The short answer

We found no Indian approval of any GLP‑1 medicine for PCOS (polycystic ovary syndrome). The 2023 international PCOS guideline says liraglutide or semaglutide could be considered for higher weight, with lifestyle change. Trials show modest weight loss and common stomach side effects. The guideline suggests metformin at a BMI of 25 or more.

Key takeaways

  • We found no Indian approval of any GLP‑1 medicine for PCOS; the launch indications for Mounjaro and Wegovy in India cover weight, diabetes and heart risk.22,23
  • The 2023 international PCOS guideline says liraglutide or semaglutide "could be considered" for higher weight in adults with PCOS, alongside lifestyle change, with contraception when pregnancy is possible.1
  • A 2026 review of 11 trials found GLP‑1 medicines lowered BMI by 1.4 points more than comparison treatments, at low certainty, with stomach side effects common.4
  • The same guideline says metformin "should be considered" in adults with PCOS and a BMI of 25 or more, and that it works about as well as lifestyle change.1
  • An ICMR-funded study of 8,993 Indian women aged 18 to 40 found PCOS in 7% to 20%, depending on which diagnostic criteria were used.2

How common is PCOS in Indian women, and how does weight fit in?

An Indian national study found PCOS in 7% to 20% of women aged 18 to 40, and 43% of those with PCOS had obesity.

PCOS (polycystic ovary syndrome) is a hormone condition. It can cause irregular periods, extra facial or body hair and trouble getting pregnant.1 In India, PCOD is often used as another name for it.28 A 2026 international consensus renamed it polyendocrine metabolic ovarian syndrome (PMOS).27 The international guideline puts it at 10% to 13% of women worldwide (Rotterdam criteria). It says PCOS may be more common in South East Asia.1

The largest Indian study was funded by the Indian Council of Medical Research (ICMR). It enrolled 8,993 women aged 18 to 40 across 5 zones of India, between 2018 and 2022.2 PCOS was found in 7.2% under the strictest criteria (NIH 1990). It was 13.6% under a middle set (AE‑PCOS) and 19.6% under the broadest (Rotterdam 2003).2

Weight was a large part of the picture. Of 1,224 women with PCOS, 529 (43.2%) had obesity and 245 (20.0%) had overweight, by the study's own Asian cut-offs.2

Fatty liver was found in 32.9%, and 91.9% had some blood-fat (lipid) problem.2 The guideline says women with PCOS carry a higher risk of type 2 diabetes "regardless of age and BMI".1

Indian cut-offs. The 2009 Indian consensus puts a healthy BMI at 18.0 to 22.9, overweight at 23 to 24.9 and obesity at 25 or more.3 Its waist cut-off for women is 80 cm (31.5 inches) (90 cm (35.4 inches) for men).3 For a woman 1.58 m tall, a BMI of 25 is about 62 kg (our arithmetic). Our BMI and waist tool uses the Indian cut-offs.

Is any GLP-1 medicine approved for PCOS in India?

We found no Indian approval of any GLP‑1 medicine for PCOS, and none of the 5 US labels we searched mentions it.

Before you go further
  • GLP‑1 medicines are prescription-only in India and cannot be bought over the counter.20
  • A government statement on 1 Apr 2026 said only endocrinologists, internal-medicine specialists and cardiologists can prescribe them.20 CDSCO expert-committee minutes attach a similar condition to some approvals (endocrinologist or internal-medicine specialist), but we found no rule covering every brand.21
  • All the risks of these medicines are in Guide 5: Side effects and risks.
  • Check for fakes and recalls in Guide 6.
  • This post describes evidence. It promises no result and is not a plan for anyone's treatment.

The Indian approvals name other conditions. Mounjaro (tirzepatide) launched in India in March 2025 for long-term weight management and type 2 diabetes.22 Its weight use covers a BMI of 30 or more, or 27 or more with a weight-related condition.22 Wegovy (semaglutide) launched in June 2025 for weight management and to lower the risk of heart attack and stroke.23 It later gained Indian approvals for teenagers aged 12 and over and for a scarring form of fatty liver (MASH).24 None of these reports lists PCOS as an approved use.22,23,24

We also text-searched 5 US labels: Wegovy, Ozempic, Saxenda, Mounjaro and Zepbound. None names PCOS.12,13,14,15,16 So when a woman with PCOS is prescribed one, PCOS itself is not the approved reason.

