BMI is 40 or more, or 35 or more with one of these: heart-muscle disease from obesity (cardiomyopathy), coronary heart disease, severe sleep apnoea, or uncontrolled type 2 diabetes;40
- at the 35 level, less invasive ways of losing weight have already failed.40
IRDAI folded this wording into a 2020 master circular. A new master circular of 29 May 2024 replaced it. That circular sets no obesity exclusion, so insurers now write their own.48 Some still use the 2019 text. Current wordings from HDFC ERGO (my: Optima Secure) and Niva Bupa (ReAssure 2.0) carry it word for word.49 So check your own policy.
These cut-offs are higher than the surgeons' Asian cut-offs of 27.5 to 35.8,40 Policies can also set waiting periods for named procedures.
For medicines, one standard policy we opened excludes "treatment taken on outpatient basis". An OPD add-on from the same insurer gives up to 20% off prescription medicines.49 A September 2026 review from AIIMS Bhopal found that Indian insurance cover for outpatient medicines "remains nonexistent".41 Our post on whether health insurance covers GLP‑1 medicines in India goes further.
What are the risks of bariatric surgery?
The main risks are those of a major operation, a second operation later for some people, and lifelong vitamin needs.14,15,43
The LABS-1 study followed 4,776 first-time operations at 10 US centres in 2005–07.15 Within 30 days, 0.3% of bypass and banding patients had died. In all, 4.3% had a major problem, such as a blood clot or another procedure.15 Sleeves, now India's most common operation, were not in that figure.3,15 A 2022 statement by US and international surgeons puts death around the time of surgery at 0.03% to 0.2%.10
The UK's NHS lists blood clots, a leak from the stomach join and wound infection. It also lists a blocked gut, gallstones, and loose, excess skin.42
In the 10-year SLEEVEPASS trial in Finland, 16% of sleeve patients and 19% of bypass patients needed another operation.14 Inflammation of the food pipe from acid reflux (oesophagitis) was found in 31% after sleeve, against 7% after bypass.14
After surgery, the NHS says people take vitamin and mineral supplements, with follow-up and blood tests for life.43
What are the risks of GLP-1 medicines?
Stomach side effects are the most common. Pancreatitis is uncommon (up to 1 in 100 on the EU Wegovy and Mounjaro labels), and most weight returns after stopping.16,19,44,45
In STEP 1 (semaglutide 2.4 mg, the Wegovy dose), 7% stopped for side effects, against 3% on the dummy.16 Gallbladder problems, mostly gallstones, affected 2.6% vs 1.2%.16 In the Zepbound (tirzepatide) weight trials, 6.7% on 15 mg stopped for side effects, against 3.4%.17 Inflamed gallbladder (cholecystitis) affected 0.7% vs 0.2% across all doses.17
On the EU Wegovy label, gallstones are common (up to 1 in 10). On the EU Mounjaro label, they are uncommon (up to 1 in 100).44,45 Unlike surgery, these risks continue for as long as the medicine is taken. Guide 5 has the full list, and pancreatitis and gallstones have their own post.
Which one puts type 2 diabetes into remission?
Surgery studies report remission, each by its own definition; GLP‑1 weight trials kept people on the medicine, so they did not measure it.13,14,46
A 2021 expert consensus defines remission as an HbA1c below 6.5%, at least 3 months after stopping diabetes medicines.47 HbA1c is the 3-month average blood-sugar test (HbA1c explained). The surgery studies used other definitions:
- SOS (Sweden, not randomised): remission meant blood glucose below 110 mg/dL with no diabetes medicine. At 2 years, 72% of the surgery group met it, against 16% with usual care. At 15 years, it was 30% vs 7%, but fewer than half were still attending checks.46 Most of this group had older operations; 55 of 343 had a bypass.46
- STAMPEDE (US trial): after 5 years, HbA1c was 6.0% or lower in 29% after bypass and 23% after sleeve. That count includes people still on medicines. With medicines alone, it was 5%.13
- SLEEVEPASS (Finland trial): after 10 years, 26% (sleeve) and 33% (bypass) of those with diabetes were in remission. The trial used its own criteria.14
We found no GLP‑1 weight trial that stopped the medicine and measured remission. More in Ozempic and type 2 diabetes in India.
Questions people ask
Is bariatric surgery cheaper than GLP-1 medicines in the long run?
Surgery is paid once: about ₹2 to 6 lakh at the private hospitals we checked, plus lifelong follow-up.22,28,43 Medicines cost every year they are taken: ₹2,13,200 a year for Wegovy 2.4 mg at MRP (our sum). So ₹2 to 6 lakh equals about 1 to 3 years of Wegovy 2.4 mg.30 Prices of both change.
Does weight come back after bariatric surgery?
Some does. In the SOS study, gastric bypass gave 32% loss at its peak and 25% after 10 years.12
Does weight come back after stopping a GLP-1 medicine?
Mostly, yes. A year after stopping semaglutide 2.4 mg, STEP 1 volunteers had regained two-thirds of the weight they had lost.19 Guide 9 covers stopping.
Is bariatric surgery covered by health insurance in India?
It can be. Some policies we checked still use IRDAI's 2019 wording: surgery is paid at a BMI of 40 or more, or at 35 or more with certain serious conditions and failed earlier attempts.40,49 IRDAI withdrew that standard in May 2024, so each policy's own wording and waiting periods decide.48
What is the most common bariatric surgery in India?
Sleeve gastrectomy. It made up 48% of the 20,242 operations India's surgeons' society counted in 2018.3
Can you get pregnant after bariatric surgery or on a GLP-1?
After surgery, pregnancy is best avoided for 12 to 18 months.43 The Indian guideline says GLP‑1 medicines are not used in pregnancy or breastfeeding.11 Our post on GLP‑1s, pregnancy and contraception has more.
How we researched this
On 5 October 2026 we read the surgical guidelines (IFSO-APC 2011, OSSI 2020, ASMBS/IFSO 2022), the Indian obesity guideline (ESI 2025), the SOS, STAMPEDE, SLEEVEPASS, LABS-1, STEP 1 and SURMOUNT-1 reports, and the US and EU labels.
Surgery prices come from 8 private hospitals' cost pages, cited without links. We found no news report with an Indian surgery price. Medicine MRPs come from makers and dated news reports; Mounjaro's, and one Wegovy strength's, from two pharmacy listings that agree. Insurance wording comes from IRDAI's 2019 guideline, its 2020 and 2024 master circulars, and three current policy wordings.
We found no Indian study comparing surgery with GLP‑1 medicines, and no reliable count of Indian operations after 2018. Hospitals are named only for their published prices, in A to Z order, unranked.
Before you move on
- We found no reported trial comparing surgery with GLP‑1 medicines; the numbers come from separate studies.
- Surgery's risks centre on the operation; the medicines' risks last while they are taken.
- Surgery is a one-off cost of about ₹2 to 6 lakh; medicines cost every year.
- Some policies we checked still use IRDAI's 2019 wording for surgery (BMI 40, or 35 with conditions), withdrawn in 2024; check yours.