The prescriber statement matters here. Many women with PCOS see a gynaecologist. Gynaecologists are not among the 3 specialties named in the 1 Apr 2026 statement.20 Our Answer Do you need a prescription for GLP‑1 medicines in India? covers the prescriber rules.

What does the 2023 international PCOS guideline say about GLP-1 medicines?

It says liraglutide or semaglutide "could be considered" for higher weight in adults with PCOS, on top of lifestyle change and with contraception.

The 2023 International Evidence-based Guideline was backed by 39 professional and patient organisations covering 71 countries.1 It says lifestyle change "should be recommended for all women with PCOS".1 It also says a healthy lifestyle brings benefits "even in the absence of weight loss".1 It found no evidence that any one type of diet works better than another.1 Indian meal ideas are in What to eat on Ozempic or Mounjaro.

Its recommendation 4.5.1 covers anti-obesity medicines, "including liraglutide, semaglutide … and orlistat".1 It says they "could be considered, in addition to active lifestyle intervention, for the management of higher weight".1 That applies to adults with PCOS, "as per general population guidelines".1

The wording is cautious on purpose. In this guideline, "could" and "should consider" mark conditional recommendations. They are used where evidence was limited.1 This one is also a consensus recommendation, made where PCOS-specific evidence was not enough.1 The review behind it found 11 trials and called the data "very limited".5

Three practice points sit under it:1

  • Contraception. Doctors "should ensure concurrent effective contraception when pregnancy is possible", because "pregnancy safety data are lacking".1
  • Slow dose steps. Gradual increases are advised to reduce stomach side effects.1
  • The long view. The talk has to cover side effects and possible long-term use.1 It also covers "the high risk for weight regain after discontinuation, and the lack of long-term safety data".1

The guideline names liraglutide and semaglutide, not tirzepatide (Mounjaro).1 One of its 13 named authors declares speaker fees from Novo Nordisk, which makes semaglutide and liraglutide.1

India's own obesity guideline (ESI 2025) does not mention PCOS; we searched its full text.19 It says a doctor may consider medicines above a BMI of 27, or above 25 with a condition like diabetes.19 That is a threshold for a doctor's decision, not a test anyone passes. Guide 4: Should I consider a GLP‑1? covers it in full.

Where does metformin fit for PCOS, and does it help weight?

The guideline says metformin "should be considered" in adults with PCOS and a BMI of 25 or more; it works about as well as lifestyle change.

Metformin is a diabetes tablet that helps the body respond to insulin. The guideline recommends it for body measurements and metabolic results, "including insulin resistance, glucose, and lipid profiles".1 That is an evidence-based recommendation, unlike the GLP‑1 one, but still conditional. The guideline rates the certainty of its evidence "very low".1 Below a BMI of 25, it says metformin "may be considered", noting limited evidence.1

On weight, the guideline is modest. It says women "should be informed that metformin and active lifestyle intervention have similar efficacy".1 Our Answer Does metformin cause weight loss? covers this.

Metformin vs the pill. A 2024 review of 36 trials compared metformin with the combined contraceptive pill.10 The pill lowered male-type hormones (androgens) more. Metformin lowered insulin and triglycerides (a blood fat) more. For excess hair, the review found no difference between them.10 It said the choice "should be based on symptoms".10

Metformin has its own side effects. In US label trials, diarrhoea affected 53% on metformin against 12% on a dummy tablet (placebo).11 About 7% developed low vitamin B12 levels over 29 weeks.11 The PCOS guideline says B12 checks may be needed at higher risk, for example on a vegan diet.1 It says stomach effects depend on the dose and usually settle, and starting low helps.1 The US label carries a boxed warning for lactic acidosis, a rare but dangerous build-up of acid in the blood.11

The same US metformin label lists only type 2 diabetes as its use; it does not mention PCOS.11 How metformin compares with other options is in Guide 3: Your options.

What have trials of GLP-1 medicines in women with PCOS found?

They found modest weight loss over a few months, mostly in small trials of older GLP‑1 medicines; one small tirzepatide trial reported in 2026.

The newest review (2026) pooled 11 randomised trials.4 They tested liraglutide, exenatide, dulaglutide and beinaglutide, mostly for 12 to 32 weeks.4 None tested semaglutide or tirzepatide.4

Added to other treatment, GLP‑1 medicines cut BMI by 1.38 points more than the comparison.4 For a woman 1.58 m tall, that is about 3.4 kg (our arithmetic). The authors rated the certainty "low", and judged 8 of the 11 trials at high risk of bias.4

One placebo-controlled trial tested liraglutide 3 mg a day (the Saxenda dose) in 82 US women for 32 weeks.6,13 Weight fell 5.7% on liraglutide and 1.4% on placebo; stomach side effects hit 58.2% against 18.5%.6

Semaglutide has a smaller, weaker record. A 2025 Chinese trial gave 100 women metformin alone, or metformin plus semaglutide 1 mg a week, for 16 weeks.7 That dose is below Wegovy's 2.4 mg weekly top dose.12 The semaglutide group lost 6.1 kg on average, against 2.3 kg on metformin alone.7 Everyone knew which treatment they got (an open-label trial), and 20% did not finish.7

Tirzepatide now has a small trial in PCOS. A 2026 Chinese trial gave 60 women metformin alone, or metformin plus tirzepatide 5 mg a week, for 16 weeks.26 That dose is below Mounjaro's 15 mg top dose.14 The tirzepatide group lost 10.4 kg, against 1.7 kg on metformin alone.26 It was open-label, at one hospital, with a short follow-up; 56 of 60 finished.26 Nausea affected 7 of 30 on tirzepatide and 5 of 30 on metformin alone. One woman on tirzepatide left with gallbladder inflammation (cholecystitis).26

A larger placebo-controlled trial, PERIODS, began in Germany in December 2025 and aims to enrol 198 women.8 It expects to finish its main measurements in December 2028 (estimated).8

Do GLP-1 medicines help periods, androgens or pregnancy in PCOS?

The evidence is too thin to say: the 2026 review found it insufficient for periods and excess hair, and pregnancy data come from two small Chinese trials.

Periods and hair. The 2026 review said evidence was "insufficient to draw conclusion" on menstrual regularity and excess hair (hirsutism).4 The liraglutide trial did lower the free androgen index.6 In both Chinese trials, more women's cycles returned when semaglutide or tirzepatide was added to metformin.7,26

Pregnancy. In the semaglutide trial, semaglutide was stopped at 16 weeks. Everyone then took metformin alone until week 40.7 Natural pregnancies in that time were 35% in the group that had taken semaglutide, against 15%.7 In the tirzepatide trial, natural pregnancies from week 25 to 48 were 11 of 28 against 4 of 28 in the group that took metformin only.26 Both were small, open-label trials from one Chongqing hospital team under one trial registration, so they are not independent checks and show only a signal.7,26

For women trying to conceive, the guideline names a different first medicine. It says letrozole "should be the first-line pharmacological treatment" to trigger ovulation in PCOS.1 It also says excess weight affects pregnancy and live-birth rates after fertility treatment.1

GLP‑1 medicines have to stop before a planned pregnancy. The European Wegovy label says to stop semaglutide at least 2 months before.17 The European Mounjaro label says at least 1 month for tirzepatide.18 India's ESI guideline says contraception must continue for 3 months after stopping.19 The European labels say the effect on human fertility is unknown.17,18

The pill and Mounjaro. The guideline lists the combined pill as a treatment for irregular cycles and excess hair in PCOS.1 The US Mounjaro label says tirzepatide may make the pill less reliable.14 It advises switching to a non-pill method, or adding condoms, for 4 weeks after starting and after each dose increase.14 The European Wegovy label says semaglutide is not expected to reduce the pill's effect.17 The full detail is in Mounjaro, Ozempic, pregnancy and the pill.

What are the downsides for women with PCOS?

Stomach side effects are common, most of the weight tends to return after stopping, and there are no long-term safety data in PCOS.

Side effects. In the US Wegovy weight trials (semaglutide 2.4 mg), 44% had nausea, against 16% on placebo.12 In the PCOS trials, stomach symptoms "were frequently observed" with GLP‑1 medicines.4

Weight regain. In the STEP 1 extension, adults with higher weight and no diabetes (not a PCOS group) stopped Wegovy-dose semaglutide (2.4 mg) after 68 weeks.25 One year later they had regained two-thirds of the weight they lost.25 Guide 9: Stopping and the long term covers what happens next.

Eating and body image. The guideline says eating disorders and disordered eating should be considered in PCOS "regardless of weight", especially during weight management.1 It also warns about weight stigma in healthcare.1

Not every woman with PCOS has higher weight. The guideline's GLP‑1 recommendation is for "higher weight".1 For women who are not overweight, it puts the focus on healthy lifestyle and preventing weight gain.1

RisksThe risks, in brief
  • Stomach effects are common. Nausea in 44% on Wegovy 2.4 mg, against 16% on placebo, in the US label's weight trials.12
  • Pancreatitis. "Uncommon" on the EU Wegovy and Mounjaro labels (1 in 100 to 1 in 1,000 people).17,18
  • Gallstones. "Common" on the EU Wegovy label (1 in 10 to 1 in 100 people). In US Wegovy trials, 1.6% against 0.7% on placebo.12,17
  • Dehydration from vomiting or diarrhoea has led to kidney injury, the US Wegovy label says.12
  • Thyroid warning. Ruled out by the US label for anyone with a personal or family history of medullary thyroid cancer (MTC) or MEN 2; tell your doctor if this applies.12
  • Not in pregnancy. The US Wegovy label says weight loss offers no benefit in pregnancy and may harm the baby.12

Questions people ask

Is Ozempic approved for PCOS in India?

We found no Indian approval of any GLP‑1 medicine, Ozempic included, for PCOS.22,23 The US Ozempic label does not name PCOS either.16 The 2023 international PCOS guideline says semaglutide "could be considered" for higher weight in adults with PCOS.1

Does metformin help with weight loss in PCOS?

The 2023 international PCOS guideline says metformin and active lifestyle change "have similar efficacy".1 It says metformin "should be considered" in adults with PCOS and a BMI of 25 or more.1 Diarrhoea is its most common side effect.11

Can metformin and a GLP-1 medicine be taken together?

Two Chinese trials from the same hospital team gave them together, one with semaglutide (2025) and one with tirzepatide (2026).7,26 For people with diabetes, India's ESI guideline suggests reducing other stomach-upsetting diabetes medicines, such as metformin, when a GLP‑1 starts.19

Do GLP-1 medicines fix irregular periods in PCOS?

A 2026 review of 11 trials found the evidence "insufficient to draw conclusion" on menstrual regularity.4 Two small Chinese trials from the same hospital team reported more cycles returning when semaglutide or tirzepatide was added to metformin.7,26

Is Mounjaro used for PCOS?

A small trial has reported. In a 2026 Chinese trial of 60 women, tirzepatide 5 mg a week plus metformin cut more weight than metformin alone over 16 weeks.26 It was open-label and short.26 A larger German placebo-controlled trial expects to finish its main measurements in December 2028 (estimated).8 The 2023 PCOS guideline does not name tirzepatide.1

How we researched this

On 5 and 5 Oct 2026 we read the 2023 international PCOS guideline and India's ESI 2025 obesity guideline in full. We read the summaries of the ICMR study, four reviews and four individual studies. We text-searched 5 US GLP‑1 labels, a US metformin label and the European Wegovy and Mounjaro labels.

We found no public copy of the Indian labels, so label figures here are US or European and say so. We could not open the 2024 Indian gynaecologists' (FOGSI) PCOS recommendations, so they are not used. We left out small trials we could read only second-hand.

Before you move on

  • We found no Indian approval of any GLP‑1 medicine for PCOS itself.
  • Trials show modest short-term weight loss; evidence on periods, hair and pregnancy is weak.
